Study On The Theory And Clinical Application Of“Five Depressions”in Traditional Chinese Medicine

Purpose of researchThe theory of “five depressions” is one of the theories in traditional Chinese medicine(TCM).It has a wide range of applications in clinical diagnosis and treatment who has high research value.This study aims to sort out,summarize and discuss the source and flow of this theory,to explore and summarize its main contents,as well as to study the clinical application of it.Through the above studie,we hope to comb the theoretical system of “five depressions”,and clarify its theoretical connotation,in order to provide some reference for the study of the theory,which is helpful to enlighten the thinking of distinguishing and treating the symptoms.Research methods1 Literature retrieval methodThe ancient documents for this study mainly come from the fifth edition of “Chinese Medical Classic”,a giant ebook series of TCM published by China Association of TCM.We input the relevant keywords to search for the purpose of the data,then use the “National Joint Catalogue of Books on TCM”to find the data of successive dynasties related to the theory,verify and supplement the purpose of the data.We consult modern and contemporary related research papers in CNKI,Wanfang,Weipu and other network databases,integrate various studies and draw on each other’s experience.2 Literature research methodsThis paper explores the experience of ancient physicians and modern scholars on the theory and its clinical application,and then uses the research method of theoretical thinking to identify,deduce,analyze and conclude.3 Research contentsThis study discusses the theory of “five depressions” in TCM and its clinical application.The main body of the paper is divided into three parts:The first part explores the source and current of the theory.We sort out and study relevant materials in ancient books and literatures,and integrate the research results of representative medical experts of all dynasties on the theory.In view of the current situation of insufficient understanding of the theory,it is emphasized that it has important research significance and necessity.The second part summarizes the main contents of the theory.This part discusses the epistemological characteristics of the theory,analyzes the connotation and extension of the concept,and clarifies the differences and connections between related concepts.we discusse the concept,pathogenesis characteristics,the connotation of treatment and prevention,and focuse on the depression of five movements and six climates.The explanation of the theory provides the corresponding guidance and theoretical basis for clinical application..The third part is the study of ancient medical cases.This part makes statistics and analysis on the types of diseases,syndromes and common prescriptions and medicines of the ancient medical cases related to “five depressions”,clarifies the application rules of the theory,and expounds the clinical application of depression of five movements and six climates.The fourth part is about the relationship between the epidemic rules of SARS and the depression of five movements and six climates.By observing and analyzing the meteorological data of the years related to the epidemic,we can know the actual changes of five movements and six climates in the years.Based on the theory,the meteorological data and the actual characteristics of five movements and six climates in the years were analyzed,and the corresponding degree was analyzed.Then with the incidence and prevalence of SARS as a reference,the relationship between the epidemic characteristics of SARS and the depression of five movements and six climates was analyzed and expounded.Results and conclusionsThis study has clarified the research contents of the theory of “five depressions” through the collation and summary of the theoretical literature in the past dynasties.We analyze the rules of clinical application of ancient medical cases,and discusse the relationship between the epidemic rules of SARS and the depression of five movements and six climates.The systematic research is carried out from the system of “five depressions” theory.1 The origin and flow of the theory of “five depressions” in TCMThis part sorts out and studies the related traditional Chinese medicine literature from Qin,Han to Ming and Qing dynasties,systematically sorts out and summarizes the thoughts on the theory of “five depressions” and its application in the representative works of various doctors according to the historical process,and focuses on its innovative developments,thus clarifying the source and flow of the theory.In Qin and Han dynasties,doctors discussed the “five depressions” based on the theory of gasification,including two aspects of natural gasification and human gasification process.In Tang and Song dynasties,on the basis of inheriting the theory of “five depressions” in Huangdi Neijing,the physicians annotated the principle of treatment of “five depressions” for the first time and proposed specific methods for its treatment.In the aspect of the depression of five movements and six climates,doctors discussed its characteristics in specific years and probed the treatment from the etiology,pathogenesis and prescription.In the development of the theory in Jin and Yuan dynasties,doctors paid more attention to the expansion of the scope of the principle of treatment and the neodoxy of wood depressions and fire depressions.During the Ming and Qing dynasties,the theory of “five depressions” was innovated and developed,including the definition of the concept,which was regarded as the the five internal organs depressions,and was distinguished from other related concepts.The etiology and pathogenesis of the theory,from the depression of five movements and six climates gradually inclined to the internal organs’ own depressions.The wood depression and fire depression were emphasized to a greater extent.The research focus of many doctors lied in the development and clinical application of the treatment principles.They were flexible in their use of therapeutic principles,put forward the relevant prescription,so that the“five depression”clinical application system was becoming more and more mature.The theory had not been elaborated from the perspective of the depression of five movements and six climates.Some doctors elaborated on the mechanism and summarize the principle of it.They comprehended the principles of the treatment from the understanding of depression of five movements and six climates.There were also someone who put the application of the depression of five movements and six climates in the clinic,proposing treatment prescriptions.2 The main content of the theory of “five depressions” in TCMThis part expounds the epistemological characteristics of the theory of“five depression”,the theoretical basis of which is the gasification theory,the core concept is holistic thinking,and the thinking method is the theory of five elements.The concept of “five depressions” is summarized and as the human body system centered on the five internal organs with qi depression as the basic characteristics of the pathogenesis or disease.Its extension mainly includes tow aspects:the depression of five movements and six climates and the depression of organs themselves.In addition,the respective concepts of“five depressions” are summarized,such as wood depression means liver or gallbladder depression,soil depression means spleen or stomach depression,gold depression means lung or large intestine depression,fire depression means the heatt can not be dispersed,water depression means kidney and bladder depression.The etiology is mainly divided into two categories: external and internal injuries.The external injury is the depression of five movements and six climates,and the main cause of internal injury is emotional depression and viscera depression.The pathogenesis of “five depressions” is divided into two parts : basic pathogenesis and special pathogenesis.Its basic pathogenesis features are as follows:abnormal air movement is the normal course of this illness.Effects of generation and restriction among five elements,as well as“five depressions” can transform or merge fire depression are the changes in pathogenesis.Dysfunction of the viscera and pathological product block are its pathology.Virtual and real transformation is its pathogenesis transformation.For the special pathogenesis of the depression of five movements and six climates,it can be summarized as:the root cause is the disorder of four seasons,overcoming and retaliation as well as release of depression are its mechanism of action,abnormally strong six qi are its pathogenesis,fire depression is a main pathogenesis of febrile disease,latent depression(three-year epidemic)is an important agent of serious epidemics.In addition,the characteristics of the pathogenesis of “five depressions” are explained.For example,the main pathogenesis of wood depression is the liver’s drainage dysfunction and the formation of depression.The main pathogenesis of fire depression is that the internal heat can not be discharged.The main pathogenesis features of soil depression are spleen failure,dereliction of function,and the qi obstruction of spleen and stomach.The main pathogenesis of gold depression is the abnormal function of lung’s dispersion and descending,and the excessive depression of lung qi.The main pathogenesis of water depression is dysfunction of lung,spleen and kidney,water overflow transport disorder,and accumulation of retained fluid.For the connotation of the five depression principle of treatment,the basic connotation is to coordinate the rise and fall and to guide the situation accordingly,to support healthy qi and dispel pathogenic factor,fill the deficiency and reduce the sthenia of pathogenic factors,take measures according to the time,without breaking the rules of cold and heat.In addition to the above,we explain the connotation of respective rules and regulations of“five depressions.Wood depression lies in restoreing the function of liver drainage,fire depression lies in dispersing internal heat,complying with its upgoing tendency.Soil depression lies in depriving of blocks of spleen and stomach,to restore the function of transportation.Gold depression lies in dispelling the block of lung qi to resume the function of lung’s dispersion and descending,water depression lies in restricting water overflow to prevent it from flooding.The prevention of “five depressions” mainly includes predicting the characteristics of five movements and six climates in specific years by using five movements and six climates calculation method,predicting the possible occurrence of “five depressions”,and grasping the opportunity before the disease.According to the prevention and control principle of “restraining the depression qi,nourishing the original qi”,humans should take timely preventive measures to make up for its deficiency,reduce its excess,and make the qi in balance,nip in the bud.For the “five depression” caused by sojourn six qi rise and fall disorder,acupuncture can be used to preventing it.In terms of health care,humans should follow the changing rules of nature,adjust our emotions,have a balanced diet,follow the regular rules of daily life,work and rest,and avoid the invasion of external pathogenic factorss,so as to make the viscera and qi organs benefit and prevent depression and illness.3 Study on the ancient medical cases of the theory of “five depressions” in TCMThis part starts with relevant medical cases contained in ancient Chinese medical books,exploreing the application of the theory of “five depressions”in its clinical manifestations.We adopts the traditional literature analysis method to make a preliminary analysis from the disease and syndrome types,specific treatment methods,prescription and medication of “five depressions”.The results show that the disease of “five depressions” is widely distributed and the syndrome types have their own characteristics.Moreover,we elaborate the formula application rules of“five depressions”.There are few ancient medical records of depression of five movements and six climates.Through the study and analysis of the ancient medical cases,it is found that its value lies in two aspects.The first is to explain the pathogenesis and pathological mechanism of “five depressions” by using it.The second is to use its thinking to guide the prevention and treatment of “five depressions”.On the basis of the conventional treatment of“five depressions”,the characteristics of five movements and six climates in specific years are defined,and the effects of them on human pathogenesis,etiology and pathogenesis are further discussed.It can not only assist to verify the accuracy of pathogenesis analysis,helping to explore the essence of pathogenesis and guide clinical prescription to improve clinical efficacy,but also can take timely preventive measures according to the characteristics of five movements and six climates in specific years and keep in good health to prevent diseases.4 Study on the relationship between the prevalence of SARS and the depression of five movements and six climatesThis part discusses the relationship between SARS and the depression of five movements and six climates from the perspective of the epidemic and climatic characteristics of SARS.It is considered that the climate disorder of Gengchen year caused the SARS epidemic within three years.Shaoyang fire befallen Yangming dry gold in Renwu year,while the gold qi was depressed,so that SARS was breeded and onset of disease.In the time of the second qi of Guiwei year,the fire was depressed by cold and dampness,therefore,SARS outbroke epidemic.On the study of the relationship between SARS syndrome characteristics and depression of five movements and six climates,it is believed that SARS is related to the the gold depression caused by climate disorder of Gengchen year,and the fire depression by cold and dampness.The dryness and heat of gold depression are the latent pathogenic factors.Cold and dampness of fire depression are current pathogenic factors.The current pathogenic factor pestilence induced the latent pathogenic factors.SARS was presented the characteristics of cold and heat combined with dryness and dampness.The depression of five movements and six climates plays an important role in the analysis of the syndromes and pathogenesis of SARS.
