Effects Of Interleukin 1 And 6 Gene Polymorphisms On The Prognosis And Cancer Pain In Patients With Non-Small Cell Lung Cancer | | Posted on:2017-12-08 | Degree:Doctor | Type:Dissertation | | Country:China | Candidate:W Jia | Full Text:PDF | | GTID:1314330512972943 | Subject:Internal medicine (pulmonology) | | Abstract/Summary: | PDF Full Text Request | | PartⅠ Clinical Study on Relationship Between IL-1 、IL-6 in Patients with Non-Small Cell Lung CancerObjectiveTo discuss the serum levels of IL-1 and IL-6 in patients with non-small cell lung cancerMthods93 patients with NSCLC who hospitalized in our hospital between May,2010 and July,2014,including 47 cases of stage Ⅲ and Ⅳ were in advanced group,46 cases of stageⅠand Ⅱwere in early-middle stage group,and 49 healthy patients who received health examination in the same hospital were selected as control group.The levels of IL-1 and IL-6 were detected and compared in the three groups.ResultsThere was statistically significant in the levels of IL-1 and IL-6 among earlymiddle stage group,advanced group and healthy group(P < 0.05).Conclusions1.The levels of IL-1 and IL-6 were significantly increased in patients with NSCLC,with the aggravation of the disease,and the level of IL-1 and IL-6 showed increasing trend,which has important signification in diagnosis and judgement.2.The levels of IL-1 and IL-6 have important signification in judgement of patients with NSCLC.Part Ⅱ Correlation Study between IL-1 gene polymorphisms and the risk of NSCLCObjectiveTo study the correlation between+3954,-511,-31 polymorphism in IL-1 gene and the risk of non-small cell lung cancer.Methods434 patients with NSCLC who hospitalized in our hospital between May,1999 and July,2012,using polymerase chain reaction and restriction fragment length polymorphism(PCR-RFLP)technique to study the polymorphism of +3954,-511 and-31 in IL-1 gene.ResultsThe experimental groups and control group,the mutation frequency of homozygous-31C/C wildtype in IL-1 gene was similar,-31T/T genotype of IL-1 gene were 29.6% and 23.3%,respectively.-31T/T genotype can increase the risk of non-small cell lung cancer compared with-31C/C gene type.Both-511C/T genotype and-511C/C genotype increase the risk of non-small cell lung cancer.There was a linkage disequilibrium between-31 and-511 loci in IL-1 gene.The analysis showed that more than three risk alleles will increase the risk of non-small cell lung cancer.Conclusions-31 and-511 site polymorphisms of IL-1 increase the risk of non-small cell lung cancer.Part Ⅲ A study on the effect of IL-6 gene polymorphism on the prognosis of non-small cell lung cancerObjectiveTo evaluate the effect of-174G/C polymorphism on the prognosis of patients with non-small cell lung cancer(NSCLC).MethodsDNA was extracted from the peripheral blood of 434 cases diagnosed with NSCLC by cytologic or histologic examination.Polymerase chain reaction-restriction fragment length polymorphism(Nla III)was used to detect the genotype of-174G/C.Based on the functional activity of the IL-6 gene polymorphism,genotypes were divided into G vector(CG/GG)(high yield)and CC genotype(low yield).Prognosis of patients was analyzed and independent risk factors were evaluated.A quantitative analysis of the degree of pain after diagnosis was performed to evaluate the correlations between gene polymorphisms and the degree of pain after using analgesics.ResultsSurvival analysis showed that survival of the patients carrying the G allele(CG/GG)was significantly lower than that of patients with CC genotype(42.31 versus 62.79 months;P=0.032).Tumor staging(P<0.001),smoking history(P = 0.013)and IL-6-174G/Cgenetic polymorphisms(P = 0.022)are the independent risk factors for overall survival rate in non-small-cell lung cancer patients.Single factor analysis showed that the IL-6-174G/C polymorphism was significantly associated with MEDD(IL-6 G/G,G/C,69.61,93.6;C/C,181.67;P=0.004).According to the gender stratification,male patients have a higher dose of opioid drugs than women.Patients with C/C genotype need the highest dose.The use of the opioid drug dosage in IL-6-174 homozygous gene C/C carriers were significantly higher than that of the GG/GC carriers(OR:4.7 95% CI=1.2-15.0)ConclusionsPolymorphism of-174G/C in IL-6 is closely related to cancer pain in NSCLC patients,using analgesics and survival prognosis.It is necessary to further confirm the related results and determine the underlying pathogenic mechanisms. | | Keywords/Search Tags: | IL-1, IL-6, NSCLC, early-middle stage group, advanced group, polymorphism, risk, non-small cell lung cancer, Interleukin 6, survival rate, cancer pain MEDD | PDF Full Text Request | Related items |
