ObjectiveThe aim of the study was to explore the TCM syndrome on chest stuffiness and pains in the Hong Kong Region and the clinical effect of Blood-activating and Heat-Clearing Decoction, in order to provide a theoretical basis and clinical evidence for Chinese medicine curing Coronary Heart Disease.MethodsThis study were consist of retrospective investigation and prospective observational study. The patient of retrospective investigation was collected from An Sheng and You Hao clinic in HongKong Region from the period of Jan 2012 to May 2014. We collect and analysis the regulation of TCM syndrome in our clinic according to differentiation standard of chest stuffiness and pains. The chest stuffiness and pains patient of the prospective observational study was collected from An Sheng and You Hao clinic in HongKong Region from the period of July 2014 to December 2015. The patients were divided in the heat-clearing group (basic therapy of western medicine plus Blood-activating and Heat - clearing Decoction, A group,75 cases)and non-heat-clearing group(basic therapy of western medicine plus Xuefu Zhuyu Decoction, B group,75 cases). After 4 weeks of treatment, we record the score of the TCM symptoms, curative effect, Seattle Angina Questionnairethe.Results1. There were 322 patients enrolled in this study, The frequency distribution of syndromes are mostly concerning heat toxin and blood stasis(55 cases,25%), heart blood stasis(40 cases,18.2%), Phlegm turbid-blood stasis(39cases,17.7%). The blood stasis syndrome mostly combine with other syndrome such as qi deficiency,qi depression, heat toxin,cold coagulation and so on.2. Baseline data analysis:the heat-clearing group:43 males and 32 females, in the age of 53.86±6.28 years, course of disease (4.21±2.21) years;non heat-clearing group:40 males and 35 females, in the age of 54.78±5.31 years, course of disease (3.89±1.79) years. There is no difference in gender, age, course of the disease between these two groups.3. The overall effect of the two groups:For A group,42 patients have responded satisfatorily,21 patients produecd effect,12 patients have failed to respond to the treatment, the total effective rate is 84%. For B group,31 patients have responded satisfatorily,21 patients produecd effect,23 patients have failed to respond to the treatment, the total effective rate is 69.3%. The total effective rate difference between two groups are statistically significant;4. The total score of symptoms before and after these treatment:The total scores of symptoms before these treatment in A proup and B group were 28.21+4.61 and 29.32±6.21, Which showe no difference. The total scores of symptoms after these treatment in A group and B group were 13.11±4.21 and 15.56+5.33, which showed the total score of symptoms in A group is better than that of B group and there is statistically significant, P<0.05, betweenA group and B group.5. The analysis of the main symptoms indicators before and after these treatment:Before the treatment, the symptom index such as chest pain, chest stufly, palpitation, shortness of breath, fever, dry mouth, bitter mouth, thirst-hicold, irritability, short red urine and stool dry showed no difference between two groups(P>0.05). After 4 weeks treatment, these symptoms such as chest pain, chest stufly, palpitation, fever, dry mouth, bitter mouth, short red urine and stool dryin A group relieved more that that in B group with statistical significance(P<0.05).6. The analysis of the Seattle Angina Questionnaire before and after these treatment:Before the treatment, the score of the SAQ showed no difference between two groups(P>0.05). After 4 weeks treatment, there was significant difference of physical activity limitation and angina stability status(P<0.05),A group were better than in B group. There were no difference in Angina pectoris.satisfaction with treatment and knowledge of the disease situation between two groups after treatment(P>0.05).ConclusionHeat toxin and blood stasis, Phlegm turbid-blood stasis were common in chest stuffiness and pains patients in some parts of HongKong region. Heat-clearing Decoction could improve TCM overall effect, TCM symptoms and the quality of life of patients with coronary heart disease. There were no adverse effects in treatment.
