| Irritable bowel syndrome (IBS) is one of the most common clinical chronic functionaldiseases, with increasing trend year by year, and can cause serious harm to the patient’sphysical and mental health. The currently treatment for IBS is mainly symptomatictreatment, but the effect is far from ideal. There are lots of clinical reports of acupuncturetreatment of IBS and its efficacy is recognized by the majority of patients, however themechanism is still unclear. Therefore, investigation of the mechanism of acupuncturetreatment of IBS possesses a very important significance for the treatment and preventionof IBS.Objective:To observe the effects of electro-acupuncture on visceral hypersensitivityin rats with C-IBS and D-IBS and to observe the effects of electro-acupuncture on CGRPand SP contents in plasma in these rats. To study the influence of electro-acupuncture onthe expression of TRPV1, PAR4, CGRP, SP from the view of mRNA and protein. Todiscuss the mechanism of electro-acupuncture’s regulation effects.Methods:(1)The C-IBS model was induced in rats with gastric instillation of0~4℃cool water daily for14days. The D-IBS model was induced in rats with colonic instillationof acetic acid and by restraint stress.(2)Pain threshold was used to indicate visceral hypersensitivity to evaluate diseasemodel, the stool consistency and stool water were graded to evaluate"symptom" model.(3)The pain threshold of colorectal distention to represent visceral hypersensitivitywas used to evaluate the therapeutic effect of electro-acupuncture.(4) The amount of CGRP and SP in plasma were detected by ELISA.(5) Levels of TRPV1, PAR4, CGRP, SP mRNA in colon tissues was examined byreal-time fluorescent quantitative PCR.(6) Levels of TRPV1, PAR4, CGRP, SP protein in colon tissues was examined using western blot. The distributions of TRPV1, PAR4, CGRP, SP in colon tissue were detectedwith method of immunohistochemistry.Results:(1)There was statistically significant difference between the C-IBS andD-IBS model in stool consistency classification and stool water content compared with thenormal group. The pain threshold in different groups was significantly different, painthreshold of the C-IBS and D-IBS rats were significantly declined compared with thenormal group. Histopathological change in each group showed no significant changes.(2) Giving electro-acupuncture treatment for seven days, there were statisticallysignificant differences in pain threshold between groups. Compared with the normal group,pain threshold of the model group was significantly decreased. Compared with the modelgroup, the pain threshold of electro-acupuncture treatment group was significantlyincreased.(3) The content of plasma CGRP and SP were significantly ascend in modelgroup(P<0.01, P<0.01) Compared with the model group, the content of plasma CGRP andSP was significantly descend (P<0.05, P<0.01) in EA Shangjuxu group, the content ofplasma CGRP and SP was significantly descend (P<0.05, P<0.01) in EA Dachangshugroup.(4) After the electro-acupuncture treatment,the levels of TRPV1, PAR4, CGRP, SPmRNA in colon tissues were significantly different.Compared with the normal group,TRPV1, PAR4, CGRP, SP mRNA expression level in the model group significantly wentup. Compared with the model group, TRPV1, PAR4, CGRP, SP mRNA expression levelsof EA Dachangshu group and EA Shangjuxu group were descend.(5) In the end, the levels of TRPV1, PAR4, CGRP, SP protein in colon tissues weresignificantly different.Compared with the normal group, TRPV1, PAR4, CGRP, SP proteinexpression level in the model group significantly went up. Compared with the model group,TRPV1, PAR4, CGRP, SP protein expression levels of EA Dachangshu group and EAShangjuxu group were descend.Conclusion:(1)Two types of IBS were successfully established. As the C-IBS modelwas induced by gastric instillation of cool water when the D-IBS model by colonicinstillation of acetic acid and by restraint stress.(2)Electro-acupuncture can efficiently increase sensation pressure thresholds to rectaldistention in visceral hypersensitivity rats. Electro-acupuncture can relieve the abdomendiscomfort and pain of C-IBS and D-IBS.(3)The levels of values of plasma CGRP and SP in irritable bowel syndrome model rats were abnormal, leading to imbalance of these twoelements, and this may be one of the mechanisms which cause irritable bowel syndrome.Electro-acupuncture cure irritable bowel syndrome by adjusting the disorder of these twoelements.(4)The levels of TRPV1, PAR4, CGRP, SP protein in colon tissues were significantlydescend after electro-acupuncture treatment.Above all, electro-acupuncture can effectively adjust gastrointestinal functions,reduce visceral sensitivity and alleviate pain, thus improve constipation and diarrhea IBSclinical syndromes. The changes TRPV1, PAR4, CGRP, SP may be one of the mainmolecular mechanisms of electro-acupuncture treatment for irritable bowel syndrome. |
