Endometriosis and adenomyosis has been a common disease that troubles both doctors and patients. The incidence of endometriosis and adenomyosis is showing a tendency to rise year after year. Because of its high incidence and low effective rate, it has been one of the focuses of clinical researches these years. Pain is the most common symptom of endometriosis and adenomyosis, and the most distressing symptom of patients. It not only makes the patients feel agonized in body, but also brings great pressure on their minds. Thus it greatly affects the patients' quality of life and health. Traditional Chinese Medicine (TCM) has an advantage in treating this disease. My supervisor, Professor Xiao believes that the pathogenesis of endometriosis and adenomyosis is deficiency in yang, cold accumulation and blood stasis. She treats this disease with the therapy of warming and clearing and this therapy has showed marked clinical effects. Objectives The aims of this research are as follows: 1) through clinical research, to evaluate the clinical effects of treating the pain of endometriosis and adenomyosis with the therapy of warming and clearing, 2) through steroid hormones, microcirculation, immunology, NO and nervous peptide, to explore the therapeutic mechanism of this therapy, 3) by giving patients questionnaires on their quality of life and minds before and after the treatment, to know the effect of pain on patients and to evaluate the effect of the treatment on patients.And sum up professor Xiao's academic thinking and characteristics of using Chinese herbs. Methods The research was based on 50 patients suffering from endometriosis and adenomyosis with the pathogenesis of cold accumulation and blood stasis. They were randomly divided into two groups, the experimental group and the control group. In the experimental group there were 30 patients and they were treated with prescription of warming yang and analgesic. In the control group there were 20 patients and they were treated with Shaofuzhuyu Tang. The course was 3 months. During the research, we observed the patients' symptom of pain , overall symptoms, the effect of pain on patients' quality of life, Depression Scale, WHOQOL-BREF, steroid hormones, CA-125, NO, TXB2 , 6-K-PGF1a, β-EP. Except that, 10 cases of endometriosis patients without pain were also set as the control group. All data were processed by SPSS11.5 statistical software. Results In the experimental group, the total effective rate, the conspicuous effect rate, and the curative rate was separately 90%, 66.7% and 36.7%. While in the control group, the total effective rate, the conspicuous effect rate, and the curative rate was separately 70%, 45% and 20%. There was no significant statistical difference in these two groups. But in relieving pain, cold limbs, fatigue, shortness of breath, the experimental group showed better clinical effect than the control group. The experimental group also showed apparently better clinical effect in improving the dysmenorrhea integrals, total pain integrals and integrated integrals than the control group. In order to evaluate the overall effect of the therapy of warming and clearing on alleviating pain, we observed the patients' quality of life in daily life, moods, ability to walk, ability to work, interpersonal relationship, sleep and interest to life. Before the treatment, the patients of these two groups showed no significant difference in quality of life. After treatment, the experimental group showed better effects in improving patients' daily life, moods, ability to walk, ability to work than the control group. According to related analysis, painful menstruation, lower abdominal pain during non-menstrual time, pain in coitus, shortness of breath, fatigue are the main factors that affect patients'quality of life.
