ObjectiveTo analyze the potential correlation between constipation and damp pathogen syndrome among patients with non-dialysis chronic kidney disease (Stage 4-5), so as to provide a theoretical basis for the Chinese herbal enema therapy (purging Fu viscera and expelling turbid)in the treatment of Chronic Renal Failure.Methods160 patients with non-dialysis chronic kidney disease (Stage 4-5) from outpatient department of Nephrology in Guangdong Provincial Hospital were enrolled in this observational research (cross-sectional study) according to the principle of sample size estimation in multivariate statistical analysis. All the information (sex, age, body mass index, serum urea, creatinine concentrations, glomerular filtration rate, the use of diuretics, calcium channel blockers, calcium supplement agents, iron supplements, phosphate binders or acid-inhibitory drugs containing aluminum, Chinese patent medicine or Chinese herb decoction containing rheum officinale, whether often eat vegetables or not, whether often eat spicy food or not, whether with damp pathogen syndrome or not) of the patients was gathered by means of face-to-face interview and was entered into PASW Statistics 17.0 to establish a database. At first, the data was analyzed retrospectively with univariate analysis to filter the factors related to the constipation of the patients. Then those factors with statistic differences were further analyzed by logistic regression analysis, P values less than 0.05 were considered significant in this study.ResultsAmong the 160 non-dialysis chronic kidney disease (Stage 4-5)patients, there were 84 males (52.5%) and 76 female (47.5%). The range of the age was from 20 to 80 years old, with an average of 59.12. There were 50 (31.3%) and 110 (68.7%) patients respectively in stage 4 and 5 of chronic kidney disease. Those with constipation took up 38.8%(62 cases) while those without constipation 61.2%(98 cases). Those with damp pathogen syndrome took up 74.4%(119 cases) while those without damp pathogen syndrome 25.6%(41 cases). The univariate analysis showed that age (t=2.409, P=0.017), serum creatinine concentration t=4.026, P=0.000), serum urea concentration( t=8.316, P=0.000), glomerular filtration rate (Mann-Whitney U=2023.000, Wilcoxon W=3976.000, Z=-3.555, P=0.000) and damp pathogen syndrome (χ2=8.596, P=0.003) were correlated with the constipation of the patients. And further logistic regression analysis indicated that age (OR=1.027,95% CI=1.002-1.053; P=0.032), serum creatinine concentration (OR=1.003,95% CI=1.001-1.004; P=0.001) and damp pathogen syndrome (OR=2.497,95% CI=1.025~6.082;P=0.044) are all independent risk factors for constipation of these patients.ConclusionThere is a certain occurrence rate of constipation among patients with non-dialysis chronic kidney disease (Stage 4-5). Age, serum creatinine concentration and damp pathogen syndrome have certain corelation with constipation of these patients; patients with damp pathogen syndrome are more likely to suffer from constipation than those without damp pathogen syndrome.
