| Objective:The incidence of dementia has increased year by year.The main features of cognitive dysfunction and memory loss have seriously affected the daily life of patients and increased the burden on families and society.At present,there is still no effective treatment for the treatment of dementia.At present,there are mainly drugs for improving cognition and some non-drug therapy to improve the quality of life of patients.Although the treatment methods are diverse,the effects are different.Acupuncture as a traditional Chinese medicine therapy has a good curative effect in the treatment of dementia from the clinical observation.However,there is still no systematic bibliometric analysis of acupuncture treatment of dementia,exploring the current research status,the selection of acupoints and selection Through regularity and lack of dialectical acupuncture treatment for dementia of a certain type of syndrome,this study explores the current research status of acupuncture treatment of dementia through bibliometric analysis,and selects the law of selection of acupoints to provide a theoretical basis for subsequent clinical application and research;At the same time,acupuncture therapy was used to refer to the acupoints of bibliometric analysis.Acupuncture treatment was applied to the treatment of senile dementia with blood stasis,and its effect on cognitive function and TCM symptom score was observed to evaluate its clinical efficacy.Method:Bibliometrics section:Using bibliometric analysis,through Medline(ovid),Embase(ovid),Cochrane library,China Knowledge Network(CNKI),Chongqing Weipu(vip),Wanfang,China Biomedical Literature Service System(CBM)seven Chinese and English databases A literature search on acupuncture treatment of dementia,a randomized controlled trial of reading screening,and a literature analysis of the literature(year publication,journal,journal type,first author unit,fund type)),clinical trial research feature analysis(acupuncture method,acupoint selection,selection law and detection index),and finally statistical analysis,inductive literature characteristics and selection of acupoints and selection rules.Clinical research section:Sixty patients with senile dementia with internal stagnation of blood stasis were randomly divided into acupuncture treatment group and drug control group by a randomized controlled trial(envelope method).The acupuncture treatment group was treated with Zhisan needle as the main point.According to the selection of acupoints and dialectical acupoints,once a day acupuncture treatment,continuous treatment for 4 weeks,the drug control group was treated with acetylcholinesterase inhibitor Ariens 5mg once daily for 4 weeks,the two groups observed the simple ensure the smooth clinical trials.Carry outFrom the clinical trial studies included in the analysis,most of the journals included are domestic journals,and international journals of the SCI type are less included;From the information distribution of the first author unit,it can be seen that the current clinical trial research on acupuncture treatment of dementia is mainly distributed in Chinese medicine hospitals and their affiliated hospitals,including Tianjin University of Traditional Chinese Medicine,Guangzhou University of Traditional Chinese Medicine,Heilongjiang University of Chinese Medicine,Anhui University of Traditional Chinese Medicine has a lot of research in this area;2 analysis of clinical trial research characteristicsFrom the types of subjects included,mainly vascular dementia and Alzheimer's disease,other types of dementia such as Lewy body dementia,frontotemporal dementia and other acupuncture studies in this area are rarely involved.From the selection of acupoints,it can be found that the high-frequency acupoints selected for acupuncture treatment of dementia are Baihui acupoints,followed by Sishencong,Shenting,Fengchi,and Zusanli.Guanxue,Shuigou,Sanyinjiao,Taixi and Fenglong;From the frequency distribution of meridians selected by the researchers,it can be found that the selection of Du Meridian is the high-frequency selection of meridians for acupuncture treatment of dementia,followed by Qi jing,foot Shaoyang bile,foot Yangming stomach,foot Shaoyin spleen,foot Shaoyin kidney,hand sputum Yinxinbao,hand Shaoyin heart,Renmai,foot sun bladder,foot sputum yin liver,hand Yangming large intestine,hand Shaoyang San Jiaojing,hand sun small intestine,hand Taiyin lungs.Clinical research part of acupuncture treatment of senile dementia with internal stasis1.There were no significant differences in age,gender,and condition between the two groups(P>0.05),indicating that there was good comparability between the two groups.There was no significant difference in the overall cognitive mmse score between the two groups before treatment(P>0.05).There was a difference between the two groups after treatment and before treatment.There was a statistical difference between the acupuncture treatment group and the control group(P<0.05);There was no significant difference between the two groups in the ADAS-cog scores before treatment(P>0.05).After treatment,the two groups were compared with the pre-treatment scores.The acupuncture group had lower ADAS-cog scores than the drug control group.The difference was statistically significant(P<0.05);4.There were no significant differences in TCM syndrome scores between the two groups.After treatment,the acupuncture treatment group was called the control group in reducing the TCM symptom score,and the difference was statistically significant(P<0.05).5.Comparison of clinical efficacy,the overall effective rate of acupuncture treatment group in the treatment of dementia was 66.7%,and the drug group was 60%,which had a certain therapeutic effect,but the difference between the two groups was not obvious.Conclusion:The above research summarizes the clinical research status of acupuncture in the treatment of dementia and the current research on acupuncture treatment of dementia and the law of selection,which has certain guiding significance for the clinical study of acupuncture treatment of dementia.Follow-up studies on acupuncture treatment of dementia provide theoretical basis and reference point selection.On the other hand,through clinical trials,it is further proved that acupuncture can effectively improve the overall cognitive function of patients with senile dementia with stagnation of blood stasis,slow down cognitive impairment,improve TCM syndromes and improve patients' quality of life.Clinical evidence is provided for the treatment of stagnation of blood stasis type dementia. |
