Objective: to explore the credibility of infrared thermal fault and temperature threshold method combined with acupoint thermal sensitivity detection,and to provide scientific basis for establishing a new optimal acupoint thermal sensitivity detection scheme in acupuncture and moxibustion clinic.Methods:1.In accord with standard of knee osteoarthritis(swelling)in the diagnosis of 100 cases of patients,according to the thermal acupuncture acupoints are the law judge standard,thermal of high incidence of all patients xue hai,nei xi yan acupuncture point,Yin ling quan to detect a total of 300 point time,according to detect whether acupuncture points in the process of thermal phenomena occur,all points can be divided into thermal acupoints group and non thermal acupoints group,moxibustion probe(24 hours)after the application of infrared scanning technology to collect each of acupoints,infrared image contrast analysis of thermal group respectively with the thermal infrared temperature differences between groups of acupoints.Infrared temperature values of three groups of acupoints were used to determine the criteria for judging thermal acupoints by the infrared method.2.The above will comply with knee osteoarthritis(swelling)diagnostic criteria of 100 cases of patients,in the sense of moxibustion method,infrared method after the test(24 hours),using quantitative analyzer for all patients felt thermal high incidence of xue hai,nei xi yan acupuncture point,Yin ling quan of thermal sensation and thermal pain,heat resistant to pain threshold,contrast analysis of thermal heat sensation between groups with and without thermal gas,thermal pain,heat resistant to pain threshold differences.The temperature threshold of three groups of acupoints was used to determine the criteria for judging thermal acupoints by the temperature threshold method.3.In this study defined temperature threshold method to determine thermal infrared method and the standard values of acupoints,the research of knee osteoarthritis(swelling)xue hai,nei xi yan acupuncture point,Yin ling qaun of infrared temperature threshold and can be divided into thermal and thermal group,with moxibustion method as the gold standard,infrared method and temperature threshold method to compare the sensitivity,specificity and accuracy,and applied to MEDCALC method compared with the method of infrared temperature threshold method two kinds of the credibility of the test method.4.Logistic regression analysis was used to establish the combined variables of infrared and temperature threshold,and the combined variables were compared with the moxibustion method to analyze their sensitivity,specificity and accuracy.The value of combined variable,infrared method and temperature threshold method for judging thermal acupoints were compared.Results:1.The baseline data of the thermal-sensitive and non-thermal-sensitive acupoints of xue hai,nei xi yan and yin ling quan group were compared.2.By the method of infrared detection,thermal and thermal group xue hai,nei xi yan acupuncture point,Yin ling quan,in comparison with the temperature of p ? 0.05,statistically significant differences,prompt thermal state and thermal state xue hai,nei xi yan acupuncture point,Yin ling quan there are differences between the infrared temperature characteristics,acupuncture point for thermal state how it showed high temperature properties.3.The temperature threshold method was used to detect the thermal sensation threshold,thermal pain threshold and thermal pain threshold temperature of xue hai point,nei xi yan point and yin ling quan point in the thermally sensitive group and non-thermally sensitive group.The p values were all less than 0.05,showing statistically significant differences among the groups.It is suggested that the temperature threshold characteristics of xue hai point,nei xi yan point and yin ling quan point are different between the patients with knee osteoarthritis(swelling type)with thermal sensitivity and non-thermal sensitivity.4.The sensitivity,specificity and accuracy of the thermally sensitive infrared moxibustion at thenei xi yan points were 75.6%,53.8% and 70.0%,respectively.The sensitivity,specificity and accuracy were 67.5%,69.2% and 68.0%,respectively.The sensitivity,specificity and accuracy were 64.8%,84.6% and 70.0%,respectively.The sensitivity,specificity and accuracy were 97.2%,53.8% and 86.0%,respectively.Compared with that of the temperature threshold method,p > 0.05 showed that the difference had no statistical significance.The results indicate that there is no difference between infrared detection and temperature threshold detection in detecting thermal sensitive knee osteoarthritis(swelling type).The sensitivity,specificity and accuracy were 50.0%,83.3% and 58.0%,respectively.The sensitivity,specificity and accuracy of thermal threshold detection and moxibustion were 42.1%,83.3% and 52.0%,respectively.The sensitivity,specificity and accuracy were 60.5%,83.3% and 66.0%,respectively.The sensitivity,specificity and accuracy were 57.8%,83.3% and 64.0%,respectively.Comparing the infrared method and the temperature threshold method,the infrared method and the thermal threshold p >0.05,compared with that of the thermal pain and thermal pain tolerance p >0.05,the difference has no statistical significance.It indicates that there is no difference between infrared detection method and temperature threshold detection method in detecting thermal sensitive knee osteoarthritis(swelling type)xue hai acupoint.The sensitivity,specificity and accuracy were 87.1%,45.4% and 78.0%,respectively.The sensitivity,specificity and accuracy were 43.5%,81.8% and 52.0%,respectively.The sensitivity,specificity and accuracy were 82.1%,72.7% and 80.0%,respectively.The sensitivity,specificity and accuracy were 58.9%,81.8% and 64.0%,respectively.The comparison between the thermally-sensitive infrared detection method and that of the temperature threshold showed that p > 0.05.The results indicate that there is no difference between infrared detection method and temperature threshold detection method in detecting thermal sensitive knee osteoarthritis(swelling type)in yin ling quan point.Compared with moxibustion,the sensitivity,specificity and accuracy of the combined method were 85.3%,92.1% and 82.0% respectively.The areas under ROC curves of combined detection,infrared,thermal sensation and thermal pain tolerance were 0.8794,0.7630,0.6943 and 0.7498,respectively.Chi-square test shows that combined detection method is less than 0.05 compared with infrared method,thermal sensation and thermal pain tolerance,which is statistically significant.It is indicated that the combined application of the two methods is of higher value in judging the thermal acupoints.Conclusion:1.Both the infrared thermal fault method and the temperature threshold method can objectively detect the thermal sensitivity of acupoints to a certain extent?2.The combined application of infrared thermal tomography and temperature threshold method can significantly improve the sensitivity,specificity and accuracy of acupoint thermal sensitivity detection,and provide scientific basis for establishing a new objective scheme for clinical thermal acupoints detection.
