Background:Professor Min Fanzhong is a professor of the second grade, a chief physician, a prominent TCM doctor in Guangxi, and among the fifth batch of TCM experts at the Guangxi Traditional Chinese Medicine University. Professor Min has been practicing in clinics, teaching and doing research for more than fifty years. He has vast clinical experiences in consumptive diseases, such as spleen-stomach weakness syndrome, hematologic diseases and emotional disorders, and so on, and has established his own unique academic ideas.Multiple myeloma (MM) is a malignant tumor of the plasma cells. Myeloma cells proliferate in the bone marrow, which induces osteolytic bone destruction, secretes monoclonal immunoglobulin, and inhibits normal polyclonal immunoglobulin protein synthesis, and is often associated with various damages caused by anemia, renal failure and myeloma cell extramedullary infiltration. Among them, myeloma bone disease (MBD) is the most common complication:up to 80% of MM patients have bone related diseases, including extensive osteoporosis, osteolytic destruction, pathological fracture and nerve compression, which seriously affect patients’quality of life and shorten survival. Currently modern medicine does not provide ideal treatments for MM and MBD because of the difficulty of controlling all of the symptoms, significant side effects, the high costs of medicines, etc. TCM treatment offers an alternative with the overall conditioning, smaller side effects, lower costs, good patient adherence, and other advantages.Objective:1. Comprehensively and systematically mining, sorting, summarizing, and inheriting Professor Min’s academic thoughts and clinical experiences, improving the theory of TCM and TCM diagnosis technologies, and mastering Professor Min’s skills.2. Through the observation of Professor Min Fanzhong’s prescription of clinical curative effects in the treatment of MM by Bu pi shen qu du decoction(BPSQDD), we preliminarily explore its clinical curative effect to provide a scientific basis for the clinical application, so as to more effectively understand and carry forward the academic thoughts and clinical experiences of Professor Min Fanzhong.Methods:Through organizing and studying classical TCM books and Professor Min Fanzhong’s academic works, workinging with outpatient doctors, making paternity copies, recording medical records, participating in centralized training of TCM, and independent clinical practice, we collect and collate prominent TCM doctors’ academic books, medical papers, and medical records etc., to mine, sort and summarize the academic thoughts and clinical experiences of Professor Min Fanzhong.We randomly divided 72 MM patients into two groups.36 patients were in the treatment group with VAD combined with BPSQDD (twice per day for 21 days, repeat once for every 4 weeks), and 36 in the control group. After 4 courses of treatment, we assessed the clinical efficacies, measured MM syndromes before and after TCM treatment, scored bone pain, KPS, β2-MG, TNF-α, etc., and statistically evaluated the results.Results:1. Through the studies of classical TCM theories and literature, we summarized the understandings and the academic view points of consumptive diseases by different physicians.2. Professor Min Fanzhong’s academic thoughts of differentiation and treatment of consumptive disease includes the following:(1) combing disease differentiation with pattern syndrome differentiation; (2) combing macrocosmic syndrome differentiation with microcosmic differentiation syndrome; (3) combing tonifying root of innate endowment with acquired essence; (4) combing TCM with traditional Zhuang medicine.3. Professor Min’s clinical experience of combined disease differentiation and syndrome differentiation in consumptive diseases includes the following three aspects:(1) treatment of spleen-stomach weakness syndrome:using the combined disease differentiation and pattern syndrome differentiation, the combined macrocosmic syndrome differentiation and microcosmic differentiation syndrome, and the combined tonifying the spleen Qi and moving Qi and substances.(2) clinical experience of hematopathy:Professor Min Fanzhong treated hematologic disease by combing disease differentiation and syndrome differentiation, paid attention to the replenishing conditioner of spleen and kidney deficiency. At the same time, Professor Min Fanzhong combined with the characteristics of Zhuang medicine in tonifying deficiency, and " Tonglonglu" and dispelling toxicity.(3) clinical experience of emotional abnormalities:encouraging the harmony of yin-yang, coordinating the heart and kidney and timing medicine.4. Independent clinical practices verified the effect of diagnosing and treating hematopathy using Professor Min Fanzhong’s academic thoughts.5. Clinical results:(1) comparing the gender,age,duration of disease,clinical atage,pathological typing and other informations, both groups were decreased without significant differences (P> 0.05).(2) comparing the clinical efficacies of the control and treatment groups, the total efficiency of traditional Chinese medicine treatment group was 77.14%, the control group 51.52%.The traditional Chinese medicine treatment group was better (P< 0.05).(3) comparing the TCM syndromes before and after treatment, the efficiency of the traditional Chinese medicine treatment group was 77.14%, the efficiency of the control group was 45.45%. The treatment group was better (P< 0.05).(4) comparing the accumulated points of TCM sign, the score of lassitude of spirit, lack of strength, anorexia, body magersucht, and pale complexion, all scores of both groups were decreased, while the scores of traditional Chinese medicine treatment group was significantly lower than that of the control group (P<0.05).(5) comparing the scores of fever, excessive thirst, and excessive thirst, both groups were decreased without significant differences (P>0.05).(6) both groups alleviated bone pain after treatment, while the pain score of the traditional Chinese medicine treatment group was significantly lower than that of control group (P< 0.05).(7) comparing the quality of life, the KPS score of treatment group was 82.86%, the control group was 48.49%. The traditional Chinese medicine treatment group was higher (P<0.05).(8) the P 2-MG levels significantly decreased in both groups after treatment (P< 0.05), and the P 2-MG levels in traditional Chinese medicine treatment group was lower than the control group (P<0.05).(9) the TNF-a level was decreased after treatment (P< 0.05), and the TNF-a level in traditional Chinese medicine treatment group was significantly lower than that in the control group (P< 0.05).(10) There are 10 cases of adverse reactions in the control group, the incidence rate is 30.3%; There are3 cases of adverse reactions in the treatment group, the incidence rate is 8.57%, side effects of the treatment group was significantly lower than the control group (P=0.031), and there is no other obvious chemotherapy-related toxicity, safe.Conclusion:1. Prominent TCM doctors’academic thoughts and clinical experiences are rich, so it is worthwhile for young doctors to study and learn.2. Through consolidating Professor Min Fanzhong’s academic works, studying with outpatient doctors, making paternity copies, summarizing his clinical experience, extracting the academic thoughts and clinical research, we can grasp Professor Min Fanzhong’s the academic idea summary And on the clinical experience of combined disease differentiation and syndrome differentiation in consumptive disease, and lay a solid foundation for the further inheritance of TCM expert Min Fanzhong’s legacies.3. Professor Min Fanzhong’s BPSQDD combined with chemotherapy significantly improves clinical curative effects, syndromes of TCM effects, life quality of MM patients, and also significantly decreases 02-MG and TNF-a levels. The effect is better than simple chemotherapy, without obvious toxic side effects.
