Objective Observing the correlation between season(spring,summer,fall,winter and the 24 solar terms)and infection rate in orthopaedics surgical patient.Methods Retrospective study of 500 cases orthopaedic surgical site infection in January 2010 to January 2017 in Fuyang Orthopaedics and Traumatology Affiliated Hospital of Zhejiang Chinese Medical University and The First Affiliated Hospital of Zhejiang Chinese Medical University.I class clean incision surgical site infection:339 cases(48272 cases,infection rate 0.70%);145 cases of open fractures cases(total 2974 cases,infection rate 4.88%);joint replacement surgery infection in 16 cases(2821 cases,infection rate 0.57%).Calculate different surgery(I class clean cut,open fracture surgery,joint replacement surgery)in different seasons(spring,summer,autumn and winter seasons and the 24 solar terms)differences in infection rates.Calculate the distribution characteristics of common infection pathogens in different seasons.Compare the difference between joint replacement cases infected group and the control group by five circuits and six qi.Results I class clean surgical site infection of spring,summer,autumn,winter rate were 0.39%,0.96%,0.88%,0.96%,statistically significant difference(P<0.05).Summer wad the highest infection rate.The slight heat infection rate was 1.86%,higher than other seasons.Open fractures surgical site infection incidence of spring,summer,autumn and winter,respectively 3.71%,7.31%,5.08%,7.31%,statistically significant difference(P<0.05).Summer,the highest infection rate.The slight heat infection rate was 10.71%,higher than other seasons.Joint replacement surgical site infection incidence of spring,summer,autumn and winter,respectively 0.29%,1.16%,0.55%,1.16%,statistically significant difference(P<0.05).Summer,the highest infection rate.Positive bacteria in I class clean surgical site infection pathogens accounted for 76.51%(228/298),mainly staphylococcus aureus(35.58%),the epidermis staphylococcus(15.44%)and dung enterococcus(12.08%).Negative bacteria accounted for(22.13%),mainly is the e.coli(6.04%),pseudomonas aeruginosa(5.37%),sewer,e.coli(4.03%).The rate positive higher than negative,with statistical difference(P<0.05).Negative bacteria of open fractures of pathogens was 58.46%(76/130),mainly staphylococcus aureus(23.85%)and the epidermis staphylococcus(13.85%),dung enterococcus(8.47%).Negative bacteria accounted for(38.46%),mainly pseudomonas aeruginosa(13.85%),e.coli(10.00%),sewer,e.coli(3.85%).The rate positive higher than negative,with statistical difference(P<0.05).137 cases of staphylococcus aureus infections,incidence of spring,summer,autumn and winter,respectively 26,48,34 and 29 cases.16 cases of infection after joint replacement,including 2 cases of spring,summer,8 cases,4 cases of autumn and winter in 2 cases.Infection group and the control group of five circuits and six qi,patients with "wood" and "Hyper-wood" and Jueyin wind-wood statistically significant difference(P=0.033,0.033,0.007).In polite company day,jue evil wind wood statistically significant(P=0.007);Main kind of double factors analysis,"the sun is cold water main gas-Yang Ming dry kind of gold" and "the sun is cold water main gas-cold water kind" statistically significant(P = 0.013);From birth in the control group(12.5%)and infection group(43.75%)the period of the day from 9 a.m.to 11 a.m.infection accounted for differences with statistical significance(P = 0.04).Conclusion Orthopaedic site infection rate has obvious seasonal,summer is the season of highest incidence,the highest incidence of slight heat in the 24 solar terms."wood","Hyper-wood","the sun is cold water main gas-Yang Ming dry kind of gold" and "the sun is cold water main gas-cold water kind" are the protective factors in the joint replacement surgery.Strengthen spleen in perioperative period and to choose Si-Shi surgery are the effective method to reduce the infection rate.
