Objectives:Through randomized controlled clinical trials,objective evaluation of the electroacupuncture"four-point acupoint" combined with the clinical efficacy of acupuncture at the waist three needles.To compare the electroacupuncture "four-point acupoint" and the clinical therapeutic effect of acupuncture at the waist three needles and simple acupuncture at the waist for the treatment of lumbar disc herniation(LDH),and to find a conservative treatment with good therapeutic significance for the disease.To support clinical decision making.Methods:According to the Western Medicine LDH diagnostic criteria and TCM syndrome diagnostic criteria,60 cases of lumbar disc herniation were collected from two branch clinics of Singapore Public Hospital from April 2018 to March 2019,grouped by randomized controlled study methods.The patients were divided into(1)treatment group(electro-acupuncture group),(2)control group(conventional acupuncture group),30 cases each.The electroacupuncture group(the treatment group)used the electro-acupuncture "four-point acupoint"(two-sided Tai Chong and bilateral collaterals to connect a group of electro-acupuncture)combined with acupuncture at the waist three needles(Yao Yang Guan,Cheng Shan,and Feng Shi).Acupoints(Chinese medicine syndrome differentiation points).Each time the electroacupuncture"Si Guan",the electro-acupuncture parameter uses a continuous wave with a frequency of 20-30 Hz.The current intensity is suitable for the patient to withstand,and the needle is left for 30 minutes.The other acupuncture points and the acupuncture needles were used to hold the needles for 30 minutes,and the needles were used once(the sputum was transferred and inserted).The conventional acupuncture group(control group)was treated with acupuncture at the waist and three needles and acupuncture points.After the needle was sputum,the needle was left for 30 minutes,and the needle was used once(the sputum was transferred and inserted).The treatment time in the two groups was 2 times a week.10 treatments are a course of treatment,and after 1 course of treatment,rest for 3 days,a total of 2 courses of treatment.A comprehensive quantitative assessment was performed before and after each treatment in the two groups,using visual analogue scale,lumbar spine disease evaluation criteria score,modified Oswestry dysfunction index and objective assessment and analysis,and using patient serum index pairs.The two groups of patients were evaluated for subjective efficacy,and their clinical efficacy was evaluated.Statistical analysis was performed using SPSS 20.0 statistical software and related data.Results:1.Baseline analysis:This study included 60 patients diagnosed with lumbar disc herniation(30 in the treatment group and 30 in the control group).The baseline demographic index of the two groups was compared at the time of enrollment(P>0.05),and the comparability was good but not statistically different.The scores of the patients in the two groups(VAS,JOA,and modified ODI questionnaires)were compared at the time of enrollment(P>0.05),and the comparability was good but there was no statistical difference.The serological indexes of the two groups were compared at the time of enrollment(P>0.05),and the comparability was good but there was no statistical difference.Efficacy evaluation:The scores of VAS,JOA and modified ODI questionnaires were observed before and after treatment.The scores of VAS,JOA and modified ODI questionnaires before treatment were compared between the two groups.The scores of the paired samples were not statistically significant,indicating that the data of the two groups were comparable.After 10 weeks of treatment,there was a significant decrease in VAS,indicating that both treatments can improve the patient's waist pain.After 10 weeks of treatment,the scores of JOA were significantly increased,indicating that both treatments can improve the symptoms and functions of the patients,and both groups have obvious curative effect.An independent sample t-test was performed on the JOA score scale items between the two groups.The results showed that the improvement of total score and daily life treatment group was significantly better than the control group,and the difference was statistically significant(P<0.05).After 10 weeks of treatment,the treatment group and the control group showed a significant decrease in the modified ODI index and before treatment.The difference was statistically significant(P<0.05),indicating that both treatments can improve the patient's symptoms.And function.Before treatment,there were no significant differences in pain and inflammatory parameters between the two groups,and the comparability was good.After 10 weeks of treatment,both groups had improvement in pain and inflammatory parameters(P<0.05).The treatment group was better than the control group.It shows that the electroacupuncture "four-point" and acupuncture at the waist can effectively reduce the painful substances and reduce the low back pain and effectively reduce the inflammatory index to reduce the bone destruction of the patient.After the treatment,the results showed that the electroacupuncture group was better than the conventional acupuncture group.2.Clinical total effective rate:?JOA clinical total effective rate comparison:After the end of treatment,there was no statistically significant difference in clinical outcome between the two groups.In the treatment group,12 patients were cured,accounting for 40%,14 were effective,accounting for 46.67%,2 were effective,accounting for 6.67%,and the total effective rate was 93.33%.In the control group,2 were cured,accounting for 6.67%,and 5 were effective.16.67%,effective 18 people,accounting for 60%,the total effective rate was 83.33%.?The total effective rate of modified ODI:After the end of treatment,the clinical efficacy of the two groups was significantly different from P value<0.05.In the treatment group,10 patients were cured,accounting for 33.33%,16 were markedly effective,accounting for 53.33%,3 were effective,accounting for 10%,and the total effective rate was 96.67%.In the control group,5 patients were cured,accounting for 16.67%,and 9 were effective.30%,effective 12 people,accounting for 40%,the total effective rate is 86.67%.?The total effective rate of the two groups:after the end of treatment,there was no statistically significant difference in the clinical outcome between the two groups.The treatment group recovered 17 patients,accounting for 56.67%,10 effective,accounting for 33.33%,effective no one,the total effective rate was 90%;the control group recovered 8 patients,accounting for 26.37%,markedly effective,accounting for 26.67%,effective 5 people,accounting for 16.67%,the total effective rate was 83.33%.After 10 weeks of treatment,there was no statistical difference between the two groups(P>0.05).The effect of the treatment group was significantly better than that of the control group.3.Safety analysis:There were no serious adverse events in this study.In the electroacupunctu-e group,2(6.67%)subjects developed subcutaneous hematoma.The doctor used a cotton ball to pr-ss the pinhole hard,and the hematoma disappeared after a few minutes.In the conventional acupun-cture group,1(3.33%)of the subjects developed redness and itching after treatment,and the positi-on was in the acupuncture position.The symptoms of redness and itching disappeared without spec-ial treatment.There were no significant changes in vital signs before and after the test,indicating th-at the electroacupuncture and acupuncture treatments were safe and reliable during treatment and had no side effects.No one falls off during treatment.Conclusion:The electroacupuncture "four-point acupoint" combined with the waist three-needle treatment of lumbar disc herniation is effective,and the curative effect is better than the acupuncture group.The electroacupuncture "Siguan acupoint" combined with the obvious effect of waist three on the pain and inflammation of LDH also improved the quality of life of patients.Adding the electroacupuncture at the four points will make the curative effect more advantageous.The advantages of electroacupuncture have fewer side effects than safety.Electroacupuncture is more effective in eliminating pain and preventing recurrence and improving and restoring back pain and quality of life caused by lumbar disc herniation.
