Study On The Effect And Mechanism Of Acupoint External Application On Airway Inflammation In Passive Smoking Rat

There is not a generally acknowledged standard in Airway inflammation disease at present,seem to involve bronchus asthma,chronic obstructive lung disease(COPD)and cystic fibrosis in the literature,more perhaps include diffuse panbronchiolitis and upper respiratory disease(chronic rhinitis, sinusitises),etc.Up until now,the scholar thinks the cause,regulation and control and consequence of Airway inflammation are all quite complicated, Its pathogenesis involves many kinds of cells,factor,medium,connect each other in series,interactive chain reaction,cause inflammation to sustain and reconstruction of organization.Chronic airway inflammatory disease is common disease and frequently-occurring disease in clinical practice, especially COPD become the main chronic disease and one of the death reasons in the global range already,and it has already become the 4th reason of causing death in the world at present.The World Bank and World Health Organization were expected COPD will become the fifth disease of largest burden in the whole world until 2020.Smoking is one of the main inducements of diseases such as chronic bronchitis,emphysema,COPD,etc.Though the harm mechanism of cigarette smoke smog to the lung organization structure has not been totally clear yet,but its mechanism's complexity is definite,including direct or indirect destruction to he lung organization structure for many kinds of compositions of the cigarette smog,inhibition of the immune function,the increased of susceptibility pathogenic microorganism,and the airflow obstruction,etc.A large number of clinic and experimental researches show in recent years,the TCM acupoint external application has unique curative effect to chronic airway inflammation,such as chronic bronchitis,emphysema, COPD,etc.Especially compared with Western medicine,there are obvious advantages in reducing the side effect,and preventing from recuring.This text has summed up the danger of environmental cigarette smog to the human body, has explained the modern medical understanding that cigarette smog caused the action mechanism of chronic airway inflammation,and summaried the mechanism of acupoint external application,and in an all-round way summarized the clinical and mechanism research of the treatment to chronic airway inflammation by acupoint external application.ObjectiveWe established chronic airway inflammation model by useing of passive smoking rat,ehoosed indexes correlated closely with chronic airway inflammation caused by passive smoking,such as the pulmonary pathology,the concent of anti-proteolytic enzyme,inflammatory cytokines,peroxidation product and the anti-oxidative enzyme activity,and detected their change after acupoint external application.Throngh the research we can study on the treatment function and mechanism of chronic airway inflammation after acupoint external application.Method32 SD rats were devided medially into four groups:blank group,model group, acupointâ… group,acupointâ…¡group.The rats of model group,acupointâ… group, acupointâ…¡group were treated with passive smoking.The rats of acupointâ… group,acupointâ…¡group were treated with placing Chinese medicine at the point of feishu,pishu for 5 hours or 10 hours after depilation.The treatment was done once every other day.After treatment we observed the situation of the bronchial pathological structure and inflammatory cell infiltration,and conclulate the score of airway inflammation;and determinate theα₁-AT,TNF-α,IL-8,MDA concentrations and the SOD activity in bronchoalveolar lavage fluid of passive smoking rat.All datawere shown in the fom of(?)±s and analyze with one-way ANOVA,LSD and dunnett T3 methods a=0.05。Result1,The effect of acupoint external application on the bronchial pathological structure and airway inflammation score of passive smoking ratThe result showed:in model group,we conld see tracheal and bronchial mucosa bronchial exuviated,struction arranged in disorder,cilia lodged,even exfoliated in various degree,the exudate of airway cavity increased,alveolar wall capillary congested,bronchial wall seed a large amount of inflammatory cells,such as macrophage,lymphocyte,neutrophil,etc.Acupointâ… group and acupointâ…¡group had been symptom described above too,but lighten than the model group.Explaining acupoint external application will have certain therapeutic function on airway inflammation.Conpareing to blank group,the score of airway bronchial exuviation, neutrophil aggregation,lymphocyte aggregation,macrophage aggregation,eosinophil aggregation,airway cavity exudation and pathological of the model group increased obviously(P＜0.01);conpareing to model group,the macrophage aggregation and neutrophil aggregation of the acupointâ… group and acupointâ…¡group decreased obviously(P＜0.01;P＜0.05):conpareing to blank group,the lymphocyte aggregation of the acupointâ… group and acupointâ…¡group increased obviously(P＜0.01,P＜0.01),the eosinophil aggregation increased obviously (P＜0.01,P＜0.05),but their was no difference between model group and acupointâ… group or acupointâ…¡group(P＞0.05);no matter according to airway struction or airway pathological score,their was no difference between acupointâ… group and acupointâ…¡group(P＞0.05).2,The blank group,the TNF-α,IL-8 content in bronchoalveolar lavage fluid of the acupointâ… group decreased obviously(P＜0.05,P＜0.05),the TNF-α,IL-8 content in bronchoalveolar lavage fluid of the acupointâ…¡group decreased obviously(P＜0.05,P＜0.01);according to the TNF-α,IL-8 content in bronchoalveolar lavage fluid,their was no difference between acupointâ… group and acupointâ…¡group(P＞0.05).4,The effect of acupoint external application on the SOD activity and MDA content in bronchoalveolar lavage fluid of passive smoking ratThe result showed:conpareing to blank group,the SOD activity in bronchoalveolar lavage fluid of the model group decreased obviously(P＜0.01), MDA content in bronchoalveolar lavage fluid of the model group increased obviously(P＜0.01);conpareing to blank group,the SOD activity in bronchoalveolar lavage fluid of the acupointâ… group increased obviously (P＜0.05),MDA content in bronchoalveolar lavage fluid of the acupointâ… group decreased obviously(P＜0.05),the SOD activity in bronchoalveolar lavage fluid of the acupointâ…¡group increased obviously(P＜0.01),MDA content in bronchoalveolar lavage fluid of the acupointâ…¡group decreased obviously (P＜0.05);according to SOD activity and MDA content in bronchoalveolar lavage fluid,their was no difference between acupointâ… group and acupointâ…¡ group(P＞0.05).Conclusion(1),Acupoint external application can increased cigarette smoke destruction to airway struction obviously,decreased the aggregation of inflammatory celts,such as macrophage and neutrophil.(2),Acupoint external application can increased theα₁-AT content in bronchoalveolar lavage fluid of passive smoking rat(3),Acupoint external application can decreased the TNF-α,IL-8 content in bronchoalveolar lavage fluid of passive smoking rat.(4),Acupoint external application can increased the SOD activity and decreased the MDA content in broncboalveolar lavage fluid of passive smoking rat.(5),Blistering or not did not influence macrophage and neutrophil aggregation of passive smoking rat,and did not influence theα₁-AT,TNF-α,IL-8,MDA content and SOD activity in bronchoalveolar lavage fluid of passive smoking rat.To sum up,the acupoint external application,on one hand,decreased the aggregation of inflammatory cells,such as macrophage and neutrophil, decreased the generation of TNF-α,IL-8 in airway tissue,decreased the inflammatory response in airway tissue;on the other hand,increased theα₁-AT content,SOD activity and decreased MDA content,decreased the destruction of inflammatory response to airway tissue.Meanwhile our research conformed from the point of experiment that of acupoint external application on chronic airway inflammation in passive smoking rat is a integrative regulative role related to many targets.The result of study revealed that blistering or not did not influence the effect of acupoint external application,but we still cannot affirm acupoint external application can increased the curative effect of airway inflammation in passive smoking rat,it remains to further study in the future.
