Research On The Characteristics Hearing-Impaired Children's Voice And Its Evaluation Profile

Since2009, Chinese government has developed "The rescue rehabilitation projects for poverty deaf children with Cochlear Implant/hearing aid" and there are more than a million of hearing-impaired children received proper hearing compensation equipments such as Cochlear Implant or hearing aids. Adequately hearing compensated, these hearing-impaired children have received the needed healing in hearing and speech function, which has greatly improved their voice disorders and speech intelligibility. in order to improve their quality of life,they need high-quality voice to expess their requirement and feeling, so there are more and more people to pay attention to hearing-impaired children's voice.This dissertation researches on hearing-impaired children's voice first time, based on research at home and abroad,include the characteristics of the hearing-impaired children's voice, the voice function measurement system, the impact on voice from the form, result and time of hearing rehabilitation. The researcher selects "voice assessment" to analysis the stable fragment from the voice samples of hearing-impaired children, extraced acoustics parameters of FO,FOSD,jitter and NNE; selects "voice measurement" to analysis the stable fragment from the voice samples of hearing-impaired children, extraced acoustic parameters of OQ,SQ,AQ and STF to study the voice of the hearing-impaired children. The main contents of the study include:(1)Through the lab research of voice parameters contrast between the normal children's voice and the hearing-impaired children's voice,to explore the characteristics of objective acoustic parameters of the hearing-impaired children's voice; through the method of GRBAS, to explore the characteristics of subjective perception of the hearing-impaired children's voice; analysis the correlation between objective acoustic parameters and auditory impression.(2)to study the impact on voice from the form, result and time of hearing rehabilitation;(3)to discuss the use of the multi-parameter evaluation profile on assessment of the voice characteristics of the hearing-impaired children, and finally to exam the consistency between the multi-parameter evaluation profile and the traditional subjective perception evaluation, finally apply to the clinical.Through a series of experiments about the hearing-impaired children's voice, some results have achieved:(1) There is a very significant difference in FOSD,jitter, STF, significant difference in FO,SQ between normal hearing children and hearing-impaired children; the characteristics of objective acoustic parameters of the hearing-impaired children's voice is FO,FOSD,jitter,STF,SQ; the characteristics of subjective perception of the hearing-impaired children's voice is strain and roughness; there is high correlation with roughness is FO, FOSD,jitter, there is high correlation with strain is FO,FOSD,STF,SQ.(2) The form of compensation does not have a impact on FO(pitch); The form of compensation have a impact on FOSD, to combine jitter,we find hearing-impaired children with Cochlear Implant easier to have voice disorder with roughness; The form of compensation does not have a impact on STF; There is no significant difference in acoustic parameters of the best, lip-reading and normal hearing, it is illustrated they have similar vioce quality; There is a significant difference in FO,FOSD,STF between the better and normal hearing, There is a significant difference in FO,FOSD,jitter,SQ,STF between good and normal hearing, it is illustrated the better and good are easier to have voice disorder with high pitch,roughness,strain; The time of compensation does not have a direct impact on the pitch of the hearing-impaired children; the time of compensation has a direct impact on FOSD,jitter(roughess) of the hearing-impaired children,expecial the first year of compensation, is the key to hearing-impaired children's voice with roughness; the time of compensation has a direct impact on STF,SQ(strain) of the hearing-impaired children, it is easier to have voice disorder with strain with age, expecial the two or three years of compensation, it is easier to have voice disorder with strain.(3) The multi-parameter objective evaluation function for the roughness and tension of voice is set up. The consistency between the multi-parameter evaluation profile and the subjective perception evaluation on the roughness and strain groups of the hearing-impaired children is up to86%and82.1%. This assessment model for the clinic has obtained a better effect assessment and treatment monitoring.
