Objective1. By the literature research on law of warm yang and notify the kidney and Chinese medicine Chuankezhi (CKZ) with atopic dermatitis, they preliminary provided the theoretical basis of law of warm yang and notify the kidney and Chinese medicine Chuankezhi (CKZ) to treat atopic dermatitis.2. By using 2,4-dinitro-fluorobenzene (DNFB) induced method, we es-tablished the atopic dermatitis-like mouse model, we preliminary explored pathophysiological characteristics on atopic dermatitis-like mouse models.3. To explored the efficacy and mechanism of Chinese medicine Chuankezhi (CKZ) to treat atopic dermatitis, we observed the formation process of inflammation of DNFB-induced atopic dermatitis mouse model intervened by the Chinese medicine Chuankezhi (CKZ).4. By the retrospective study of atopic dermatitis of pattern of Chinese medicine classification, we evaluated the pattern of Chinese medicine of kidney yang deficiency of the adult patients of atopic dermatitis. We aimed to provide the evidence of efficacy of Chinese medicine Chuankezhi (CKZ) to treat atopic dermatitis.5. By the prospective, randomized, controlled study, we preliminary evaluated efficacy of Chinese medicine Chuankezhi (CKZ) to treat adult atopic dermatitis.Methods1. The research literature based on Pubmed database, Cochrane Library, Embase database, sinoMED database, CNKI database, VIP database, Wanfang Database, and we screened the literatures about the Chinese patent medicine to treat atopic dermatitis, and systematic literature analysis about law of warm yang and notify the kidney and atopic dermatitis. We also explored the contemporary dermatologist of Chinese medicine’s sayings about law of warm yang and notify the kidney to treat allergic and immunological skin diseases.2. Experimental study:1) We used 2,4-dinitrofluorobenzene(DNFB) induced by the C57BL/6 mice to established the atopic dermatitis-like mouse model and detected the clinical manifestations, skin pathology, serum IgE, IL-4, IFN-γ, lesions expression of microRNA-155.2) 2,4-dinitrofluorobenzene(DNFB) induced atopic dermatitis-like mice were randomly divided into control group, model group, Chuankezhi (CKZ) high dose group, Chuankezhi (CKZ) small dose group. After treated by the Chuankezhi (CKZ), we explored the clinical manifestations, under-dermoscope’s manifestations, back and ear skin common pathology, mast cell’s staining pathology, serum IgE, IFN-y, IL-4 levels, lesions IFN-y, IL-4 mRNA expression, and we evaluated efficacy of Chuankezhi(CKZ).3. Clinical Research:1) We started a retrospective study about 314 cases of atopic dermatitis patients, and we evaluated the general condition, clinical characteristics, type of Chinese medicine.2) We started a ret-rospective study about 28 cases of atopic dermatitis patients who treated by the Chinese medicine Chuankezhi (CKZ), and we evaluated SCORAD, investigator global assessment(IGA).3) We started a randomized, controlled trial about adult patients of atopic dermatitis who were randomly divided into treatment group (basic treatment + Chinese medicine Chuankezhi (CKZ)) and control group (basic treatment), and the observation period includes 4 weeks and 4 weeks of follow-up. We evaluated SCORAD, stratified analysis (severity of lesions, lesion area, pruritus, insomnia degree), investigator global assessment (IGA), intensity of intolerance of cold, risk of recurrence and serum IgE on the time of treatment before, the first 2 weeks,4 weeks and 8 weeks, respectively.Results1. The study found that the presence of literature about Chinese medicine Chuankezhi (CKZ) and law of warm yang and notify the kidney with atopic dermatitis, and the Chinese medicine Chuankezhi (CKZ) had the pharmacological effects of anti-inflammation and immunomodulation.2. By using 2,4-dinitrofluorobenzene (DNFB), we successfully established atopic dermatitis-like model. The model showed the erythema, dryness, scaling, crusting and other symptoms, and the biopsy showed spongisis dermatitis. We also found the serum IgE and IL-4 were increased, and mi- croRNA-155 in the skin lesions was increased.3. By the treated of Chinese medicine Chuankezhi (CKZ), the mouse model’sclinical symptoms and under dermsocpy symptoms were improved, the biopsy showed skin thickening were alleviated and the number of inflammatory cells decreased. We also found the number of mast cells, the expression of serum IgE and IL-4 and IL-4 mRNA in the lesions were decreased.4. By the clinical retrospective study, we found that patients with atopic dermatitis, especially adult atopic dermatitis patients, who frequently belonged to or accompany with the pattern of kidney yang deficiency, and after treated by the Chinese medicine Chuankezhi (CKZ), the patient’s SCORAD score, IGA score were decreased, the difference was statistically significant (P <0.05).5. The results of randomized, controlled study showed that Chinese medicine Chuankezhi (CKZ) could dereased the SCORAD score, stratified analysis (lesion area, the severity of skin lesions, pruritus, insomnia degree), IGA score, risk of relapse, compared to the control group, the difference was statistically significant (P<0.05). Besides, the intensity of intolerance of cold, quality of life, serum IgE of the patients were showed a slight downward trend, the difference was not statistically significant (P>0.05).Conclusion1. The results showed that Chinese medicine Chuankezhi (CKZ) and law of warm yang and notify the kidney to treat atopic dermatitis had the theoretical basis.2. The results showed that Chinese medicine Chuankezhi(CKZ) could in-hibited formation process of inflammation, and its mechanism may be played a role in the regulation of Thl/Th2 immune imbalance and suppression of mast cells.3. Chinese medicine Chuankezhi(CKZ) has the therapeutical effect on the improve patient’s condition, reduce recurrence, enhance the quality of life and other aspects of treatment.
