Objective:To explore the traditional Chinese medicine and western medicine points of views in treating sequelae of pelvic inflammatory disease. To further clarify the mechanism of nourishing kidney and removing blood stasis to intervene fiber cells proliferation and differentiation, as well as the differences between nourishing kidney and removing blood stasis.Methods:This study cleared diagnosis and treatment present situation of pelvic inflammatory disease through literature research. According clinical research to observe traditional Chinese medicine treatment effects of nourishing kidney and removing blood stasis in improving female fertility with sequelae of pelvic inflammatory disease. In vitro experiments, NIH/3T3cells were incubated with different concentration of YSHY, YS and HY. Cell proliferation was detected by MTT method. Moreover, we investigated influences of different prescriptions for bFGF, TGF-β, HOP, extracellular matrix such as HA, type Ⅲ collagen, fibroblasts and its STAS6signal transduction pathway.Results:The clinical study indicated that the treatment group of nourishing kidney and removing blood stasis decreased VAS score (P<0.01), digestive system and the total rate of adverse reactions (P<0.05), meanwhile increased clinical pregnancy rate (P<0.05) significantly. Another study compared with nourishing kidney and removing blood stasis sequential group, separated removing blood stasis group, and control group. It found that the sequential group had a better total efficiency than control group (T<0.05). Disease rating scale (SIP) results showed that sequelae of pelvic inflammatory disease patients were suffered physical dysfunction, social and psychological damage. After three different treatments, each item scores decreased (P<0.05) in all groups, but the sequential group total scores and psychosocial scores decreased more significantly than the other two groups (P<0.01).In vitro experiments, it showed that the treatment of nourishing kidney and removing blood stasis could effectively inhibit the expression of fibroblast proliferation and cytokine, and reduce the production of HOP in a dose-dependent manner. Separated method of nourishing kidney or removing blood stasis reduced inhibitory effect of basic fibroblast proliferation compared with comprehensive treatment (P<0.05), but the latter effect degree was more than the former.Type Ⅲ collagen and HA expression were significantly decreased in all intervention groups (P<0.01), and comprehensive treatment were better than other two separated method (P<0.01). By comparing2-△△Ct method to analyze the expression of cell type Ⅲ collagen mRNA:the housekeeping gene β-actin as a reference gene by relative quantitative, calculate△△Ct values of type Ⅲ collagen mRNA. Target gene amount==2-△△Ct. Results showed that collagen type Ⅲ mRNA expression displayed a decreasing trend in all intervention groups compared with control group, especially in the group of nourishing kidney and removing blood stasis (P<0.01).There were significant differences of a-SMA and STAT6protein levels between groups. Whether α-SMA or STAT6protein expression decreased in above three groups respectively (P<0.01), and there was significant difference between any of the two groups (P<0.01).Conclusion:The treatment of nourishing kidney and removing blood stasis had an significant inhibitory effect on basic fibroblast proliferation of sequelae of pelvic inflammatory disease, which through inhibiting the expression of fibroblast proliferation related cytokines and extracellular matrix of collagen to realize. Compared with separated nourishing kidney or removing blood stasis, comprehensive treatment obtained the best effect with statistically significant difference. |
