Research On The Value Of Ankle-brachial Index And Fundus Arteriosclerosis In Predicting The Risk Of Ischemic Cardiovascular Disease In Chinese

Objective The aim of this study was to establish the relationship between ankle-brachial index(ABI), fundus arteriosclerosis and ischemic cardiovascular disease (ICVD), and to assess the the prediction and evaluation model for the patients with ICVD in our country based on the traditional risk factors and provide reliable data for further prospective research.Method A total of186in-patients with ICVD and113in-patients without ICVD in our hospital from December2011to March2013were enrolled in this study. There height, weight, blood pressure, smoking, fasting plasma glucose (FPG), blood lipids, ankle-brachial index (ABI), pulse wave velocity(PWV), fundoscopy and intimal medial thickness (IMT) were measured. Their diseases histories and smoking history were recorded. The differences in these two groups were analyzed.Results In patients with ICVD, the levels of age, BMI, cholesterol, low density lipoprotein cholesterol(LDL) and uric acid were higher than those in the control group. And except LDL, the differences of other indicators between the two groups had statistically significant. Compared with the control group, in patients with ICVD,the levels of systolic blood pressure (SBP), fasting blood glucose levels, homocysteine levels, C-reactive protein were all higher than those in the control group,and the ABI level was lower than the control group, and the differences all had statistically significant. In ICVD group, the patients with history of hypertension, diabetes, smoking and fundus arteriosclerosis were more than the control group. But the differences in the sex ratio and history of smoking between the two groups were without statistical significance. In the multi-factor Logistic regression analysis, the results showed that risk factors associated with ICVD including the age, history of hypertension, diabetes, cholesterol, uric acid, IMT, ABI and fundus arteriosclerosis (P<0.05). The area under receiver operator characteristic (ROC) curve(ACU) of the model was0.980,95%confidence interal=0.965-0.994(P<0.05).Conclusion ABI test and fundus examination might be important for ICVD prediction, then optimum and most convenient prediction model for ICVD in Chinese can be established based on the traditional risk factors.
