| Objective1.To evaluate the relationship of Yang with the skin temperature of the line of governor meridian by observing the difference of the skin temperature of the line of governor meridian between different ages and different sex people.2. To reveal the pathogenesis feature of "du Yang deficiency" of degenerative lumbar spinal stenosis by comparing the skin temperature of the line of governor meridian of general population and that of patients with degenerative lumbar spinal stenosis.3. To evaluate the clinical significance of "du Yang deficiency" as the basic pathogenesis of degenerative lumbar spinal stenosis by observing the clinical results of "Wen Tong du meridian method" for treatment of degenerative lumbar spinal stenosis.Methods1. Pathogenesis StudyFrom May 2014 to October 2014,180 people from the prevention and health section of the Third Affiliated Hospital of Guangxi University of TCM.75 years of age or older as a group, under the age of 75, for every 15 years old as a group. There were 30 cases in each age group. There were no acute disease at the time of test. Detect the skin temperature of the line of governor meridian of each subjects by Thermo Tracre 6T67. Then compare the skin temperature of the line of governor meridian between different ages and different sex people. Then, at the same time, compare the skin temperature of back du meridian line line between the general population and the 30 patients with degenerative lumbar spinal stenosis who from the same age group.2.Clinical StudyFrom June 2013 to June 2014,70 patients with degenerative lumbar spinal stenosis were treated. All of them were came from the Third Affiliated Hospital of Guangxi University of TCM.All cases were definitly diagnosised based on X ray, CT and MRI.70 cases were randomly divided into "Wen Tong Du meridian" treatment group and western medicine treatment control group. Eventually,a total of 69 cases were finished the study. The clinical efficacy, visual analogue scale, VAS, JOA score and the skin temperature of back du meridian follow line line were observed.Results1.Pathogenesis Study①The skin temperature of back Du meridian follow line line of general population was highest in the age group 0 to 14. And significantly higher than that of other age groups, the difference was statistically significant (p<0.05); The skin temperature of back Du meridian follow line line of general population of age group 15 to 29 was significantly low than that of 0 to 14 age group, but higher than that of 45 to 59 age group and 75 years of age or older age group of the general population, there were statistically differences, (p<0.05). The skin temperature of back Du meridian follow line line of general population of age group 15 to 29 was also higher than that of 30 to 44 years old age group and 60 to 74 years group, but there were no statistically differences, (p>0.05). The skin temperature of back Du meridian follow line line of general population of age group 30 to 44 was higher than that of 45 to 59 group and 75 years of age or older age group, lower than 60-74 age group, but there were no statistically difference between the groups, (p>0.05).The skin temperature of back Du meridian follow line line of general population of age group 45 to 59 was higher than an that of the group of 75 years of age or older, but lower than that of the 60-74 age group, but there were no statistically difference between the groups, (p>0.05).The skin temperature of back Du meridian follow line line of general population of age group 60-74 was significantly higher than that of the 75 years of age or older group, and there was statistically difference, (p<0.05).Visibly, as the growth of the age, the skin temperature of back Du meridian follow line line of general population of a downward trend. But the skin temperature of back Du meridian follow line line of general population of 60-74 was higher than that of the 30 to 44 group and that of the 45 to 59 group, but there were no statistically difference between the groups, (p>0.05). The skin temperature of back Du meridian follow line line of general population of age group of 75 years old and above was the lowest in all age groups.②Not as the traditional knowledge that women have a higher surface temperature than male, this research shown that the average male back Du meridian skin temperature was 32.04±3.47℃, higher than that of women’s 31.45 ±2.54℃, but there was no statistically difference, (p>0.05).③The skin temperature of upper back Du meridian follow line line of patients with degenerative lumbar spinal stenosis was 31.29±1.34℃, lower than that of the general populations 32.94±1.53℃, here was statistically difference, (p<0.051); The skin temperature of lower back Du meridian follow line line of patients of degenerative lumbar spinal stenosis was 31.69+ 1.54℃, lower than that of the general population s 32.81±1.55℃, here was statistically difference, (p<0.05).2. Clinical Study①Clinical EfficacyWith reference to "The Diagnosis of Disease and Curative Effect of Traditional Chinese Medicine standard", after treatment, the two groups of cases all achieved good result, but the clinical cure rate of the "Wen Tong Du meridian" treatment group was obviously higher than that of the control group, the differences between the two groups was statistically significant (P<0.05).②Visual Analogue Scale, VASAfter treatment, the VAS score of the two groups all improved significantly (P<0.05), but the VAS score of the "Wen Tong Du meridian" treatment group improved better than that of the control group (P<0.05).③JOA ScoreAfter treatment, the JOA score of the two groups all improved significantly (P<0.05), but the JOA score of the "Wen Tong Du meridian" treatment group improved better than that of control group (P<0.05).④The Skin Temperature of Upper Back Du meridian Follow Line LineThere were no difference of the skin temperature of upper back Du meridian follow line line between the two groups of the patients, (p>0.05). After treatment, the skin temperature of upper back Du meridian follow line line of the two groups of cases all improved (P<0.05),but the "Wen Tong du meridian" treatment group improved better than that of control group (P< 0.05).Conclusion1. The skin temperature of back Du meridian follow line line changes with the rise and fall of Yang. The skin temperature of back Du meridian follow line line of general population of age group 60-74 was higher than that of the group of 30-44 and 45-59, though there was no statistically difference, (p>0.05), but this "anomaly" phenomenon may infer that people of this age group became to lose balance of Yin and Yang, and Yang become floating.2. Male have an average higher skin temperature of back Du meridian follow line line, though there was no statistically difference, (p>0.05), but this "anomaly" phenomenon may also infer that the Traditional Chinese Medicine theory such as "Yang master wander", " Men to Yang" and "Du meridian is the sea of Yang pulse" have their objective facts.3. Compared with general population, patients with degenerative lumbar spinal stenosis had a lower skin temperature of upper and lower back Du meridian follow line line which infers that the "Deficiency of Du Yang" is the basic pathogenesis of the disease of degenerative lumbar spinal stenosis.4. The therapy of "Wen Tong Du meridian" in the treatment of degenerative lumbar spinal stenosis disease can obtain good clinical results. |
