Depression syndrome is a kind of traditional Chinese medicine disease with emotional factors as the main cause and clinical manifestations.It is common in modern medical diseases such as neurasthenia,hysteria,anxiety disorder,depression and so on.With the rapid development of society,the pace of people' s work and life is speeding up,the pressure in all aspects is gradually increasing,and the incidence of emotional diseases is also showing a rising trend.Traditional Chinese medicine has a long history of development in the understanding and treatment of depression syndrome,and its curative effect is obvious.In the long history of the development of Chinese medicine for thousands of years,physicians of past dynasties have recorded their own diagnosis and treatment process and accumulated a large number of valuable medical records,including the records of relevant diagnosis and treatment experience of depression syndrome.The study of TCM medical cases is one of the important ways of academic research of TCM and the inheritance of the experience of famous and old TCM.Based on the data of ancient and modern depression medical cases and data mining as the main research method,this study studied the law of differentiation and treatment of depression syndrome,with a view to providing reference for modern clinical diagnosis and treatment of depression syndrome and TCM case study.1.Research purposesCollect and sort out ancient and modern depression medical records,construct structured ancient depression medical records database and modern depression medical records database;analyze and excavate the law of depression syndrome differentiation and treatment,find out the change of the law of depression syndrome differentiation and treatment in ancient and modern,provide reference for the clinical treatment of modern depression syndrome.2.Research contents and methods(1)Document research to determine the scope of diseaseRelevant textbooks,standards and professional books published within 10 years were investigated to further clarify the concept,clinical manifestations,etiology and pathogenesis,diagnostic points and differential diagnosis of modern depression syndrome.(2)Establishment of inclusion and exclusion criteriaAccording to the different sources of data,we set the inclusion and exclusion criteria of ancient depression medical records,modern medical records and modern medical documents,and the inclusion and exclusion criteria of modern medical records.(3)Data collation and database establishmentTextual processing of ancient medical records and modern medical records is carried out by using text recognition software,screening medical records,and structuring medical records by using medical case splitting software.Filling data,cleaning data,standardizing terminology and standardization of medical case terms are mainly referred to "Thesaurus of Traditional Chinese Medicine" and "Number of Chinese Traditional Medicine".Database,retrieval system,Ancient Chinese Dictionary and Dictionary of Traditional Chinese Medicine,etc.Using ancient and modern medical records cloud platform to construct structured special medical records database of depression syndrome(4)Analytical mining,analysis resultsData mining of ancient and modern medical records of depression syndrome and modern medical cases of depression syndrome was carried out by using ancient and modern medical records cloud platform,TCM Miner,SPSS 22.0 software.Based on mathematical statistics,correlation analysis,clustering analysis,complex network,support vector machine and other methods,the causes,clinical manifestations,treatment methods,prescriptions and other information of medical cases were analyzed,and the results were interpreted.3.Research resultsA special medical record database of depression syndrome was constructed.The database contains 2348 structured medical records of depression syndrome with a total of 4079 visits.Among them,364 cases of ancient depression syndrome were diagnosed 461 times,while 1984 cases of modern depression syndrome were diagnosed 3618 times.In ancient times,most of the patients with depression syndrome were young and middle-aged women aged 20-49 years old.The causes of depression were mostly emotional factors,and the most common symptoms were insomnia.Commonly used treatment methods are phlegm-resolving,liver-soothing,depress ion-invigorating and spleen-invigorating;the high-frequency prescriptions used in treatment are Guipi Decoction(27 times),Xiaoyao powder(22 times),Jiawei Xiaoyao powder(15 times);high-frequency traditional Chinese medicine includes Poria Cocos(186 times),orange peel(115 times),licorice(113 times),ginseng(101 times),Angelica(98 times),Atractylodes macrocephala(94 times),jujube seed(80 times),etc.Spicy,return to the spleen meridian,lung meridian;commonly used Chinese herbal medicine combinations are Atractylodes macrocephala-Poria cocos,Poria cocos-licorice,Poria cocos-Angelica sinensis,Baizhu-licorice,etc;commonly used core prescriptions are based on Danzhi Xiaoyao Powder.The high-frequency Chinese medicines used in the treatment of depression syndrome in Ming Dynasty include ginseng(50 times),liquorice(49 times),Poria cocos(49 times),Angelica(47 times),Atractylodes macrocephala(45 times),Astragalus(32 times),Bupleurum(28 times),Gardenia(26 times),jujube seed(26 times),etc.The meridians of sexual taste are mostly warm,flat,bitter,bitter,belong to the spleen meridian and lung meridian;the commonly used medicines combinations are Atractylodes macrocephala-fuling,Atractylodes macrocephala-ginseng,angelica-ginseng and so on.Licorice-ginseng,liquorice-Atractylodes macrocephala,Atractylodes macrocephala-angelica,etc.The core prescription is Guipi Decoction.The high-frequency Chinese medicines used in the treatment of depression syndrome in Qing Dynasty included Poria Cocos(137 times),orange peel(89 times),Poria God(68 times),Paeonia lactiflora(67 times),licorice(64 times),tulip(63 times),peony peel(58 times),Pinellia ternata(57 times),Xiangfu(55 times),jujube seed(54 times),etc.The main types of sexual taste were warmth,flatness,sweetness,bitterness and bitterness,which were attributed to the lung meridian and spleen meridian meridian.Poria cocos-licorice,Atractylodes macrocephala-poria cocos,angelica-poria cocos,Fushen-jujube seed,angelica-paeonia lactiflora and so on;the core prescriptions are based on Danzhi Xiaoyao powder and Guipi decoction.The morbidity of modern depression syndrome is concentrated in 20-59 years old,mostly young and middle-aged women.Common symptoms include insomnia,stupor,distress,depression,chest tightness,fatigue,foreign body sensation in throat,fatigue,palpitation and so on.Red tongue,yellow fur and strings are the most common veins of the tongue.The common treatment methods are soothing liver and regulating qi,relieving depression,resolving phlegm and calming the mind.The high frequency prescriptions used in the treatment included Xiaoyao Powder(278 times),Banxia Houpu Decoction(162 times),Ganmai Dazao Decoction(147 times),Guipi Decoction(104 times),Wendan Decoction(92 times),Chaihu Shugan San(91 times).High-frequency Chinese medicines include Bupleurum chinense(1546 times),Poria Cocos(1210 times),Angelica sinensis(1167 times),Paeonia lactiflora(1109 times),Tulip(1091 times),Licorice(939 times),Prepared Licorice(752 times),Xiangfu(624 times),Date(614 times),Ligusticum Chuanxiong(605 times),Pinellia ternata(577 times),Acorus tatarinowii(576 times),Chen Pi(558 times),Magnolia(553 times),Stir-fried Jujube Seeds(535 times).Drug attributes are mostly warm,flat,bitter and spicy,which belong to the spleen meridian,lung meridian,liver meridian and heart meridian.Common combinations of Chinese medicines are:Paeonia lactiflora-Bupleurum,Angelica-Bupleurum,Bupleurum-Tulip,Angelica-Paeonia lactiflora,Angelica-Paeonia lactiflora-Bupleurum and so on.The core prescriptions are sour jujube seed decoction,Chaihu Shugan powder and Erchen decoction.The high-frequency Chinese medicines used in the treatment of stagnation of Liver-qi syndrome are Bupleurum chinense(299 times),Angelica sinensis(240 times),Paeonia lactiflora(226 times),Tulip(220 times),Xiangfu(175 times),Poria Cocos(154 times),fried jujube seed(115 times),Chuanxiong(114 times)and Licorice root(113 times).The core prescription is based on Chaihu Shugan powder.The high-frequency Chinese medicines used in the treatment of Qiyu Huahuo depression syndrome are Bupleurum chinense(350 times),Tulip(266 times),Paeonia lactiflora(262 times),Angelica sinensis(221 times),Pori a Cocos(184 times),Acorus tatarinowii(167 times),Licorice Glycyrrhizae(162 times),Ligusticum Chuanxiong(140 times)and Albizia bark(127 times).The core prescriptions are based on Xiaoyaosan and Dingzhidan.The high frequency Chinese medicines used in the treatment of phlegm-qi stagnation syndrome are Poria Cocos(260 times),Magnolia officinalis(210 times),Licorice(175 times),Albizzia Juba bark(50 times),Ligusticum Chuanxiong(45 times),Schisandra chinensis(41 times),Poria(41 times),Night Jiaotong vine(36 times),Coptis chinensis(36 times),Dangsheng(31 times),Huangqi(31 times),Ligu cattai(27 times),raw Rehmannia glutinosa(27 times),cooked Rehmannia glutinosa(25 times)and so on.The core prescriptions are Tianwang Buxindan and Chaihushugan Powder.Using the SVM classification model of modern medical record training of depression syndrome included in this study,the diagnostic characteristic symptoms of depression syndrome were calculated,such as Liver-qi Depression Syndrome(belching,shortness of breath,hypochondria,etc.),qi depression and fire syndrome(dry stool,bitter mouth,irritability,etc.),phlegm-qi depression syndrome(foreign body sensation in throat,belching,hypochondria,etc.),and mental distress syndrome(good depression,good crying,cowardice,fear,etc.).The high-weight symptoms of deficiency of both heart and spleen(belching,dullness,difficulty in falling asleep,etc.)are less accurate in calculating the high-weight symptoms of deficiency of heart and kidney yin.4.Research conclusionIn ancient times,the patients with depression syndrome were mainly young and middle-aged women;the causes were mainly emotional factors,and most of them suffered from anxiety;the onset of depression first affected the heart and mind,liver and spleen were obviously affected;the treatment focused on liver and spleen,mainly invigorating the spleen;the treatment of depression in Ming Dynasty was mainly invigorating the spleen and soothing the liver;the treatment of depression in Qing Dynasty was mainly invigorating the spleen and soothing the liver.Modern depression syndrome patients are mainly young and middle-aged women;the disease involves five viscera,tongue pulse characteristics are obvious;the treatment of liver and spleen,focusing on soothing the liver,not forget to calm the mind.The treatment of modern depression syndrome is based on the principle of soothing the liver,regulating qi and relieving depression,in which the stagnation of Liver-qi is supplemented by tranquilizing the mind;the stagnation of Qi is supplemented by clearing heat,nourishing blood and tranquilizing the mind;the stagnation of phlegm is supplemented by Invigorating the spleen and invigorating the spleen;the deficiency of heart and spleen is supplemented by Invigorating Qi and blood,invigorating the spleen and nourishing the heart;the deficiency of heart and kidney is supplemented by nourishing blood,invigorating the spleen and invigorating the spleen.Nourishing Yin,clearing away heat and calming the mind.The analysis method based on SVM can better find the relationship between symptoms and syndromes of depression syndrome.Both ancient and modern depression syndrome patients were mainly young and middle-aged women.The age of modern depression syndrome patients in ancient times increased by 10 years.The diagnosis and treatment of modern depression medical cases pay more attention to tongue image than ancient ones.Modern physicians pay more attention to summarizing the law of disease development,classifying and treating depression syndrome according to the change of pathogenesis in the course of disease development,and using drugs closely to the pathogenesis.Ancient and modern treatment of depression syndrome pay attention to harmonizing the liver and spleen,calming the nerves,but modern physicians are more soothing liver.
