Research On "Pulse Syndrome And Treatment" Of Ischemic Stroke In Modern Medical Records And Summary Of Professor Qi Xianghua's Experience In Treating Ischemic Stroke

Objective:Part of modern medical records: This study collects modern medical records of ischemic stroke since the founding of the people's Republic of China,excavates some general laws of pulse condition,tongue picture,symptom,syndrome and prescription and medication of modern doctors in treating ischemic stroke,analyzes the correlation between " pulse syndrome and treatment",and discusses the application value of pulse diagnosis in clinical diagnosis and treatment of ischemic stroke.Part of clinical research: This research excavates professor qi xianghua's rule of syndrome differentiation and treatment of ischemic stroke in order to provide reference for the diagnosis and treatment of ischemic stroke in traditional Chinese medicine and scientific research.Methods:Part of modern medical records: This research uses the methods of literature search and data mining to collect the medical records of modern famous Chinese medicine in treating ischemic stroke since the founding of the people's Republic of China,sort out the information of pulse condition,tongue picture,symptom,syndrome type,syndrome elements and medicines in the medical records,and use SPSS 20.0 to carry out frequency analysis,cluster analysis and factor analysis,and use apriori algorithm of SPSS modeler 18.0 to carry out association rule analysis.Part of clinical research: This study takes the ischemic stroke patients diagnosed and treated by professor qi xianghua as the research object,records their basic information,syndrome type and prescription,and uses the above statistical methods to dig out the rule of syndrome and treatment.Results:Part of modern medical records:1.Basic information: 656 cases of ischemic stroke are included in this study,of which the most cases are in the acute phase.The ratio of male to female in medical records is 2.13: 1.The average age is 60.11 ± 10.48 years,and the incidence of 50-70 age is the highest.The main cause of the disease was emotional paralysis.Patients with hypertension are the most common.2.Frequency analysis:(1)The description of pulse condition is based on the whole pulse condition,and the most common single pulse is string pulse.The most common phase and pulse is string sliding pulse.(2)Tongue is often greasy with fur.(3)Hemiplegia is the most common of the main symptoms,and dizziness is common in accompanied symptom.This disease easily leads to emotional problems such as irritability.(4)The most common type of syndrome is wind-phlegm obstruction of collaterals.(5)The disease is located in the brain and is most closely related to the liver.Phlegm syndrome is the most common disease syndrome.(6)The top 10 drugs with the highest frequency of use are Angelica,Ligusticum chuanxiong,Acorus calamus,earthworm,astragalus,safflower,Poria cocos,Paeonia lactiflora,licorice,peach kernel.The most drugs are blood-activating and stasis-removing drugs,followed by liver-calming and wind-calming drugs and deficiency-tonifying drugs.The most commonly used drugs for promoting blood circulation and removing blood stasis were used in each stage.3.Cluster analysis and factor analysis: Symptoms are grouped into 8 categories,representing different pathogenesis of diseases.The traditional Chinese medicines are grouped into 14 categories and 19 common factors are extracted.The " syndrome based on syndrome differentiation" and " syndrome based on prescription" have certain rules,which embody the theory of " syndrome differentiation and treatment" in traditional Chinese medicine.4.Analysis of association rules:(1)Compatibility rules of traditional Chinese medicines: The most confident drug pairs are polygala tenuifolia and acorus tatarinowii schott.The three kinds of herbs,four kinds of herbs and five kinds of herbs are mainly composed of seven kinds of herbs in buyanghuanwu decoction.(2)Internal correlation of pulse condition: The correlation between slippery pulse and string pulse is the most common,and the confidence between knotted and intermittent pulse is the highest.(3)Internal correlation of tongue image: The most common combination is thick and greasy fur,and the highest confidence is thick fur,red tongue and greasy fur.(4)The relationship between pulse condition and tongue image: The most common combination is string pulse and yellow fur.(5)Internal association of disease syndrome elements: The highest confidence level is between fu-organ,phlegm and fire.(6)Pulse syndrome related results: Disease syndrome elements are related to pulse conditions: String pulse can be seen in the diagnosis of phlegm,blood stasis,wind,qi deficiency,fire,yin deficiency,yang hyperactivity,fu-organ,qi stagnation,cold and blood deficiency.Pulse condition is related to disease syndrome elements: Associated with unsmooth pulse and blood stasis syndrome,slippery pulse and sputum syndrome,soft pulse and blood stasis syndrome,weak pulse,unsmooth pulse,soft pulse,moderate pulse and qi deficiency syndrome,rapid pulse + slippery pulse and fiery syndrome etc.There is good consistency between pulse condition and disease syndrome,which reflects the theory of " pulse and syndrome related " in traditional Chinese medicine.(7)Related results of syndrome and drug: The consistency of the correlation between syndrome and drug is good,which reflects the theory of "syndrome and treatment related" in traditional Chinese medicine.(8)Pulse medicine related results: the correlation law conforms to the clinical reality and can reflect the theory of " pulse and treatment related" in traditional Chinese medicine.Part of clinical research:1.Professor qi xianghua believes that ischemic stroke is located in the brain and is related to the liver,kidney,heart and spleen.He emphasizes the important role of emotional factors in the pathogenesis of ischemic stroke and believes that the disorder of qi movement is the main pathogenesis of stroke.wind,phlegm,fire,blood stasis and stagnation are the pathological products of the disorder of qi movement and affect each other to cause common diseases.The disease is usually accompanied by deficiency and excess syndrome.2.Professor qi xianghua's treatment of ischemic stroke focuses on regulating qi or invigorating qi to promote qi movement,so as to help eliminate phlegm,promote blood circulation,calm liver and dispel pathogenic factors.The common clinical syndrome type is yin deficiency and wind-driven syndrome.Qi stagnation syndrome is the most common syndrome element.Commonly used formulae are Tianma gouteng decoction,Xuefu Zhuyu decoction,Gualou xiebai Banxia decoction,etc.The association rules between syndrome type and prescription show that syndrome differentiation has strong association with latent prescription.Common traditional Chinese medicines are Paeonia lactiflora,Radix Angelicae sinensis,Achyranthes bidentata,Glycyrrhiza uralensis,angelica,chuanxiong,bupleurum,safflower,Cynanchum paniculata,gardenia,etc.The most frequently used medicine is the medicine for promoting blood circulation and removing blood stasis.At the same time,professor qi xianghua has original opinions on the functions of exterior-relieving medicine and rheumatism-dispelling medicine.3.The association rules of the compatibility of Chinese medicine: The two flavors with the highest confidence are the perilla,sandalwood and Allium,Herba Leonuri and Radix Polygonum multiflorum,stone cassia and Gastrodia elata,the confidence degree is 100%.The compatibility of three flavor,four flavor and five flavor regimen is relatively regular.Conclusion:1.The results of data mining are mostly in line with the clinical reality and can be used as a reference for clinical diagnosis,syndrome differentiation,prescription and medication.2.The " pulse syndrome and treatment" of ischemic stroke in modern medical records has certain relevance.Pulse diagnosis has high clinical application value,but there is a problem that pulse conditions are not fully recorded.3.Professor qi xianghua has a certain consensus with modern medical furniture in the dialectical diagnosis and treatment of ischemic stroke.He has a deeper understanding of the etiology and pathogenesis,and the dialectical diagnosis and treatment are unique.
