Background:Synovitis is a common early manifestation of Osteoarthritis(OA),a secondary reactive inflammation.The synovial thickness can be used to assess the severity of synovitis in different parts of OA patients.Postmenopausal women FSH,LH,E2 hormones and IL-1,IL-6,TNF-a inflammation and osteoarthritis less reported.Therefore,to study the hormone status of chronic persistent inflammatory state and metabolic abnormalities in postmenopausal women and to further reveal the correlation between synovial MRI manifestations,synovial pathology and ankle pain and function is of concem.At the same time,OA is the most common joint disease in middle-aged and elderly people.It is the leading cause of joint dysfunction in the elderly and seriously affects the quality of life of the elderly.Therefore,the search for the pathogenesis and treatment of osteoarthritis has become a research hotspot in the field of joint surgery.Objective:The subject mainly elucidates the correlation between FSH,LH,E2 hormones,IL-1,IL-6,TNF-aincreased levels and osteoarthritis in postmenopausal women.It shows that synovial thickness,synovial pathology score,ankle joint Pain and functional score between the post-menopausal women to take a positive intervention in OA,early prevention and development of an effective step-by-step treatment to provide a theoretical basis.Methods:Fifty-two patients(52 males)who met the enrolling criteria were enrolled in this study.All patients were evaluated as ankle joint OA according to the radiographic criteria and healthy women as controls.FSH,LH,E2,IL-1,IL-?,synovial specimens taken arthroscopy pathology,according to the AOFAS ankle before and after surgery,VAS pain score assessment.Injecting interleukin-1 receptor antagonist into the animal model of knee osteoarthritis,we detected and evaluated the articular cartilage,synovial fluid and synovial membrane in rat KOA model by histological and biochemical techniques.Results:1.Postmenopausal women FSH and LH hormone levels reached the highest value at 60 years of age,E2 hormone levels gradually decreased with age,the expression of FSH and LH hormone levels in patients with arthritis than the normal control group,E2 hormone lower than the normal group,The levels of IL-1,IL-6 and TNF-a were all higher than those in the normal group(P<0.05)2.There were no significant differences in the thickness of synovial membrane,the synovial pathological score and the average pathological index(F = 2.127 P = 0.132)in the four observation sites of the lateral trough of the talus,the tibial articular surface,the talar articular surface and the talar medial sulcus.),And the three had no correlation with the function and pain score of ankle.The MRI thickness classification of chronic unstable ankle synovitis had a positive correlation with the corresponding pathological score(rs:0.74,0.56,0.59 and 0.71,P<0.01).There was a positive correlation with pathological parameters(rs:0.44,0.47 and 0.39,P<0.05).3.Using the Hulth arthroplasty in the rat model of osteoarthritis,the semi-quantitative score of histopathological changes in the experimental group was significantly different from that in the hematoxylin-eosin(HE)staining and safranin O staining and Mankin's articular cartilage histological score,Synovial fluid NO,IL-1? content decreased significantly,articular cartilage matrix type II collagen positive staining increased.Conclusions:Postmenopausal women FSH,LH and E2 hormones,IL-1,IL-6 and TNF-a levels of inflammation have changed,and AOFAS function and VAS pain score was not related.MRI examination for the selection of patients with synovitis OA is an effective clinical diagnosis of disease.Chronic Instability The thickness of the MRI grading and pathological score of ankle synovitis in ankle can not be used as an indicator of the severity of synovitis to determine whether an ankle arthroplasty is necessary or to assess the progression of disease and the outcome of treatment.Osteoarthritis Hulth intra-articular arthroplasty,high success rate of modeling method,good stability,interleukin-1 receptor antagonist can reduce synovial inflammation in animal models of KOA,promote the synthesis of type II collagen and promote joint Cartilage extracellular matrix synthesis,the newly synthesized matrix degradation significantly inhibited,thereby delaying the degeneration of articular cartilage.
