Background and ObjectiveTeacher-apprentice education has been, for thousands of years, not only a key approach of Chinese medicine education, but also an effective pattern to inherit the academic thoughts, experience, and technical expertise of Chinese medicine. Professor Fu rulin, being one of the third group of distinguished national veteran doctors of TCM, was acknowledged to be the instructor in the workshop of Chinese medicine inheritance by the State Administration of Traditional Chinese Medicine of China in 2011. He is also one of the fifth group of instructors of the national Chinese medicine academic experience inheritance. Having been working on TCM clinical teaching and research for nearly fifty years, Professor Fu rulin gained rich clinical experience to treat common internal diseases, hematological disease and miscellaneous diseases, such as immune thrombocytopenic purpura, aplastic anemia and henoch-Schonlein purpura. Thus he forms particular academic thoughts. At present, published relevant papers are about only certain special disease, medicine or subject. Further comprehensive systematical summary is needed to summarize his academic thoughts and clinical experience. This paper is to systematically sort, analyze, research and summarize the academic thoughts, origin and development and the forming process of professor Fu rulin's treatment of hematologic diseases. And it also concludes the clinical treating experience of the refractory polycythemia vera, so as to master and further spread its dialectical medication rules.Research methods and content:This paper study mainly has the following several parts.The first part is the Professor Fu Rulin's academic thought. The paper summarizes the origin of Professor Fu Rulin's academic thought with the method of interview and analysis. It combed the Professor Fu Rulin's transmission spectrum and the present situation of inheritance, including Professor's road to success and to learn. The paper analyzes the data of out-patients which contains four nature of drugs, five flavours, channel tropism, frequencies, the rules of combination and association of high frequency drug and core drug combination with the help of Traditional Chinese Medicine Inheritance Support System(V2.5). It discusses the law of medications. By using the method of interview and analysis, the paper summarizes the academic thought of Professor Fu Rulin from four aspects. First, the paper pays attention to the body and regulate, in the diagnosis and treatment of haematology. The paper realizes that the Yin blood is in the visible matter distribution in different parts of the body. "Yin" and "blood" are relative and they complement each other. The syndrome of Yin deficiency, qi deficiency, blood deficiency, Yang deficiency and deficiency of essence are coexistence. In the diagnosis and treatment, Professor Fu cures on the basis of method of treating yin deficiency by reinforcing body fluid, and combined removing blood stasis, blood tonic and heat cool blood therapy. Second, the paper pays attention to clearing heat and removing toxicity. Professor Fu thought blood deficiency and heat don't leave the body because the heat injurys Yin. And it is difficult to restore Yinjin. So to cool and detoxicate is one of the important methods to treat various diseases of the blood. In the process of application, Professor Fu thought clear heat, detoxification and cool blood can work with nourishing yin and supplementing blood and cooperate with the vital qi of drugs. This method can cool blood and detoxicate to prompt new blood generation. the generation of new blood can promote the recovery of vital qi and ease blood disease symptoms. Three, the paper pays attention to nourishing liver and kidney Yin and kidney essence. The Professor Fu thought, kidney Yin and kidney Yang root alternate. Each is both independent and interdependent. All Yin of visceral organ comes from the kidney Yin and all Yang of visceral organ comes from the kidney Yang. The functions of the kidney Yin and kidney Yang are related with persistence of activity. In the process of treatment, Professor Fu emphasizes to want to notice of nourishing liver and kidney Yin and kidney essence. It is very important for fever in the hemopathy to recover without treatment. Four, to be good at apply the method of promoting circulation and removing stasis. Blood stasis is the pathological products and pathogenic factors. How to grasp of the method of promoting circulation and removing stasis is the problems which has been emphasized by Professor Fu Rulin. Professor Fu thought there must be some pathological basis of blood stasis, so he opposed to just blindly promoting blood circulation to remove blood stasis. And the treatment should cure this disease. In clinic, Professor Fu pays attention to research and practice of six methods on promoting blood circulation to remove blood stasis. They are Qi promoting blood circulation to remove blood stasis method, Yang promoting blood circulation to remove blood stasis method, nourishing yin and clearing heat blood circulation to remove blood stasis method, nourishing liver and kidney blood circulation to remove blood stasis method, clearing away heat and toxic materials blood circulation to remove blood stasis method and removing phlegm and disintegrating mass blood circulation to remove blood stasis method. Professor Fu used these methods to cure blood stasis on syndrome differentiation. And Professor Fu often uses Peach Seed, Carthamus tinctorius, Radix Paeoniae Rubra, Salvia miltiorrhiza, Caulis Spatholobi and Sanchi Powder to cure blood stasis. Professor Fu also be good at using Sappan Wood and Hirudo as well.The second part is a summary of the clinical experience of Professor Fu Rulin on polycythemia vera by differentiation of "stasis".Polycythemia vera, with a high rate of subsequent visit, is eminently curative by the treatment of traditional Chinese medicine and is one of the predominance disease entities of traditional Chinese medicine, the curative effect of traditional Chinese medicine treatment or of the combined traditional Chinese and Western medicine treatment is better than that of the western medicine. This dissertation is to summarize and inherit the experience of the famous old doctor of traditional Chinese medicine so as to study the regulation and method of experienced doctors and to summarize the clinical experience, regularity of prescription, and the academic thoughts of the famous doctors. Polycythemia vera is a disease of chronic myeloproliferative disorders(CMPD). The cause of this disease remains unclear. And there is no effective method to cure this disease. The aim of treatment is to maintain red blood cell count (RBC), whole body volume and hemoglobin (HGB) within normal limits or close to the normal limits to reduce complication and make the disease stable. Modern medicine science uses the methods of Venipunture, Chemotherapy, Interferon and Simple RBC Sampling to treat this disease. But there are some different disadvantages in these methods. This treatment can not make the disease stable for long time. It can bring high risk of causing other disease and tumor and the cost of treating this disease may be high. Traditional Chinese Medical herbal treatment can relieve patients last longer and without the risk of bone marrow suppression and tumor induction. So Traditional Chinese Medical herbal treatment has a wide applied space and development prospects. Professor Fu Rulin is rich in the clinical treatment of polycythemia vera. According to his theory the disease lesions is in the bone marrow, and closely related to liver, spleen, and kidney. The pathogenesis is complicated and changeable, but the key and core of the disease is "blood stasis". Professor Fu is skilled in integrated the treatment of syndrome and symptom differentiation mutually combined with staging and typing. In the early stage of the disease, evil is in its prime, thus the treatment should eliminate the evil and not assist it; in the mid period, the vital energy is weak but evil is prevailing, so the treatment should strengthen the body resistance and eliminate the evil as well; when to the end-stage of the disease, the body is weakened by the overweighted exhausting, the key of treatment is to strengthen and tonify the body resistance. The treatment stresses on strengthen the body resistance and eliminate the evil. According to the clinical symptoms of patients and the cases of maladjustment of yin and yang with the internal organs dialectical, the theory classified polycythemia vera into five types:liver stagnation and qi stagnation stasis; hyperactivity of liver-yang stasis; deficiency of both qi and blood stasis; deficiency of both spleen and kidney yang stasis; deficiency of both liver and kidney yin stasis. The general therapeutic method is to promote blood circulation and to remove stasis. The "xuefu zhu yu decoction" is the basic treatment of dialectical therapy.
