Objective:To study the clinical outcome of two methods of drainage after total knee arthroplasty and to determine the effect on blood loss、blood transfusion and other indicators. Methods:We retrospectively collected the clinical data of94patients who underwent total knee arthroplasty in the second affliated hospital of ZheJiang University school of medicine, the surgeries were performed during2014-8-1to2015-3-1by the same medical group. The patients were divided into two groups, everyone routinely used drainage after TKA, group A had44patients (including12males and32females), who used normal pressure drainage initially, then changed into suction drainage until it was removed, group B had50patients (including16males and34females), whose drain was clamped for4h postoperatively, after which it was released and kept suction drainage for the left time Patient’s demographic details, the amount of drainage fluid, blood transfusion, postoperative blood loss, and the proportion of infected cases and other related datas were recorded, all datas were analyzed by SPSS (version19.0) software. Results:In the group A the total postoperative drainage was171.30±112.86ml ml compared with286.10±167.49ml ml in the group B, and the difference is statistically very significant (p<0.01), also the0-6h drainage and6-12h drainage of group A were both less than group B (both P<0.01),while the12-24h drainage was similar(p=0.96).The blood loss in the first day of group A was less than group B(631±358ml VS843±509ml,P<0.01),however,the total postoperation blood loss in the3days between the two group have no significant difference (P=0.68).Other indicators like temperature, rate of infection cases and DVT cases were all similar (P>0.05)Conclusion:According to TKA patients, the drainage way of normal pressure+suction pressure can reduce the amount of postoperation blood loss, compared to the4h clamping drainage group, and it can also reduce the rate of blood transfusion, with the similarity of other indicators like the incidence rate of infection, DVT or other complications.
