The Study Of Best Effective Protocol For Dysphagia After Stroke By Acupuncture

Objective: To study the factors of affecting the curative effects of acupuncture forpseudobular palsy dysphagia （shortly called PBP below） with orthogonal design,and the mechanism of acupuncture treatment for it.Method: With the clinic 81 inpatients of PBP dysphagia after stroke, the patientswere randomly departed into three groups according to orthogonal design L₉（3⁴）. Thatis acute stage, convalescence stage and sequela stage. Each group contains 8 patients.Meanwhile, study the stage of acupuncture, acupoint selection method, quantity ofstimulus and course of treatment, screening the best treatment method.Result: Acupuncture is valid for dysphagia, in four factors, the role of the stage ofacupuncture is most important. Their effect are of significant difference for differentlevels. As for the stage of acupuncture, the effect of acute stage is better thanconvalescence stage, and better than sequela stage. For acupoint selection method,prescription 1 is better than prescription 3 and prescription 2. For quantity of stimulus,moderate stimulation is better than subexcite and heavy stimulation. As for course oftreatment, 20 days treatment is better than 30 days and 10 days treatment. And fromthe smaller amounts of samples of PBP patients, it reveals that hemodnamics is in thestate of high glue, thick and conglomeration before the treatment,its main index signsare obviously lower by acupuncture treatment, suggesting acupuncture can apparentlyreduce the lung infection and malnutrition etc. complicated with PBP patients.Acupuncture can reduce MDA and fibrinogen, increase SOD.Conclusion: Orthogonal design is an objective way and method very worthy ofspreading in acupuncture clinic research. Acupuncture is a very promising therepyfor treating dysphagia. Mechanism of acupuncture treatment for dysphagia may beprofound. Analyzed from the angle of nerve reflect arc, dysphagia complicated withPBP are in essence due to the obstacle in one of taches of nerve reflect arc.Acupuncture can make the destroyed nerve reflect arc reestablished, then restore thefunction of paralyzed nerve. Analyzed from the braim blood circulation, acupuncture decreases the states of high glue, thick and conglomeration of blood, and improvethe microcirculation obstacle and metabolism, make the ischemic cerebral districtand its "half dark zone "get valid perfusion, and secure sufficient supply of nutrientssuch as oxygen and glucose, etc, then promote the restoration of nerve functionto the biggest limit, make the symptoms of dysphagia patients improved.
