Objective:Acute coronary syndrome (ACS) is a clinical complication of rupture or erosion of atherosclerosis in which lead to form secondary thromb completely or uncompletely within coronary lesions. inflammation is a major role.In addition to the immunoregulatoryproperties by CD40-CD40L interaction, evidence suggests that tissue factor is upregulated through CD40ligation by activated lympho-cytes and platelets.Level of sCD40L、Fas、IL-6、IL-10is associated with appearing and developping of CHD.Genetic gene have a key role in the onset of CHD.Domestic Have reported the relationship IL-6-572G>C SNP and genetic Susceptibility to Han people in china, not-174C>G. Many studies of abroad report about IL-6-174G>C and-572G>C SNP, but its result is not agreement.And so far, there are not reports about comparative study of CD40L, IL-6blood level and SNP between Uigur and Han population in Domestic.This study first part:Combined to the results of coronary angiography and contrastively investigate the association of plasma levels soluble CD40ligand (sCD40L), C-reactive protein (CRP), Interleukin-6(IL-6) with ACS;Second part Combined to the results of coronary angiography and contrastively investigate the association of single nucleotide polymorphisms of CD40L with ACS; Third part:Combined to the results of coronary angiography and contrastively investigate the association of single nucleotide polymorphisms of IL-6gene With ACS between Uigur and Han population of China. Methods:This study include two groups:one group is Uigur population, another is Han populations.Every one include110patient with ACS,40stable angina patient (SAP) and 40healthy controls.And All blood testing sCD40L, CRP, IL-6level by ELISA method, IL-6-572G>C,-174G>C and CD40L-3459AG (rs3092952) were analysised by Real-time PCR (A&B System). And combined the severity and extent of coronary lesions was analyzed by coronary angiography and typified by means of Gensini coronary score system. Results:(1) ACS patients group of Uygur CRP is (5.36±1.03) mg/L, sCD40L is (5.17±0.68) ng/ml, IL-6is (65.92±.52) ng/ml, ACS patients group of Han CRP is (4,85±1.11) mg/L, sCD40L is (4.58±0.56) ng/ml, IL-6is (64.55±1.06) ng/ml.All were increased significantly compared with control group.(All P<0.05). Uygur Compare with Han nationality, regulation of the3indexes is same, and there is positive correlation.Of the3indexes increased.(2) We identified a SNP in the CD40LG gene likely assoiated with Uigur and Han population of China.It likely assoiated with CD40L plasma levels;and maybe have no association with with the severity of coronary atherosclerosis.(3) IL-6-572G>C are likely to assoiated with Han ACS patient, and it is positively associated with IL-6plasma levels and for the associated haplotype.The IL-6-174G>C is likely associated with Uigur ACS patient, and it is positively associated with IL-6plasma levels, and maybe have association with the severity of coronary atherosclerosis.(3) Multivariate logistc regression analysis showed that TC, Lp (a), age, TG sex, high blood pressure were risk factors for ACS.There are risk factor of ACS with IL-6-572G>C in Han people and with IL6-174G>C in Uigur people. The CD40LG gene is likely risk factors of Uigur and Han population of China. Conclusion:(1)The blood level rising of sCD40L, CRP、IL-6in Uygur and Han nationality is independent risk factors of acute coronary events, there is all closely related to the development with the occurrence of acute coronary syndrome. There was a good correlation between the3indexes.Uygur patients with acute coronary syndrome in these three indicators has increased slightly, than Han patients, but no statistical significance.(2)We identified a SNP in the CD40LG gene likely assoiated with SAP, ACS patient; and assoiated with rising the level of CD40L in Uigur and Han population of China; maybe have association with the severity of coronary atherosclerosis.(3)IL-6-572G>C (rs1800796) are likely to assoiated with Han ACS patient, and maybe have association with the severity of coronary atherosclerosis.The IL6-174G>C (rs1800795) may be likely associated with Uigur ACS patient and rising the level of IL-6; with the severity of coronary atherosclerosis.(4)Multivariate logistc regression analysis showed that TC, Lp (a), age, TG sex, high blood pressure were risk factors for ACS.There are risk factor of ACS with IL-6-572G> C in Han people and with IL6-174G>C in Uigur people.
