Objective To investigate the expression of Sox9 and Runx2 of articular cartilage in different Outerbridge grades from the patients with osteoarthritis, and explore their relationship in the osteoarthritis process and their clinical significance.Methods Collected male patients were divided into four groups, which A group: 10 patients undergoing total hip replacement due to femoral neck fracture whose articular cartilage was in outerbridge 0 grade; B group: 10 patients undergoing total knee replacement due to osteoarthritis whose articular cartilage was in outerbridge 1 grade; C group:10 patients undergoing total knee replacement due to osteoarthritis whose articular cartilage was in outerbridge 2 grade; D group:10 patients undergoing total knee replacement due to osteoarthritis whose articular cartilage was in outerbridge 3 grade. Collected female patients were divided into four groups, which A group: 10 patients undergoing total hip replacement due to femoral neck fracture whose articular cartilage was in outerbridge 0 grade; B group: 10 patients undergoing total knee replacement due to osteoarthritis whose articular cartilage was in outerbridge 1 grade; C group:10 patients undergoing total knee replacement due to osteoarthritis whose articular cartilage was in outerbridge 2 grade; D group:10 patients undergoing total knee replacement due to osteoarthritis whose articular cartilage was in outerbridge 3 grade. The morphological changes of articular cartilage were observed by HE straining. Extract total RNA and total protein from the articular cartilage for RT-PCR, and WB were performed to detect the changes of Sox9 and Runx2 genes expressions and proteins synthesis inside the articular cartilage.Results Articular cartilage in outerbridge 0 grade appears light white, which HE straining appears uniform cells distribution. Articular cartilage in outerbridge 1 grade tarnishes, which HE straining appears nonuniform cells distribution. Articular cartilage in outerbridge 2 grade has less 50% defect and ulcer, which HE straining appears broken tidemark. Articular cartilage in outerbridge 3 grade has more 50% defect and unvisible subchondral bone exposed, which HE straining appears less cells、a larger range of fibrosis.and increased cell density. The mankin score of articular cartilage from male patients in group B、C and D is 3.20±0.79、6.90±0.74 and 11.30±1.34 respectively. There are obvious statistical difference among them. The mankin score of articular cartilage from female patients in group B、C and D is 3.50±1.08、7.50±1.18 and 12.20±1.14 respectively. There are obvious statistical difference among them. The gray level ratio between Sox9 andβ-actin of articular cartilage from male patients in group B、C and D is 1.25±0.12、0.64±0.13 and 0.37±0.09 respectively. There are obvious statistical difference among them. The gray level ratio between Sox9 andβ-actin of articular cartilage from female patients in group B、C and D is 1.21±0.07、0.57±0.49 and 0.33±0.46 respectively. There are obvious statistical difference among them. The gray level ratio between Runx2 andβ-actin of articular cartilage from male patients in group B、C and D is 1.74±0.13、2.10±0.03 and 2.22±0.02 respectively. There are obvious statistical difference among them. The gray level ratio between Runx2 andβ-actin of articular cartilage from female patients in group B、C and D is 1.66±0.13、2.02±0.06 ' 2.14±0.10 respectively. There are obvious statistical difference among them. The Sox9 m RNA expression of articular cartilage from male patients in group B、C and D with respect to group A is 0.810±0.081、0.408±0.075 and 0.143±0.068 respectively. There are obvious statistical difference among them. The Sox9 m RNA expression of articular cartilage from female patients in group B、C and D with respect to group A is 0.772±0.049、0.362±0.053 and 0.103±0.038 respectively. There are obvious statistical difference among them. The Runx2 m RNA expression of articular cartilage from male patients in group B、C and D with respect to group A is 1.120±0.048、1.233±0.063 and 1.356±0.032 respectively. There are obvious statistical difference among them. The Runx2 m RNA expression of articular cartilage from female patients in group B、C and D with respect to group A is 1.074±0.039、1.183±0.054 and 1.306±0.047 respectively. There are obvious statistical difference among them.Conclusion With Outerbridge grade of cartilage of osteoarthritis patients gradually increased, cartilage cells lossed gradually, the level of Sox9 expression decreased significanly, the level of Runx2 expression increased significanly.
