| Objective1.The academic dissertation analyzed and excavated the papers of related hormone-dependent dermatitis in the past 5 years,will analyze the related treatment plan.For example,western medicine treatment plan,tradition Chinese medicine treatment plan will be all contained in this dissertation.Traditional Chinese treatment scheme,the dialectical classification of Chinese medicine,the characteristics of the Chinese herbal medicine which used in these papers,types of medication used incorrectly and so on will be all analyzed and evaluated.These data will provide evidence for further clinical trials and laboratory test and so on.2.A randomized controlled clinical study will be conducted between liquid nitrogen skin rub combined with oral Chinese medicine treatment and Tacrolimus on the skin combined with oral Chinese medicine.The effect of this study will give the data support and basis for the further laboratory research.3.The laboratory investigation of liquid nitrogen rub combined with traditional Chinese medicine by comparison with Tacrolimus combined with traditional Chinese medicine on the KM mouse will be carried out to give us the further proof of the validity of my reasoning.The changes of inflammatory cells will be observed by HE stains pathological images.We will find more or little lymphocyte and tissue edema under the microscope.4.Through analysis and discussion,we will get the reasonability by using liquid nitrogen rubbing combined with oral Chinese medicine to treat facial hormone dependent dermatitis.We will be able to point out the advantages and disadvantages of this treatment regimen.And we will therefore make more rational choice of treatment.Methods1.Documents research:Searched the literature on China National Knowledge Infrastructure(CNKI)from 2012-2017 about hormone dependent dermatitis,we found morn than 290 papers on clinical observation.TCM treatment program and western medicine treatment program and combined treatment of Chinese and western medicine were all found.We found these words in these papers and input these words to the TXT files,these words contained the name of Chinese herbal medicine,symptoms,syndrome of TCM,therapeutic principles and so on.Then,the above documents were calculated with software.Next step,we re-enter the output of the software into the Microsoft office Excel document and sorted.In the process of data consolidation,we use data analysis and SPSS 19.0 and other software.In the end,we saw the results in data table on the Microsoft office Excel 2007.2.Clinical research:People involved in the experiment were all the patients of our department and with hormone-dependent dermatitis.These patients came in our department from March 2015 to March 2017.We recruited subjects accorded to our plan and patient' s symptoms.According to the related requirements of clinical trials,we consider case withdrawal and loss of access,so we recruited 62 patients.All the patients were divided into two groups in order of recruitment,one group was given treatment of liquid nitrogen and traditional Chinese medicine(Group A),and the other one was given tacrolimus and traditional Chinese medicine(Group B).Each group had 31 patients.The ratio of the two groups was 1:1.The Chinese medicine was called Shilancao mixture.Therapeutic schedule:Treatment of liquid nitrogen and traditional Chinese medicine(Group A):Treatment of liquid nitrogen and traditional Chinese medicine were all given to patients in this group.Liquid nitrogen scrub treatment means used liquid nitrogen to reduce skin temperature to reduce inflammation,and frost 3 times per site,at the same time the patient would feel a little sting.In the end,at the local site we could see the color of the skin turnned to red,local itch sensation was replaced by swelling tissue.Freeze treatment was given one time a week,and 7 times would be finished for this treatment.At the same time,the Chinese herbal medicine which was called Shilancaomixture was also given to these patients.The main herbs were Shigao 30g,Banlangen 30g,Gancao 6g,Longdan 10,Cheqianzi 30g,Huangqin 10g,Shengdi 30g,Danpi 10g,Huashi 20g,Chishao 10g,Machixian 30g,and the above list of Chinese herbs and weight was one patient' s daily dose.All the herbs were given to our subsidiary pharmaceutical factory to make a 100ml bottle of mixture.Every patient got one bottle of Shilancao mixture a day,and they should take 50ml orally after breakfast and take 50ml after dinner.The mixture was taken orally by the patient every day for 7 weeks until the end of the animal experiment.The group for Tacrolimus and traditional Chinese medicine(Group B):Tacrolimus was given to one patient 2 times every day for 7 weeks until the end of the experiment.At the same time,Shilancao mixture was also given to these patients of Group B.The way to take the Shilancao mixture was administered the same way to group A,and the same dose and times.The also should take 100ml bottle of mixture one day and they should take 50ml orally after breakfast and take 50ml after dinner.The mixture was taken orally by the patient every day for 7 weeks until the end of the animal experiment.All the patients were given 7 weeks' treatment,follow-up 4 weeks after treatment to observe the effect.The skin lesions such as erythema,papule,bubble,pruritus was recorded at the following four occasions:the occasion before the treatment,the occasion aftertreatment four weeks,the occasion after the treatment for 7 weeks,the occasion at four weeks after treatment.The results were statistically analyzed by SPSS19.0.3 Animal experimental studyThis animal experimental study was done in two steps.The two steps involved three experiments.The first step:40 KM mouse was bought form Laboratory animal center and they were all 9 weeks old.The weight of these mouse was 26-28g.The goal of this experiment was to find the correct regent for choosing in animal models.The second step:60 KM mouse was bought form Laboratory animal center and they were all 9 weeks old.The weight of these mouse was 26-28g.The experiment' objective was to proof the liquid nitrogen freezing combined Chinese medicine was correct in the treatment of hormone dependent dermatitis.After apply medication to the skin 15 days and stop medication for 3 days,5 mice was chose from these 60 mouse to be rubbed the liquid nitrogen once and be kill one day after this day,and these five mouse was killed all.The experiment 3:All the remaining 55 mice was involved in this experiment.According to the sequence of the catch,this mouse was divided into 3 groups,and the first group had 20 mice,and they were giving Liquid nitrogen freezing and Chinese herb(A group).The second group was giving Tacrolimus and Chinese herb(B group),and 20 mice in this group.The third group was giving no treatment and 15 mice in this group.Experimental scheme:The first experiment:Firstly,the 40 mice were divided into four groups.Every group have 10 mice.The four groups were respectively called group A,group B,group C,and group D.An area of 1.5cm square was selected on the back of every mice.Then shaved the hair in these area to a length of 0.5cm.4 kinds of reagents were applied to the skin.These reagents were 0.05%Clobetasol propionate for group A,0.05%Clobetasol propionate tincture for group B,0.05%Clobetasol propionate for group C,75%ethyl alcohol for group D.Then the area of skin damage used for the experiment of the mice in the group A was painted with these reagent,2 times one day and stopped for 15 days,then observed 3 days.Red spots and other reactions appear on the daub was sign of success.These mice were killed after this experiment and the skin of the back was cut off and sent to the pathological image.Then to observe and compare the pathological changes between the different groups.The second experiment:5 mice was choosing from the 60 mice.The 0.05%Clobetasol propionate tincture was applied to the skin of the back 2 times every day and stopped coating for 3 days.Then observed the skin reaction and applied liquid nitrogen to skin and cream 3 times.Then killed one mouse a day for the next five days and the skin of the back was cut off and sent to the pathological image.Then to observe and compare the pathological changes between the different groups.The third experiment:To the remaining 55 mice,the 0.05%Clobetasol propionate tincture was applied to the skin of the back 2 times every day and stopped coating for 3 days.Then the mice was divided into 3 groups in the order of fetching.The group A contained 20 mice,the group B contained 20 mice,the group 3 contained 15 mice.And to the group A,the scheme was liquid nitrogen to wipe the skin frost three times.A moderate amount of Chinese herbs mixture was fed into their stomachs.The dosage of the Chinese herb mixture was calculated as 0.1ml per 10g mouse.So,each mouse was given about 0.3ml each time and 2 times were given to one mouse per day.7 weeks of persistent treatment was needed.To the group B,the scheme was a moderate amount of tacrolimus was applied to the skin 2 times a day,and the traditional Chinese medicine was fed to stomach was 0.3ml per mouse,2 times one day for 7 weeks.To group C,the scheme was given no treatment until the end of the experiment.The experiment' s material:Cut the skin before each freeze treatment and 4 weeks after the experiment from the three groups for 1 mouse each time.Then observe the skin under HE is staining and to find out the distribution of inflammatory cells under the skin.The changes and expression of inflammatory were analyzed and evaluated.All data was input SPSS19.0 software for analysis.Results1.Document research:A total of 336 documents were retrieved,and I excluded unrelated literatures and repeated literatures,and more than 290 literatures were included in the data mining system.In these literatures,134 kinds of western medicine participate in the treatment.236 kinds of Chinese herbs were used.2.Clinical research:The 63 clinical cases were treated by group,and the efficacy of liquid nitrogen freezing combined with traditional Chinese medicine was better than that of other groups.There are advantages in terms of treatment time.3.Experiment study:Model experiment were made on 40 mice successively and 60 mice with hormone dependent dermatitis were treated.0.05%Clobetasol propionate tincture was the best building drug.In experiment 3,the skin lesions were cut before each treatment and after 4 weeks of the end of the experiment.The skin of the mouse was dyed and a lot of pathological images.And we found the cutting layer of the skin in the cry preservation group was significantly thinner.No apparent thinning was seen in the skin of the tacrolimus mice.But,the number of inflammatory cells in subcutaneous tissues of the liquid nitrogen freezing group was significantly less than that of tacrolimus groups.Conclusion1.Document research:In these more than 290 literatures,most of them tended to use traditional Chinese medicine and western medicine to treat this disease.2.Clinical research:During the two months,the efficacy of the group of liquid nitrogen freezing combined with traditional Chinese medicine was better than the group with tacrolimus.3.Experiment study:The group of liquid nitrogen and traditional Chinese medicine were much better because the inflammatory cell was disappeared,and the other group can't get this effect. |
