| Hypospadias is a kind of congenital malformation of the urinary system, to estimated the incidence rate of the disease is 1:300 born infant in our country. Surgery is the only method to cure the disease, but the repairing operation is difficult and has high complication. Up to now there are beyond 250 methods of surgery but none is the standard method in treatment of hypospadias. After plastic surgery principle and micromanipulation were introduced in operation the healing rate obviously improved, but the result of surgery is not very satisfactory because the most hypospadias patients have no enough tissue to repair urethra and it will influence shape of penis and scrotum when taking too much skin to repair urthra.From 1997 we started to use buccal mucous to repair urethra in hypospadias patients without enough skin of penis or scrotum. The buccal mucous is excellent compared with skin graft and bladder mucous, but the amount of buccal mucous is limited so it is not suit for serious hypospadias patients. Considered the advantage of buccal mucous and skin flap of scrotum, professor Li takes combination principle of invention to construct urethra using buccal mucous onlay scrotum flap. The method solves the problem of material shortage of hypospadias in some degree. In operation the buccal mucous is translated on ventral tunica albuginea of penis and onlay with penis or scrotum flap to form new urethra. Compared with method using one material to reconstruct urethra, the method takes less amount of flap, so it is reduce effect on shape of penis and scrotum. Compared with method taking flap into tube this method can avoid fold and press on flap stem, so it can guarantee the blood supply of flap. From initial clinical observation this method get great success in the treatment of hypospadias.With development of technique of cell culture and tissue engineer, it is possible to use cultured mucous to rebuild urethra. The method of using buccal mucous onlay local flap to form urethra also need amount of buccal mucous, so it increase risk of operation and damage to patient at some degree. We conceive taking a little amount of buccal mucous before operation and culturing mucous cell in vitro to form mucous chaff to repair urethra. After experiment we successfully cultured buccal mucous cells and the tecique of culture has been established. Buccal mucous cell could be maintained 9-10 passages, surviving 50-60 days in vitro. Electron microscope revealed desmosomes between the culture cells. The culture cells showed positive staining of cytokeratin antibody.
