Aims: To observe the effects and possible mechanism of polygonatum sibiricum and resistant starch(RS)on early diabetic nephropathy(DN)through animal experiments;To investigate the best processing technology of high resistant starch and polygonatum sibiricum cookies,and observe the effect of dietary intervention on patients with early diabetic nephropathy through population studies.Methods: 1.To construct DN rat model and observe the effects of RS,polygonatum sibiricum and combined action on early DN rats.2.We confirmed the optimum formula and technology for making high RS and polygonatum sibiricum cookies through single factor experiment and orthogonal experimental design.3.To explore the clinical efficacy of high RS and polygonatum sibiricum cookies in early DN patients by a single-center clinical trial.Results: 1.Animal experiment: After 4 weeks,the levels of FBG decreased in all intervention groups except the combined low dose group.The levels of TC and LDL-C in polygonatum,RS and the combined group all decreased in varying degrees.Compared with the model group,the serum GSP,TNF-? and CRP levels in polygonatum sibiricum combined with RS group decreased significantly(P<0.05).2.The best processing technology of high RS and polygonatum sibiricum cookies is as follows: base on 100% of low gluten flour,then adds 28% of RS,40% of peanut oil,18% of xylitol,3.5% of baking powder,24% of polygonatum sibiricum,10% of egg and 30% of water.The surface baking temperature is 170 ?,primer temperature is 150 ?and baking time is 20-25 min.3.Population experiment: After intervention,the waist circumference,hip circumference,FBG and Hb A1 c of the two groups decreased,and Hb A1 c of the experimental group all decreased significantly(P<0.05).The m Alb,ACR and MDA all showed an rising trend,and the increase of the experimental group was less than control group.The urinary ?2 microglobulin of experimental group was on the decline;the SOD levels of the two groups were significantly increased(P<0.05).The TNF-?,IL-6 and CRP level were all decreased in the experimental group,and IL-6 was significantly decreased(P<0.05).Many TCM symptoms of the experimental group were significantly improved,and the effective rate was significantly higher than control group(P<0.05).Conclusions: 1.Polygonatum combined with RS can significantly reduce the levels of GSP,TNF-? and CRP in early DN rats,suggesting that it has obvious effect in controlling blood glucose and reducing inflammation level.QRT-PCR and Western blot results showed that Polygonatum and RS could not significantly improve the expression of ECM and related proteins in kidneys of early DN rats,which may be related to the shorter intervention time.2.In this study,a nutritional biscuit with low calorie,low protein,low GI and high RS was prepared by optimizing the formula and process.3.High RS and polygonatum sibiricum cookies can significantly reduce the levels of Hb A1 c and IL-6 in early DN patients,and increase the level of serum T-SOD.At the same time,it can improve the symptoms of traditional Chinese medicine and delay the increase of urinary microalbumin,which may be related to reducing inflammatory reaction and oxidative stress.
