Experimental And Clinical Research Of Yiqi Yangyin Tongluo Sanjie Recipe In Treating Early Diabetic Heart Disease

Diabetic heart disease,which is one of the diabetes chronic complications,the pathogenesis of the microangiopathy is too complicated to be entirely illuminated.lt is lack of specificity strong quantitative indicators to assist in diagnosis,although western medicine in controlling blood sugar is exact,but the lack of specific treatment for this disease,which is still the main direction of future researeh.The view of TCM based on Qi and Yin deficiency,and blood stasis is the major causative factor in the disease process and pathological product,has been the consensus of contemporary physicians.In this thesis,based on the preliminary Yi-qi Yang-yin Tong-luo San-jie studies on diabetic cardiopathy,to discuss furtherly the possible mechanism of action of curative effect through the perspective of cytokines and clinical research.This paper consists of three parts,including two summaries of documents,experimental study and clinical research.Objective:To study the effects of Yi-qi Yang-yin Tong-luo San-jie herbal compound on cardiomyopathy of STZ diabetic rats model and KKAy mice model and the expression of Ang ? and of myocardial tissue.The clinical study observes the clinical effect of Yi-qi Yang-yin Tong-luo San-jie herbal compound on treating diabetic cardiopathy of early stage.Methods:The experimental part of this project through the establishment of STZ diabetic rats and KKAy mouse model to observe changes in cardiac morphology,expression of Ang ? and IGF-1 in serum by ELISA.expression of Ang ? and IGF-1 protein and mRNA in myocardial by immunohistochemical,westernblot,In situ hybridization and RT-PCR.The clinical part of this project observed early diabetes patients with heart disease.The patients conforming to the diagnostic criterion were randomly divided intotreatment group and control group.The control group was given basic treatment alone.On the basis of the control group treatment,the treatment group was treated with Yi-qi Yang-yin Tong-luo San-jie herbal compound.The clinical symptoms,blood sugar,glycosylated hemoglobin,blood lipid,ultrasonic echocardiography were observed after 8 weeks.Experimental data were analyzed statistically with SPSS 17.0.Results:1.The experimental research,The experimental research,STZ diabetic rats model weight growth slow in the normal group(P<0.01),the weight of traditional Chinese medicine group compared with model group increased(P<0.01);Angll protein and mRNA expression of model group was obviously increased(P<0.01),Chinese medicine group to significantly reduce Angll expression(P<0.01);IGF 1 protein expression of model group significantly reduced(P<0.01)3 compared with model group,IGF 1 protein and mRNA was significantly increased in Chinese medicine group(P<0.05).The same trend in 4 weeks and 8 weeks.2.The experimental research,KKAy diabetic mice model group,compared with normal group,the experiment of 4th,8th,12th weeks of FBG,TG,TC,LDL-C significantly increased,significantly lower HDL-C(P<0.01).Compared with model group,FBG obviously decreased in Chinese medicine group in the 12th week(P<0.01),in the 8th week,blood TG,TC and LDL-C was decreased(P<0.05),blood HDL-C has a rising trend(P<0.05).Compared with normal group,the serum and myocardial tissue Angll expression increased significantly(P<0.01),compared with model group,Chinese medicine group of serum Angll expressed in 8th and 12th weeks decreased obviously(P<0.05),Angll protein of myocardial tissue expressed in 4,8,12 weeks were significantly lower(P<0.05),Angll mRNA expressed significantly lower in 12 weeks(P<0.05).In experiment 4th,8th,12th weeks,compared with normal group,the expression of IGF-1 in serum and myocardial tissue decreased obviously(P<0.01),compared with model group,Chinese medicine group of serum IGF-1 in 8 and 12 weeks expression significantly increased(P<0.05).3.The clinical research,compared with the control group,indicating that the treatment group decreased in patients with FBG,TG,TC,LDL-C(P<0.05,P<0.01)and increased HDL-C(P<0.01),and improve the A/E,AO,an LVDs and LVDd,LPW,EF ultrasonic echocardiography index(P<0.05,P<0.01),and improve the chest tightness,chest pain,weakness of tired,thirsty like drinking,numbness symptoms(P<0.05,P<0.01),the treatment group of TCM syndrome score was significantly lower than the control group(16.55 vs23.44),total effective rate is obviously better than that of control group(90.32%vs68.75%).Conclusion:Yi-qi Yang-yin Tong-luo San-jie herbal compound improved the Angll expression in serum and myocardial tissue;regulated the expression of IGF-1 in serum and myocardial tissue,to protect the heart of the diabetic animal models.Yi-qi Yang-yin Tong-luo San-jie herbal compound improved the left ventricular diastolic function of patients with diabetes,the glucolipid metabolic disorders and the clinical symptoms.Therefore,Yi-qi Yang-yin Tong-luo San-jie herbal compound has exact effect on diabetic heart Qi and Yin deficiency blood stasis.
