| ObjectiveAfter nearly 50 years of studying, exploration and practice, Professor Shen Ying-sen’s distinctive clinical experience and academic thoughts had been formed, especially as an advocate and practitioner of Lingnan TCM. In order to better highlight the clinical experience and academic thoughts, we summarized Professor Shen’s academic thoughts, dialectical thinking, clinical experience, regularity of drug use and the way of academic inheritance. We excavated, sorted, summarized relevant literature, basis, manuscripts and other materials and systematically summarized Professor Shen Ying-sen’s drug use of lingnan features and clinical experience with the purpose of inheriting Professor Shen Ying-sen’s academic thoughts. Through the standardized clinical research, we summarized Professor Shen regularity of drug use, in order to explore the way of inheriting the academic thoughts and experience of the distinguished veteran doctors of TCM, so as to effectively attaining the goal of inheriting TCM.Methods1. We explored Professor Shen’s academic thought by the perspective of heritage. Combined with construction work of THE HERITAGE STUDIO OF THE DISTINGUISHED VETERAN DOCTERS OF TCM PROFESSOR SHEN YING-SEN, in the base of Professor Shen’s clinical and diagnostic work, we sorted out the clinical data by category, including dictation, Professor Shen’s works had been published, papers and academic meeting notes, clinical cases, case reporting, individual reports and other information, in addition incorporating the papers and clinical cases of Professor Shen’s students in regard to Professor Shen’s clinical experience and academic thought. So that we could summarize the parts of Professor Shen’s clinical experience and academic thought which express the feature of Lingnan TCM.2. We retrospectively analyzed the regularity of drug use of Professor Shen treating aphthous ulcer. The prescribing informations which were diagnosed of "aphthous ulcers" or "oral ulcer"by Professor Shen from January 1,2011 to December 31,2013 in Jiangmen Wuyi hospital of TCM were exported from outpatient clinic HIS system database by workers in the information center in the form of Microsoft Excel. The related information of the selected formula, such as Patients’ general information, composition, drug and dosage, were established into a data table on Microsoft Excel. All data had been analysized, in oder to explore the regularity of drug use on Professor Shen treating aphthous ulcers patients.3. Discuss on Professor Shen treating aphthous ulcer with the theory of nourishing Yin by clinical study. Observating the clinical effect of Professor Shen’s empirical formula "Xuan-Dong Stomatitis Oral Liquid" treating 141 patients with ulcers with randomized, double-blind clinical research methods, using the "Kangfuxin Oral Liquid" as the control. We observed before and after treatment in patients with pain index, ulcers, duration, recurrence, and recurrence time indicators, and analyzed the clinical effect.Ach i evements1. The academic thoughts and clinical experiences of Professor Shen.Five books,89 works,592 basis,13 lectures and 6 master’s thesis about Professor Shen’s clinical experience, my notes which I learned to Professor Shen and other data were collected and analyzised, in order to sort out the clinical experience and thoughts with characteristic of Lingnan TCM of Professor Shen. Those include as the fellow:(1) Professor Shen’s academic thoughts of TCM, including the definition and understanding of Lingnan TCM, the clinical viewpoint of spleen and stomach, nourishing Yin thoughts, and etc. Those thoughts gave expression to the characteristics of Lingnan TCM. (2) We summarized and clinically developed Professor Shen’s viewpoint of spleen and stomach in treatment of the diseases largely occuring in Lingnan and the clinical experience with the thoughts of protecting Qi and Jin and reinforcing Qi and nourishing Yin. (3) Professor Shen’s experience using TCM materials with characteristics of Lingnan TCM, including medicine principle, the technique about using the Lingnan materials, couplet materials and triangle materials. The empirical formula prescriptions of Professor Shen, which are 12 in total, including "Xuan-Dong Stomatitis Oral Liquid", "Yangwei Decoction", "Five Nephritis Decoction", "Stimulated Decoction", "Infantile Anorexia Prescription", "Ascaridole Decoction", "Add and Subtract Jin-shui-liu-lun decoction", "Add to Yigong San Powder ".2. The prescription regularity of treating aphthous ulcer.173 prescriptions conforming to the requirements were analyzed which involving 101 TCM materials. The frequency of all kinds appearing in prescriptions is 1913 times. The core drugs and their frequency are as fallows: Scrophularia ningpoensis Hemsl shows up 173 times (100%), Ophiopogon japonicus shows up 173 times(100%), Anemarrhena asphodeloides Bunge shows up 173 times(100%), Gypsum shows up 148 times(85.55%), Rehmannia glutinosa Libosch shows up 144 times (83.23%), seeds of Setaria italica(L.)Beauv shows up 104 times(60.12%). According to the analysis of the usage amount of core drug, we can make a conclusion of Professor shen’s basic prescription to treat canker sores:Scrophularia ningpoensis Hemsl 10g, Ophiopogon japonicus 10g, Anemarrhena asphodeloides Bunge 10g, the bark of Phellodendron chinense Schneid 10g, Gypsum 15g, Rehmannia glutinosa Libosch 10-15g, seeds of Setaria italica(L.)Beauv 30g.3. The results of the study of "Xuan-Dong Stomatitis Oral Liquid" treating aphthas ulcer patiengts.The observation have shown "Xuan-Dong Stomatitis Oral Liquid" can effectively control the aphthous ulcer surface pain symptoms, shorten the course of the disease. And its working time and pain index felling is obviously better than "Kangfuxin Oral Liquid" (p<0.01), which indicts that "Xuan-Dong Stomatitis Oral Liquid" does have well clinical therapeutic effect. The recurrence rate is significantly lower than "Kangfuxin Oral Liquid" (p<0.05), and the recurrence time is longer than "Kangfuxin Oral Liquid", which indicts that "Xuan-Dong Stomatitis Oral Liquid" also has obvious advantage over "Kangfuxin Oral Liquid" on long-term effect on relapse prevention. The difficulty about aphthous treatment is not short-term healing wounds, but lies in how to reduce the recurrence, and because of that, the effect of "Xuan-Dong Stomatitis Oral Liquid" is all the more remarkable.ConelusionProfessor Shen is not only the presenter of the concept of Lingnan TCM, but also a representative and practitioners of Lingnan TCM thought. As a urger, Professor Shen advocated the Lingnan TCM thought in "Lingnan TCM" which was written by himself in 1989, and practiced in his clinic as well. The viewpoint of spleen and stomach, the thoughts of protecting Qi and Jin and reinforcing Qi and nourishing Yin which Professor Shen uses to treat diseases largely occur in Lingnan are practical and can be extensively used.As the guide of nourishing Yin thought, Professor Shen often treat aphthas ulcers by nourishing Yin. By analyzing, we can make a conclusion on the basic prescription by which Professor Shen treat aphthas ulcer:Scrophularia ningpoensis Hemsl 10g, Ophiopogon japonicus 10g, Anemarrhena asphodeloides Bunge 10g, the bark of Phellodendron chinense Schneid 10g, Gypsum 15g, Rehmannia glutinosa Libosch 10-15g, seeds of Setaria italica(L.)Beauv 30g. This basic prescription in accordance with "Xuan-Dong Stomatitis Oral Liquid". So we can infer that Professor Shen’s thought of treating aphthas ulcers by nourishing Yin was derived from clinic and tested by clinic.According to the the result of the clinical study, "Xuan-Dong Stomatitis Oral Liquid" significantly relieved symptoms of patients with aphthas ulcers, shoot the duration of symptoms, reduced recurrence, all of which confirmed Professor Shen’s thoughts of treating aphthas ulcers by nourishing Yin. The causes of aphthas ulcers include a variety of pathogeny such as damp-heat in the stomach and intestine, the insidious fire in the Spleen and stomach, deficiency of Qi and blood, fire excess from Yin deficiency, deficient cold and Yang floating, and etc. But the key causes of aphthas ulcers is depletion of fluid which results in the pathology of the water can not control the fire. So treating aphthas ulcers by nourishing Yin are practical and can be extensively used. |
