| ObjectiveProposed from the perspective of retrospective case analysis,this study analysis in nearly 12 years of laryngeal cancer syndrome elements distribution and type of syndrome of traditional Chinese medicine,and,according to the results of the retrospective analysis to study the effect of huoxue phlegm method for laryngeal cancer animal model,selection of samples to detect the expression of micrornas,hope to be able to make the preliminary discussion on the above problems,lay the foundation for further studies,and seek research point.Methods??Clinical researchCollected on patients with laryngeal squamous cell carcinoma,from January 1,2003 to December 31,2014,in otorhinolaryngology department of guangdong hospital of TCM,pathological diagnosis definitely,have detailed medical records,without radiation,chemotherapy and surgery treatment,With laryngeal cancer information collection form,to collect the patient's general information,clinical symptoms,tongue pulse information,hospital inspection results,past medical history,personal history,etc.Converts data input EpiData3.1 software,Excel format check the lack of correction,imported the data to SPSS17.0 statistical software.First,use the descriptive analysis,statistics analysis of all symptoms,tongue and pulse frequency,delete information that is less than 10%.Second,marked appear to 1,don't appear to 0,import the data to SPSS17.0 again for correlation analysis?factor analysis and Clustering analysis.??Experimental studyAccording to the result of retrospective study and theory of phlegm and blood stasis cause tumors,this research observe blood,phlegm and activating blood phlegm method for laryngeal cancer animal models into the role of the tumor process,compare the three methods for laryngeal cancer animal model tumors had the volume,density,tumor weight differences with the control,and selection of samples for influence of high-throughput sequencing?Results??Clinical research(?)The frequency analysis of symptoms shows that,?the main symptoms of the laryngeal cancer patients is hoarseness(121 cases,80.1%),followed is shortness of breath,sore throat,neck neoplasm,pharynx foreign body sensation,frequency are within 10%.?Laryngeal cancer patients' symptoms with the highest frequency is still hoarseness(138 cases,91.4%),and anther four symptoms have the frequency in more than 20%;the pharynx foreign body sensation,cough,dry throat,sore throat.Impeded,pharyngeal itching,difficulty breathing and swallowing,neck mass,swallow burning,this 5 kinds of symptoms,its frequency is below 10%.?The most frequency Tongue is tongue dark or petechiae(109 cases,72.2%),there are other appear more tongue sorted by frequency from high to low in turn is red tongue,yellow greasy moss,tongue reddish,white greasy moss,moss thin white,appears the frequency of not less than 10%.?The most frequency pulse is pulse slip(120 cases,79.5%).Other pulse symptoms,which have more frequency,are pulse string(75 cases,49.7%),pulse fine(20 cases,13.2%),occurrence frequency is more than 10%.(?)The correlation analysis of symptoms shows that:?hoarseness,which has the most frequency in the laryngeal cancer patients,is negatively related with sore throat and red tongue(P<0.05).?Symptoms of pharynx foreign body sensation,cough,dry throat,sore throat,are not related with other tongue veins symptoms(P>0.05).?This tongue and pulse group respectively reflect four elements:phlegm wet,damp and hot,blood stasis.(?)The factor analysis of symptoms showed that,this group of symptoms can be extracted six common factor,the six common factor has 68.6%cumulative contribution rate.According to the related definition of the factor,six common factor can be grouped into four categories,reflect three kinds of syndrome elements:damp and hot,blood stasis,deficiency.(?)Cluster analysis of the symptoms showed that,this group of symptoms elements can be divided into three categories:the first category:sore throat,tongue reddish,thin white moss,white greasy moss,pulse fine.Syndrome differentiation for phlegm dampness syndromes.The second category:hoarseness,tongue dark or petechiaed,pulse slip;syndrome differentiation for the phlegm and blood stasis.Third category:dry throat,pharynx foreign body sensation,cough,red tongue,yellow greasy moss,pulse string;syndrome differentiation for damp heat syndrome.(?)Sample clustering of 151 patients suggests that,this group of patients can be clustering for 3 classes:phlegmy wet clip stasis,phlegm heat and blood stasis syndrome,blood stasis.Phlegm heat and blood stasis accounted for the most proportion,52.3%of the total.(?)Analysis the difference of TCM syndrome types resulted from clustering,in different period,the T stage,different age,sex,disease prognosis,complained of the distribution,the results had not statistical significance.??experimental study(?)Through animal experiments,this study compared activating blood,phlegm and blood phlegm method for laryngeal cancer animal models,the results found that:?From the growth curve,phlegm treatment method group's tumors growth curve,is low and flat than blood treatment method,the circulation and phlegm treatment method and the control group.? Compared the weekly measurement result about tumors volume of each group to known:the tumors'volume of Phlegm group had significant difference compared with controls in first,second and four weeks(P<0.05);The tumors volume of the other two groups has no statistically significant difference,compared with controls(P>0.05).?For each group animal tumors's tumor size,tumor weight,density of comparison,compared with control group,there were no statistically significant difference(P>0.05);When the data is cos function transformation,the tumor weight of promoting blood flow method group,has statistically significant,compared with control group(P<0.05).(?)The sequencing results show that,(1)blood,phlegm and activating blood phlegm method can lead to miR-451-a(known as tumor suppressor genes expression),and this three treatment group compared with control group,difference has statistical significance.(2)The phlegm method can make the miR-19a-3p(suspected is a tumor suppressor gene)expression level was significantly raised,invigorate the circulation of phlegm and activating blood method and the method of the miRNA expression level no difference-compared with controls;Huoxue phlegm method can make the gene of miR-378-a-3p(suspected is a tumor suppressor)expression level was significantly raised compared with controls,and the expression of miRNAs in phlegm and activating blood group level compared with the control group no significant difference.(3)Phlegm method can make the miR-199-a-5p,miR-99-b-5p,miR-183-5p,miR-148-a-5p(suspicious tumor miRNAs)expression had significantly lower compared with controls,may be a treatment target for the phlegm method;Method of huoxue phlegm is suspicious tumor miRNA-96-5p expression is significantly lower,may be the method of huoxue phlegm treatment targets.Conclusion??Clinical research(?)The most complained symptoms of the laryngeal cancer patients is hoarseness.(?)Laryngeal cancer is closely related to the elements of traditional Chinese medicine "phlegm,"stasis" and "hot;"phlegm" and "stasis",this two elements are more important in the early days of laryngeal cancer;Phlegm and blood stasis by each may be a basic type of syndrome of patients with laryngeal cancer.(?)Development relationship between TCM syndrome type of throat cancer are as follows:With elements of traditional Chinese medicine "phlegm" and "blood stasis" as the leading factor,syndrome types will be gradually to developed as time migration and tumor invasion.??Experimental study(?)Experiments on animalsPhlegm method in hep2 form of early laryngeal cancer cell proliferation,may have the effect of inhibiting tumor growth,probably because of the lack of drug dose and action time is insufficient,the tumors had macroeconomic indicators such as volume,density,tumor reflect inadequate aspects and so on.(?)The miRNA detectionCombined with growth curve of groups,the treatment group of tumors volume,density,tumor weight and the control group and the comparison of differences of high-throughput sequencing results,this study thinks activating blood,phlegm,invigorate the circulation of phlegm method for the treatment of laryngeal cancer may all can play a role,support phlegm method for laryngeal cancer therapeutic evidence is sufficient. |
