Objective:To observe the Therapeutic effect of Black Dihuang pill on spleen deficient syndrome in chronic renal failure and the effect on brain-gut peptides and brain-gut axis through clinical and experimental research and explore its functional mechanism.Methods:1.Clinical research:Collecting clinical 70 cases of CRF(CKD2-4) in patients with Spleen Deficient Syndrome.They were further randomly divided into experimental group and control group.and treated with CRF regular modern medicine. The experimental group was treated to take Black Dihuang Pill and the control group was treated to take Niao Du Qing.After two months of treatment, the changes of clinic symptom and laboratory index were examined.2.Experimental research:Setting up 5/6 nephrectomy in the rat model with chronic renal failure.The 64 Building successful rats were divided into 4 groups randomly,which are the Black Dihuang Pill of high dose groups(0.648g/100g.d)(16), the Black Dihuang Pill of low dose groups(0.162g/100g.d)(16), the control group(Niao Du Qing0.225g/100g.d)(16),the model group(normal salin)(16)as well as the blank group(normal salin)(10).To observe the weight, average intake,swimming time,24 hours urinary protein content and the changes of Hb、RBC、A1b、TP、TG、Ca.Using the ELISA method to test the changes of Ghrelin and ss in blood and Stomach tissue homogenate. Using the immunohistochemical method to test the expression of Ghrelin and ss in stomach an duodenum.RT-PCR and Western-blot method was used to detect the content of Ghrelin and ss in hypothalamus.Result:1.Clinical research (1)The effective rate of syndrome in experimental group was 81.25%.The effective rate of the control group was74.19%.There was no remarkable difference (P>0.05).(2)In the experimental group,the level of Scr dropped,the level of Ccr、Hb、ALb and TP rose (P<0.05) (3) In the experimental group,the level of Ghrelin dropped and the level of SS rose,which was better than that of control group (P<0.05).(4)The total effective rate of experimental group was 78.12%,and the control group was 70.97%.There is not difference between the two groups(P>0.05).2.Experimental research (1)The Black Dihuang Pill of high and low dose groups can improve the weight,average intake and swimming time(P<0.05orP<0.01).(2)The Black Dihuang Pill of high and low dose groups can reduce urinary protein,improve the Hb and RBC and regulate the imblance of Alb、TP、 TG and Ca in blood (P<0.05 or P<0.01).(3)The Black Dihuang Pill of high and low dose groups can improve the level of Ghrelin in Stomach tissue homogenate(P<0.05).There was no remarkable difference between the high does group and the control group(P>0.05).The Black Dihuang Pill of high does can improve the level of SS in Stomach tissue homogenate(P<0.05).There was no remarkable difference between the high does group and the control group(P>0.05).The Black Dihuang Pill of high does can improve the level of SS in blood (P<0.05).There was no remarkable difference between the high does group and the control group (P>0.05).(4)The Black Dihuang Pill of high does can increase the expression of Ghrelin and SS in stomach and duodenum tissue(P<0.05).There was no remarkable difference between the high does group and the control group (P>0.05).(5)The Black Dihuang Pill of high does can increase the expression of Ghrelin mRNA in the hypothalamus (P<0.01).There was no remarkable difference between the high does group and the control group. (P>0.05).The Black Dihuang Pill of high and low dose groups can increase the expression of SS mRNA in the hypothalamus (P<0.01 or P<0.05).The high does group was better than the control group (P<0.05).(6)The Black Dihuang Pill of high and low dose groups can improve the content of Ghrelin in the hypothalamus (P<0.01 or P<0.05),which are better than the control group (P<0.05).The Black Dihuang Pill of high and low dose groups can improve the content of SS in the hypothalamus (P<0.01 or P<0.05) without statistical significance than the control group (P>0.05).Conclusion:The Black Dihuang pill has the functions of improving CRF spleen deficient syndrome,reducing Scr, improving Hb、ALb TP,reducing Ghrelin content,increasing SS content,improving the weight,the average intake and the swimming time of rats,reducing proteinuria,increasing the level of Ghrelin and SS in stomach tissue homogenate,reducing the level of Ghrelin in blood,improving the level of SS in blood and incraesing the expression of Ghrelin and SS in the hypothalamus.According to the regulation to the brain-gut axis,The Black Dihuang Pill plays a significant role on spleen deficient syndrome in chronic renal failure.
