ObjectiveOwing to increase the development of business and industry society, large numbers of patients who suffered from chronic eczema rose gradually and rapidly in the world. The chronic eczema was a delayed allergic dermatitis inflammation; it was etiologic factor of disease-producing included internal factors and external factors. The morbidity of chronic eczema was increased in recent years; the series of serious symptoms of chronic eczema was appeared on local lesion and the whole body. The mainly manifested symptoms of chronic eczema were included eruption to form a highly erratic pattern, like a green bean or cone size, ruddy pimple, maculopapular eruption, infiltration pachismus, lichenoid dermatitis, on the surface area of skin with branny desquamation, scratch mark, rhagadiform skin, cicatrization, unclear edge of skin lesion, papulo-vesicle, topical disease of popular with symmetrical distribution on the extremities internal region, besides, the another region of human body also easily was suffered from chronic eczema, such as sexual organ eczema, as well as anus eczema. The course of chronic eczema had run different courses and usually repeated attack, it was difficult to cure and relieve, the patients suffered from chronic eczema to feel much itch, and loss their self-confidence with friends, to be affected the attention of learning as well as working, in recent years,many enthusiastic practitioners hoped to find the best way to solve these problems and they tried their best to find the method of traditional Chinese medicine as well as acupuncture therapy with safety, no side-effect, no drug resistance and dependence, and the practitioners hoped the patients would have a new life style again.In the study object in this thesis was to be evaluated clinical observation for the therapeutic efficacy of acupuncture combined with medicinal granules-infusion of Chinese herbs to treat chronic eczema. The characteristic of Mr. Chen’s Fly needle, which was the extremely skilled method of self-creating, the method of placing the acupuncture needles could release pricking with expert hands to swift needle into acupuncture-points on the surface of the skin during acupuncture, then, the practitioner used finger-manipulation of Mr. Chen’s fly needle to make a acupuncture point-stimulation to produce qi-promoting as the patient was on the period of retain acupuncture needle, as post needling, the practitioner was used to manipulate the needle to make a strong arrival of qi to tend to the area of focus lesion, at the same time, the practitioner also used the method of three grades of manipulation of reinforcement and reduction or even reinforcing and reducing mild reinforcing and attenuating acupuncture, or reinforcing with puncturing along channel course and reducing with puncturing adverse to the channel course. It was in order to adjust the viscera as well as recuperation for regulate and nourish qi and the blood. We had some questionnaire about the eczema area and severity index (EASI), Visual analogue scale (VAS), the quality of life scale for chronic eczema patients-prior test version (EPQOLS), the therapeutic effect of skin lesion in purposes to determine and evaluate the result of the treatment, and we assessed the condition of pathological reaction of dermatitis. The purpose of this investigation was to hope to find out the way of the effect of treatment with a short time, to decrease the rate of repeat attack, and to advocate unity method of traditional Chinese medicine acupuncture therapy of curing a chronic eczema.MethodTo get through the symptoms by traditional Chinese medicinal discrimination, we chose the patients who suffered from chronic eczema of the symptoms complex of insufficiency of the spleen with abundance of dampness, and deficiency of yin with damp symptoms complex. Patients were randomly divided into a treatment group and a control group, the treatment group was treated by acupuncture combined with medicinal granules-infusion of Chinese herbs, the control group was treated by medicinal granules-infusion of Chinese herbs only. Both groups were asked the questionnaire about the tables of EASI, EPQOLS, VAS, and therapeutic effect of skin lesion to evaluate the efficacy. We chose to lift and thrust, twirl or rotate rapidly and manipulation-practicing for qi-promoting on the acupuncture point-stimulation which was Hegu, Quchi, Sanyinjao, Xuehai, Zusanli in the treatment group, at the same time, we inserted the needle into the muscle of the skin by using Mr. Chen’s fly needle, then we also used the method of three grades of manipulation of reinforcement and reduction of Mr Chen’s fly needle, or reinforcing with puncturing along channel course, and reducing with puncturing adverse to the channel course, then, we could handle the needles for emitting qi with finger-manipulation and manipulation-practicing to make stronger qi to the skin lesion, afterwards, the patients were retained the needles and waited for qi about30minutes during this period, we posted needle for qi-promoting with manipulation-practicing for every5min. The treatment was3times a week; the treatment was3weeks as a course. The treatment group was also to be taken medicinal granules-infusion of Chinese herbs which the method was as same as the control group. The control group was taken medicinal granules-infusion of Chinese herbs after being infused in warm boiled water, the medicinal cubes-infusion was used suitable warm boiled water to dissolve the medicinal granules-infusion about200ml at first, one dosage a day, the treatment was3weeks as a course, on the other hands,the function of the medicinal granules-infusion was for strength the spleen for excreting dampness and the function of the another medicinal granules-infusion was for nourishing yin and excreting dampness, the both medicinal granules-infusion was created by famous past practitioners by trial and error in order to cure the chronic eczema for many years ago, both groups were required to protect and kept on the skin surface moist, the chronic eczema patients should not touch all detergent, bath liquid, soap and so on, neither to be avoided taking a bath at the high temperature nor to have a bathing time too long. Both groups must be asked the questionnaire about the tables to estimate the effect by researchers before treatment a week until the3weeks as a course, the treatment group and the control group was made a therapeutic comparison after successive one course.ResultBy means of clinic observation, to explore of inquiry study paper, to examine the condition of skin lesion, to compare the grade of itching, we analysed that the efficacy difference between the method of acupuncture combined with medicinal granule-infusion of chinese herbs and the method of medicinal granule-infusion of chinese medicine, the treatment group not only was shown the best improvement the curative effect of chronic eczema, but also was shown that method had marked effects on chronic eczema and the result of investigation was strong proved that the method of treatment group had excellent effect for all symptoms and signs of disease, such as antipruritic action, release skin lesion, improved the quality of life. The treatment method should be suggested to use for clinical cure repeatly and stablely. We compared with both groups after treatment, we discovered that both groups could have effect of curative for chronic eczema but the treatment group had been combined with the acupuncture and medicinal granule-infusion of chinese herbs that was better than control group which was taken medicinal granule-infusion only, it was because acupuncture and medicinal granule-infusion of Chinese herbs was complementary to each other, the result of study was proved that the treatment group could more improve the curative effect for chronic eczema than the control group. we had a result from this study (I)the general data was included sex, age, course of disease of both groups, which were no significant difference (P>0.05), it meant that both groups were need to further to study to find their different effect between two groups,(II) in a view before treatment was shown that both groups were no significant efficiency (P>0.05) of table EASI, VAS, EPQOLS, and therapeutic effect of skin lesion, it meant that both group were worth of further to detect their different therapeutic efficacy between two group by statistical analysis,(Ⅲ)To compare with difference between before and after treatment, both groups were evaluated for the table of VAS, EASI, EPQOLS, skin lesion had significant that meant both groups could cure the chronic eczema and to be worthy of study.(IV) Although it was shown that both groups could improve the curative effect chronic eczema, cure-remarkable rate was over60%, the efficacy difference of both groups were significant efficiency(P<0.05) but the treatment group was higher cure-remarkable rate than the control group, the total effective rate of antipruritic action of the treatment group was96.87%, the cured rate of the treatment group was34.37%respectively; the total effective rate of skin lesion of the treatment group was93.75%, the cured rate of the treatment group was25%respectively; the total effective rate of EASI of treatment group was93.75%, the cured rate of treatment group was50%respectively. The total effective rate of antipruritic action of the control group was86.68%, the cured rate of the control group was10%respectively; the total effective rate of skin lesion of the control group was86.66%, the cured rate of the control group was10%respectively; the total effective rate of EASI of the control group was86.66%, the cured rate of the control group was13.33%respectively. The efficacy difference of VAS at the time of treatment two weeks later both groups had significant efficiency(P<0.05), the efficacy difference of EASI at the time of treatment one week later both groups had significant efficiency (P<0.05), the efficacy difference of EPQOLS at the time of treatment two weeks later both groups had statistically significant (P<0.05), the efficacy difference of psychology and physiology index of EPQOLS at the time of two weeks later both groups had statistically significant (P<0.05), the efficacy difference of social behavior index of EQOLS at the time of one week later both groups had statistically significant(α=0.05). As above the statistical analysis, it was accounted for the treatment group had not only therapeutic efficacy, therapeutic action, and also the course of cure was better than the control group, therefore, this method of treatment was recommended to use for clinical therapy.ConclusionThe study was proved that acupuncture combined with medicinal granules-infusion of Chinese herbs could relieve the condition of xerosis cutis, cutaneous pruritus, skin lesion eruption, erythema, the course of treatment in a short time, and it was proved that acupuncture and chinese herbs were complementary to each other, and it could solve many limited problems of chronic eczema. It made the decrease the grade of symptoms and signs of disease, physiology, psychology, social behavior and this method could improve the symptoms of insomnia, the quality of life, and it made the patients self-confidence of curative effect again. It provided another choice to cure chronic eczema that was worth of further investigation and promotion.Some lecturers stressed that the modern medical model was from the model of biology be changed to the model of biology-psychology-society and expressed that the disease-producing by physiology factor, psychology factor and social behavior factors which all factors must be considered. As the whole to say that it was because the patient of chronic eczema could not tolerate itch, they scratched the skin lesion and eruption, papulo-vesicle, papule, erythema, erythematous, maculopapule, lichenification branny desquamation and so on, furthermore, the chronic eczema was always recrudescence, the pathological reaction made that the patient often had loss self-confidence, anxious, insomnia, agitation, mania, it was because their face was appeared by many erythema, or maculopapular eruption, these factors made the patients were sick of the mental and intellectual simulation or some series symptoms. So the following day, we cure the chronic eczema be considered the patients’ psychology and physiology.
