Research On The Syndrome And Treatment Of Epilepsy Based On Ancient And Modern Medical Records

ObjectiveTo explore the rules of syndrome differentiation and treatment of epilepsy based on the ancient and modern epilepsy medical records data.Comparing the similarities and differences between ancient and modern epileptic syndrome differentiation and medication,deepening the theoretical understanding of traditional Chinese medicine on epilepsy,and exploring the best treatment of epilepsy,so as to provide basis and expand ideas for the clinical treatment of modern epilepsy.MethodsThe ancient and modern medical records cloud platform(v2.3.8),Chinese Science and Technology Journal Database(VIP),China National Knowledge Internet(CNKI)and China Academic Journal Database(Wanfang Database,CSPD)were used as data sources to collect and sort out the medical case literature meeting the selection criteria.The standardization function of ancient and modern medical records cloud platform(v2.3.8)is used to standardize the data,and the methods of frequency analysis,association rules,cluster analysis and complex network analysis are used for data mining.Results1.192 medical records were included in the ancient medical records database.The top five were Dangguilonghui Pill(6),Yuxian Pill(4),Buzhongyiqi Decoction(4),Bazhen Decoction(3)and Liuwei Pill(3).A total of 244 drugs were involved,with a cumulative frequency of 1975 times.The high-frequency drugs were Arisaema Cum Bile(69 times,35.94%),Tangerine peel(68 times,35.42%),Pinellia ternata(61 times,31.77%),Acori graminei Rhizoma(59 times,30.73%),Glycyrrhiza(54 times,28.12%),Poria with Hostwood(50 times,26.04%),Angelica sinensis(49 times,25.52%),Poria Cocos(48 times,25%),Coptis(45 times,23.44%)and Polygala(43 times,22.40%).Most of the herbs were warm,cold,neutral,sweet,bitter or pungent,channel tropisms mainly belong to heart,lung or liver.According to the classification of efficacy of high-frequency traditional Chinese medicines,the top five are:Liver-calming and Wind-soothing Herbs,Defect-tonifying Herbs,Phlegm-reducing and Antiasthmatic Herbs,Herbs for Blood circulation and Blood Stasis removing,Mind-soothing Herbs and Qi-regulating Herbs.Commonly used drug compatibility in the order:Pinellia ternate-Tangerine peel,Acori graminei Rhizoma-Arisaema Cum Bile,Polygala-Acori graminei Rhizoma,Uncaria-Tangerine peel,Radix Curcumae-Arisaema Cum Bile,Concretio Silicea-Arisaema Cum Bile,Uncaria-Pinellia ternate,Atractylodes-Ginseng,Atractylodes-Angelica,Atractylodes-Glycyrrhiza,Radix Astragal-Angelica,Gastrodia elata-Arisaema Cum Bile,Gastrodia elata-Pinellia ternate,Jhuru-Tangerine peel.Through complex network analysis,the core prescriptions were Dingxian Pill,Wendan Decoction and Liujunzi Decoction.2.A total of 320 medical records were included in the modern epilepsy medical records database.A total of 209 prescriptions were identified,and the top five were:Chaihu plus Longgu Muli Decoction(20),Wendan Decoction(20),Ditan Decoction(14),Dingxian Pill(10),Liangge Powder(7).The top 10 symptoms with high frequency were:convulsions(191 times,59.69%),sudden coma(170 times,53.12%),foaming at the mouth(100 times,31.25%),staring into the eyes(86 times,26.88%),weak(64 times,20.00%),dizziness(51 times,15.94%),fatigue(51 times,15.94%),insomnia(50 times,15.62%),headache(48 times,15.00%),anorexia(48 times,15.00%).A total of 322 herbs were involved,with a cumulative frequency of 4468 times.The high-frequency herbs were Pinellia ternata(165 times,51.56%),Glycyrrhiza(157 times,49.06%),Acori graminei Rhizoma(157 times,49.06%),Arisaema Cum Bile(139 times,43.44%),Bombyx Batryticatus(135 times,42.19%),Scorpion(130 times,40.62%),Poria Cocos(124 times,38.75%),Gastrodia elata(107 times,33.44%),Polygala(107 times,33.44%)and Tangerine peel(107 times,33.44%).Most of the herbs were neutral,warm,cold,pungent,sweet and bitter,channel tropisms mainly belong to liver,lung and heart.Commonly used drug compatibility in the order:Tangerine peel-Pinellia ternata,Acori graminei Rhizoma-Pinellia ternata,Poria Cocos-Pinellia ternata,Arisaema Cum Bile-Acori graminei Rhizoma,Arisaema Cum Bile-Pinellia ternata,Polygala-Acori graminei Rhizoma,Bombyx Batryticatus-Arisaema Cum Bile,Arisaema Cum Bile-Bombyx Batryticatus,Scorpion-Bombyx Batryticatus,Tangerine peel-Poria Cocos.Through complex network analysis,the core prescription were Chaihu plus Longgu Muli Decoction,Dingxian Pill,Wendan Decoction and Lingguizhugan Decoction.Conclusions1.Ancient physicians believed that the pathogenic factors of epilepsy were related to wind,fire,phlegm,blood stasis and deficiency,especially deficiency.The position of heart is important,also related to lung,liver,spleen and kidney.The treatment is mainly to recuperate the spirit,to pay equal attention to the five viscera,treating both symptoms and root causes as the basic principle.The prescription is based on the principle of strengthening the body and dispelling the pathogenic factors,and attaches importance to the use of the medicine to tonify deficiency.2.According to the analysis of modern medical case data,there are more male patients,mainly teenagers.The pathogenic factors are related to wind,fire,phlegm,blood stasis and deficiency.The disease is located in the liver,and is related to the lung,heart,spleen and kidney.The treatment mainly focuses on soothing liver and regulating qi,with the basic principle of removing pathogenic factors and reducing adverse conditions,and keeping healthy.The prescription principle is mainly to dispel evil spirits and pay attention to the use of drugs to calm the liver and wind.3.The pathogenic factors of both ancient and modern epilepsy are related to wind,fire,phlegm,blood stasis and deficiency.Ancient doctors attach importance to deficiency,while modern doctors attach importance to qi;Ancient and modern doctors are good at overall grasp of the disease,comprehensive syndrome differentiation,medication closely related to the pathogenesis,but ancient doctors focus on the mind to calm down,modern doctors focus on the liver and regulate qi.4.Epilepsy should be early detected and early treated.The pathogenic factors of epilepsy include wind,fire,phlegm,blood stasis and deficiency.The combination of syndromes is complex and diverse,and there are individual differences.The pathogenic factors are relatively stable.Epilepsy treatment should take into account the five viscera,treat both symptoms and root causes.
