Based on observing the physiologic and pharmacologic actions of glucocorticoids, this study postulates a new hypothesis regarding mechanisms of action for glucocorticoid from the perspective of the six channels of Classic Chinese Medicine. According to the study based on physiological dosage, the GC mainly affects the functions of the Taiyang's opening, Yangming's closing and Shaoyang's pivot by switching on the Yang-qi's function of "defensing exterior, so that securing the body." When exceeding the physiological dosage, the GC mainly affects the functions of the Taiyin's opening, Jueyin's closing and Shaoyin's pivot by switching on the Yin's functions of storing Jing(ç²¾) to activating the Yang-qi for dealing with critically urgent situations. The study proposes that the mechanism of glucocorticoids-induced osteoporosis(GIO) is functional disorder of the Six Channels, leading to a heavy discharging of the original qi stored deeply in the body, making the Yang-qi unable to go downward and inside. With this failure of the kidney to store, the kidney essence is depleted and the bone marrow is not nourished. The study also proposes that when the osteoporosis is established, the original-qi has become severely exhausted which makes the condition difficult to treat. Prevention is paramount, hence a four stage treatment plan is recommended. First, treat the primary disease by combining GC with Chinese medicine to ease the primary disease as soon as possible while shortening the time for administering drugs. A combination will attain a faster response thus reducing the incidence rate of GIO. The second stage is when the patient manifests flushing, polyphagia, red tongue and floating and slippery pulse, and the treatment must focus on protecting the spleen and stomach by reinforcing the earth and subduing the fire (which is caused by GC). The third stage is when the patient manifests vexing heat in the five hearts, feeling dry mouth and sore swollen throat, sores, bitter taste and sticky slimy sensation in the mouth, glomus and distention in the stomach duct, red tongue and yellow thick coating, etc. This symptom picture is usually called "the pattern of hyperactivity of yang due to yin vacuity with dampness," and it can be used with the method of sealing the marrow and subduing yang. This can be helpful in recovering the normal functions of opening, closing and pivoting the three yin and three yang. The fourth stage is when the patient manifests fear of cold and cold limbs, being ease to get common cold, anorexia, loose stool, pale tongue, white and glossy tongue fur, deep and weak pulse, this is called the pattern of the spleen-kidney yang vacuity.
