Based On The Theory Of "body Disease Related", The Clinical Study On The Treatment Of Psoriasis By "regulating The Air - Body - Main Disease"

Background:In 1996,WHO in a report entitled "the challenge to meet the challenges of the year twenty-first Century" noted that in the year of twenty-first Century,medicine will evolve from "disease medicine" to "health medicine",from group therapy to individual treatment.Therefore,the focus of medical research also transfers from the study of the "disease"to the study of the "people".Traditional Chinese medicine regards the patient’s constitution as an important basis for the prescription principle,and it is the specific embodiment of personalized diagnosis and treatment.Wang Qi founded the Chinese Medicine Constitution in 1970s,and put forward the theory of "body constitution and disease related",and set up the model of syndrome differentiation diagnosis and treatment.The five circuits and six Qi is the key factor affecting the body constitution and physical condition.Based on this understanding,the establishment of a new model of clinical diagnosis and treatment of "Regulating Qi-regulating body consititution-main disease" is worth exploring.Objectives:Analyse the psoriasis patients’ body constitution,TCM syndrome types and the characteristics of patients’ five elements and six pathogens,and mining their association to provide theoretical basis for regulating Qi-regulating body constitution-main disease";and compare the curative effects of" Regulating Qi-regulating body constitution-main disease "as the guiding ideology for the diagnosis and the treatment of patients with common method.Method:(1)the enrolled psoriasis patients were divided into 2 groups and differentiate their body constitution according the standard published by the China Association of Chinese Medicine,and the TCM syndrome types according to Chinese medicine clinical research guiding principles;The patients’ date of birth(lunar new year)information were recorded in certain types based on the five elements and six pathogens theory in Excel and to the explore their relations by using statistical software SPSS 17.(2)Compare the the severity index(PASI score),self rating patients with pruritus the degree of score(VAS score)and dermatology life quality index(DLQI score)of the two groups of patients before treatment and 1 months,3 months,6 months.Result:(1)The majority of the psoriasis patients’ body constitution type is damp heat,qi stagnation,phlegm constitution;The majority of the TCM syndrome type are damp heat syndrome,followed by blood heat syndrome;People born in specific years(土运太过之年,五之气阳明燥金所主时段,客气为太阴湿土,少阴君火司天,阳明敛金在泉)are more likely to suffer from psoriasis.People born in other specific years(火运不及之年,三之气少阳相火所主时段,客气为厥阴风木或者阳明燥金,阳明撰金司天,少阴君火在泉或太阴湿土司天,太阳寒水在泉)are not likely to suffer from psoriasis.(2)There is correlation between TCM constitution and TCM syndromes.The majority body constitution of people with damp heat and blood heat syndrome were damp heat constitution.The majority patients were damp heat constitution,and the Qi deficiency body constitution people were tend to be blood heat syndrome;the Yin deficiency body constitution people were tend to be blood stasis syndrome;the Qi stagnation body contitiution people are tend to be blood stasis syndrome.(3)The characterists of the five circuits and six qi are related with the the patients body constitution and TCM syndrome.Patients with damp heat body constitution are more likely born in specifc years(士运太过之年者,主气为五之气阳明燥金所主时段,客气为太阴湿土,少阳相火司天,厥阴风木在泉),and there is no significant difference between the distribution of main Qi(主气),and significant difference among other factors.Patients with damp heat syndrome are more likely to be born in specific years(木运太过或土运不及之年,四之气太阴湿土时段,客气为太阴湿土时段)and has certain features(以厥阴风木司无,少阳相火在泉或少阳相火司天,厥阴风木在泉).There is significant difference between the distribution of Sui yun(岁运)(P<0.05)(4)The comparative study based on the analysis of "Regulating Qi-regulating body constitution and the main disease" shows that the difference of the score of the two groups of patients before treatment was not statistically significant(P>0.05),and the scores of both groups gradually declines after 3 sections of treatment.The PASI and VAS score has significant difference after 1 month of treatment(P<0.05),and DLQI score has significant difference after 6 months treatment(P<0.05).It is shown that the theory has advantages over the traditional syndrome differentiation,and the curative effect can be seen after 1 month treatment and the life quality can be greatly improved after 6 months’ treatment.Conclusion:There is correlation between TCM constitution and TCM syndrome types.The characteristics of the five circuits and six qi are related with the TCM syndrome.The therory "Regulating Qi-regulating body consititution-main disease" shows clinical advantage over the traditional syndrome differentiation,and it can effectively guide the clinical application of in treating psoriasis,especially the synptoms of iching and the severity of the skin lesion and improve the quality of the life.
