Background and ObjectivesCytomegalovirus(CMV)and Epstein-Barr virus(EBV)and are two of the most common pathogens that cause infectious diseases,which might lead to multi-system disorders.CMV and EBV infections are widely distributed worldwide,with general population being susceptible to both of them.Reactivation of EBV may happen in individuals with immunodeficiency who had latent EBV infection.Patients with inflammatory Bowel Disease(IBD)are often treated with immunosuppressant and/or biological preparations which might lead to EBV reactivation,disease exacerbation,or even death.Therefore,knowing of the related risk factors might lead to better prognosis.This study aims to:(1)to see the seroprevalence of CMV and EBV infection in Chinese population;(2)to explore the risk factors associated with EBV infection in IBD patients.Part ?.Seroprevalence of Cytomegalovirus and Epstein-Barr virus infection in Chinese community population.Methods Epidemiological Investigation of the Common Digestive Diseases in China uses stratified multi-stage random sampling,which selected a community population of 27,612 individuals from 11 provincial administrative divisions as the research object.All basic population sociology data were collected and the serum samples were obtained.Sample size is estimated according to published seroprevalence of CMV and EBV.Detection of CMV IgG and EBV-VCA IgG uses ELISA.For all the data analysis,t-test was used to analyze the difference between the quantitative indicators.Chi-square test was used to analyze the difference between the different groups.P<0.05 was considered statistically significant.Results1.All entry individuals have an average age of 36.8 years old,ranging from 20 to 60.754 males and 731 females were included.2.1485 qualified sero samples were tested for CMV IgG.The overall prevalence of CMV IgG is 97.2%,of which the male prevalence is 96%,and female 98.5%,The prevalence of the two sex group was significantly different(P=0.014).3.1477 qualifiecd serol samples were tested for EBV-VCA IgG,The overall prevalence of EBV-VCA IgG is 97%with the average age being 36.8 years old.The prevalence of CMV IgG in male population was 96.1%,and 97.9%in female population.Conclusion The prevalence of CMV IgG in Chinese community populaiton is 97.2%,with female a little higher than that of male.The prevalence of CMV is significantly different between different age groups,which increases with age.The overall prevalence of EBV-IgG is 97.2%,without significant difference in different regions,age nor gender groups.Part ?.Risk factors of EBV infection in IBD patientsMethods:Hospitalized patients who meet the criteria of IBD diagnosis and EBV infection in Peking Union Medical College Hospital during 2010-2018 were included in the case group.Matched with age,sex,diagnosis,with the ratio of 1:1,the control group includes hospitalized patients in PUMCH diagnosed IBD without any EBV infection continuously at the same period.All Data were collected from the digital medical records system,SPSS 22.0 were used for statistics analysis.P<0.05 were setted as the standard for statistically significance.Results:The study included 52 IBD patients,of which 26 were diagnosed IBD with EBV infection in the case group.Another 26 IBD without EBV infection patients were included in the control group.The average age was 39.3±14.4 years old,with 17 men(65.4%)and 9 women(34.6%)in each group.Among them,18 cases were diagnosed UC,which accounted for 69.2%.For UC patients,the EBV infection group had a higher Mayo endoscopic score(P=0.002),while for CD patients,EBV infection group were more likely to have their anal spared(P=0.016,OR=0.333).Smoking rate is higher in IBD patients with EBV infection group(P=0.044,OR=3.333).Conclusion:IBD patients with smoking habit,increased neutrophils count are more likely to be susceptible to EBV infection.However,due to the limitations of case-control study,with the sample size of cases group being relatively small,statistically significant evidence cannot be find in possible related risk factors which have already been published,such as azathioprine consuming;biological preparations,and corticosteroid therapy.
