BackgroundHyperlipemia also known as dyslipidemia,is a lipid metabolic disease characterized by elevated total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL-C)and/or low high density lipoprotein cholesterol(HDL-C)in plasma due to abnormal lipid metabolism and operation caused by various reasons.It is an extremely important risk factor for atherosclerosis in important target organs such as heart and brain.Among them,dyslipidemia characterized by elevated LDL-C is the key to the formation of atherosclerosis.Currently,statins are recognized as clinically effective lipid-lowering drugs.However,with the wide spread of their clinical application,adverse reactions such as elevated serum transaminase,liver and kidney damage,myotoxicity and other adverse reactions caused by large dose,high intensity and irrational drug use have been paid close attention to clinically.In addition,in the treatment of multiple diseases and combined medication,its safety is also questioned.In the clinical practice of traditional Chinese medicine,acupuncture and moxibustion therapy is an effective method to treat hyperlipidemia.Based on the above considerations,we have screened and optimized the programs for treating hyperlipidemia in recent years,and conducted random control tests on the basis of following evidence-based medicine,hoping to provide objective basis for the effectiveness of acupuncture on hyperlipidemia,so that acupuncture on hyperlipidemia can be widely popularized in clinical practice.Objective1.Confirm the effectiveness of electroacupuncture therapy and acupoint catgut embedding therapy in treating hyperlipidemia;2.To evaluate the characteristics and advantages of different therapeutic schemes in treating hyperlipidemia;3.To explore the relationship between lipid metabolism and insulin resistance and the effect of electroacupuncture and catgut embedding therapy on it.Methods:Ninety eligible patients with hyperlipidemia admitted to the acupuncture clinic of the first affiliated hospital of Guangzhou university of traditional Chinese medicine from may 1,2017 to June 1,2018 and volunteers who meet the standardswere selected and randomly divided into three groups according to the random number table method,including education group(30 cases),electroacupuncture group(30 cases)and acupoint catgut embedding group(30 cases).The propaganda and education group was given food energy manual and general diet therapy propaganda and education,with regular follow-up guidance once every 2 weeks.The electroacupuncture group was treated with acupuncture at Zhongwan,Dai Mai(double),Neiguan(double),Zusanli(double)and Sanyinjiao(double).On this basis,G-6805 ? electro acupuncture instrument was selected,with density wave frequency of 2 Hz-10 Hz-100 Hz,30 minutes each time and twice a week.Catgut embedding at acupoint group was treated with catgut embedding at acupoint with electro acupuncture group.Once a week.The above three groups of treatment,one course of treatment per week,a total of 8 courses of treatment.The differences of body weight,BMI,total cholesterol,triglyceride,low density lipoprotein and high density lipoprotein,fasting insulin,fasting blood glucose,insulin resistance index between the three groups before and after treatment and the scores of clinical observation table of TCM symptoms were compared to evaluate the curative effect.ResultsIn this study,90 cases of treatment were completed,1 case was lost in missionary group and 1 case was lost in acupoint catgut embedding group.1.The gender,age,course of disease,blood test components and quantitative scores of TCM symptoms of the three groups of patients before treatment have no statistical significance and are comparable.2.Weight comparisonThe weight of the patients in the three groups changed to different degrees after treatment,and there was no statistical difference before and after treatment in the education group(p>0.05),while there was a statistical difference before and after treatment in the electro acupuncture group and the acupoint catgut embedding group(p<0.05).Compared between groups,electroacupuncture group and acupoint catgut embedding group are superior to propaganda and education group(p<0.05),and there is no statistical difference between electroacupuncture group and acupoint catgut embedding group(p>0.05).3.waist circumference comparisonThe waist circumference of the patients in the three groups changed to different degrees after treatment,and there was no statistical difference before and after treatment in the education group(p>0.05),while there was statistical difference before and after treatment in the electroacupuncture group and the acupoint catgut embedding group(p<0.01,p<0.05).Compared between groups,electro acupuncture group and acupoint catgut embedding group are superior to propaganda and education group(p<0.05,p<0.01),and there is no statistical difference between electroacupuncture group and acupoint catgut embedding group(p>0.05).4.BMI comparisonThe BMI of the three groups of patients changed in different degrees after treatment,among which there was no statistical difference before and after treatment in the education group(p>0.05),while there was statistical difference before and after treatment in the electroacupuncture group and the acupoint catgut embedding group(p<0.01).Compared between groups,electroacupuncture group and acupoint catgut embedding group are superior to propaganda and education group(p<0.05),and there is no statistical difference between electroacupuncture group and acupoint catgut embedding group(p>0.05).5.Comparison of serum TC contentTC of the three groups of patients changed to different degrees after treatment,and the difference before and after treatment was statistically significant(p<0.01).Compared between groups,electroacupuncture group and acupoint catgut embedding group are superior to propaganda and education group(p<0.05,p<0.01),and there is no statistical difference between electro acupuncture group and acupoint catgut embedding group(p>0.05).6.Comparison of serum TG contentThe TG of the three groups of patients changed in different degrees after treatment,and the difference before and after treatment was statistically significant(p<0.01).Compared between groups,electroacupuncture group and acupoint catgut embedding group are superior to propaganda and education group(p<0.05,p<0.01),and there is no statistical difference between electro acupuncture group and acupoint catgut embedding group(p>0.05).7.comparison of serum LDL-C contentThe LDL-C of the three groups of patients changed to different degrees after treatment,and the difference before and after treatment was statistically significant(p<0.01).Compared between groups,electroacupuncture group and acupoint catgut embedding group are superior to propaganda and education group(p<0.05,p<0.01),and acupoint catgut embedding group is superior to electroacupuncture group(p<0.05).8.comparison of serum HDL-C contentAfter treatment,LDL-C in the three groups of patients changed to different degrees,among which there was no significant difference before and after treatment in the education group(p>0.05),while there was significant difference before and after treatment in the electroacupuncture group and acupoint catgut embedding group(p<0.01).Compared between groups,catgut embedding at acupoint is superior to propaganda and education group(p<0.05),catgut embedding at acupoint is superior to electroacupuncture group(p<0.05),and there is no significant difference between propaganda and education group and electroacupuncture group(p>0.05).9.Fasting blood glucose content comparisonThe fasting blood glucose of the three groups of patients changed to different degrees after treatment.there was no statistical difference between the propaganda and education group and the electroacupuncture group before and after treatment for fasting blood glucose reduction(p>0.05),while there was a statistical difference between the acupoint catgut embedding group before and after treatment for fasting blood glucose reduction(p<0.01).Compared between groups,electroacupuncture group and acupoint catgut embedding group are superior to propaganda and education group(p<0.01),and there is no statistical difference between electroacupuncture group and acupoint catgut embedding group(p>0.05).10.Fasting insulin content comparisonThe fasting insulin of the three groups changed to different degrees after treatment,and the difference before and after treatment was statistically significant(p<0.01).The electroacupuncture group and the acupoint catgut embedding group are superior to the propaganda and education group(p<0.05,p<0.01).there is no statistical difference between the electroacupuncture group and the acupoint catgut embedding group(p>0.05).11.Comparison of insulin resistance indexThe insulin resistance index of the three groups of patients changed to different degrees after treatment,and the difference before and after treatment was statistically significant(p<0.01).The electro acupuncture group and the acupoint catgut embedding group are superior to the propaganda and education group(p<0.01),and there is no statistical difference between the electro acupuncture group and the acupoint catgut embedding group(p>0.05).12.Comparison of clinical efficacyThe total effective rate in the propaganda and education group was 60.0%,the total effective rate in the electroacupuncture group was 83.3%,and the total effective rate in the acupoint catgut embedding group was 90.0%.there were statistical differences among the three groups.the curative effects in the electroacupuncture group and the acupoint catgut embedding group were better than those in the propaganda and education group(p<0.05,p<0.01).Conclusion1.Education,electroacupuncture and acupoint catgut embedding therapy under the guidance of Jin Sanzhen can effectively improve dyslipidemia in patients with hyperlipidemia,with reliable curative effect and no special side effects.However,Jin Sanzhen acupoint catgut embedding therapy is the best in improving the clinical efficacy of TCM symptoms.2.By comparing different schemes to interfere with hyperlipidemia,it provides clinical basis for Jin Sanzhen combined with acupoint catgut embedding therapy to treat hyperlipidemia.3.The application of Jin Sanzhen acupoint catgut embedding therapy for hyperlipidemia can not only regulate lipid metabolism disorder but also improve insulin resistance.
