A Study Of The Huangliantang Formula Presentation

Aim:This research aims to use the available clinical evidence to describe and discuss the Huangliantang(HLT)formula presentation(fangzheng)in a way that emphasizes concrete clinical indications,in order to improve clinical outcomes and provide reference material for future research.Methods:In order to maintain a clinically relevant focus,clinical materials relating to HLT,including 28 ancient case studies,100 modern case studies,50 cases from advisor Professor Huang Huang's clinical practice,and 15 published clinical studies,were selected for analysis.Data from these materials were compiled and analyzed in order to identify the characteristics of HLT fangzheng with regard to HLT's "suitable constitution" and "suitable conditions."Results:(1)Suitable constitution.Based on analysis of the clinical data,the population suited to using HLT is largely male and middle-aged.The patient's build is typically thin;the complexion is sallow,dusky and lusterless;the lips are red or dusky;the patient easily sweats;the tongue body is pale,red or dusky;the tongue coating is greasy;the pulse is wiry,fine or feeble;on abdominal exam the upper abdomen feels hard and full,there is tenderness in the upper abdomen,and there are pulsations felt around the umbilicus.Psychologically,the patient often experiences irritability,fatigue or insomnia,and sometimes exhibits a tendency toward depression or alcohol abuse.From the perspective of disease tendencies,the patient often experiences various gastrointestinal symptoms as a result of dietary irregularities,alcohol consumption or mood changes,and may have a personal or family history of digestive disease,heart disease or diabetes.Certain lifestyle factors such as dietary irregularities or alcohol abuse may directly or indirectly contribute to the development of HLT fangzheng.In general terms,HLT's suitable constitution may be understood as marked Spleen and Stomach deficiency with a simultaneous tendency toward heat or damp-heat.(2)Suitable conditions.The principal symptom categories treated by HLT include abdominal pain and related symptoms(mostly consisting of upper abdominal pain,but also sometimes presenting as lower abdominal pain,abdominal distention,etc.),nausea and vomiting and related symptoms(nausea,vomiting,poor appetite,reflux symptoms,etc.),diarrhea and related symptoms(loose stool,frequent stool,borborygmus,etc.),insomnia(difficulty falling asleep,waking easily,etc.),and chest symptoms(oppressive feeling in the chest,palpitations,etc.).Secondary symptom categories include oral symptoms(oral ulcers,tongue pain,bitter taste in the mouth,excessive thirst,etc.),temperature-related changes or sensations of the body(aversion to cold,fever,etc.),symptoms of the urinary and reproductive systems(urinary hesitancy,urinary frequency,erectile dysfunction,abnormal uterine bleeding,etc.),weight loss,and dizziness and related symptoms(dizziness,vertigo,etc.).Inflammatory conditions of the digestive tract and their related conditions together represent the most important category of diseases treated by HLT,and include chronic gastritis,gastroesophageal reflux disease,oral ulcers,chronic enteritis or colitis,peptic ulcers,acute gastroenteritis,Helicobacter pylori infection,etc.Other important diseases and disease categories treated by HLT include cholecystitis,cardiac disease(premature ventricular or atrial complexes,coronary artery disease,etc.),functional gastrointestinal diseases and similar conditions,Type 2 diabetes mellitus and its complications,nausea and vomiting of pregnancy,renal disease,and mesenteric lymphadenitis.Conclusion:For the first time,the suitable constitution and suitable conditions associated with HLT are identified using data from case reports,clinical records and clinical studies;also for the first time,Professor Huang Huang' s clinical experience using HLT is analyzed and summarized.Based on the study results,the meaning of the original passages pertaining to HLT in the Shanghanlun,differentiation between HLT fangzheng and other fangzheng,commbn additions to HLT and their clinical indications,and certain considerations in the usage of HLT are also discussed.
