The course of atherosclerosis is long and the forming mechanism is complicated. It is the exciting and difficult domain in academia that study the forming mechanism of atherosclerosis. Traditional Chinese Medicine has the potential superiority and distinct characteristics in the treatment atherosclerosis. According to the methods and principles of clinical epidemiology, this study probes into the TCM syndrome pattern of primary hyperlipidemia that is the important risk factor of atherosclerosis. In order to present the basis in microcosmic determination of treatment based on differentiation of syndromes, we study the relation between laboratory indices and TCM syndromes. Using the techniques of Molecular biology, we aim at the pilot studies on the mechanisms of the method to invigorate the spleen to reduce phlegm and remove blood stasis anti-atherosclerosis in protein level. Method 1. In the clinical research, our group studied clinical cases in multi-center prospective method. In accordance with 480 primary hyperlipidemia patients' clinic data provided by many hospitals in Beijing and using the factor analysis method, this study probes into the TCM syndrome pattern of primary hyperlipidemia. Using Canonical analysis method, this study probes into the relation between age,BMI,laboratory indices and the TCM syndrome pattern. 2. In the experiment study, the atherosclerosis animal model was established by feeding apoE gene knock-out mice with high fat diet. The efficacious compound of representative medicine of the method to invigorate the spleen to reduce phlegm and remove blood stasis——Radix Ginseng panaxoside,Pollen Typhae flavonoids and Rhizoma Alismatis terpenoid was investigated, Simvastitin was taken as control. TC,TG,LDL-C and HDL-C was measuring on biochemical autoanalyzer in order to observe the serum lipid level of the experimental atherosclerosis mice affected by the Chinese medicine. The expression of HSPG was analyzed by immunoblot of liver samples, in order to observe the effect made by the method to invigorate the spleen to reduce phlegm and remove blood stasis. Results and conclusions: 1. Clinical research Results: 1. Using the factor analysis method, 8 TCM syndromes of primary hyperlipidemia were found; 2. There is no significant statistical difference in course of disease over the distribution of the 8 TCM syndromes (P>0.05); There is significant statistical difference in sex,age and other accompanying diseases over the distribution of the 8 TCM syndromes(P<0.001);3. Canonical analysis show that the patient's age,BMI and laboratory indices have relation with the TCM syndrome pattern of primary hyperlipidemia. The patient's age is in direct proportion with para-deficiency in spleen and kidney; The BMI is in direct proportion with para-obstruction by phlegm; The level of TC is in direct proportion with para-flaming of fire from yin deficiency; The level of LDL-C is in direct proportion with para-obstruction by phlegm and blood stasis. Conclusions: 1.The TCM syndrome pattern of primary hyperlipidemia mainly involve in spleen,kidney,liver,phlegm and blood stasis. The deficiency of spleen and kidney, and the obstruction of phlegm and blood stasis is the important pathogenesis; 2. The TCM syndrome patterns of primary hyperlipidemia have no relation with the patients' course of disease and have some relation with the patients' sex and other accompanying diseases. The male's distribution in para-obstruction of phlegm and para-phlegm-dampness conversion into heat is more; the female's distribution in para- phlegm-dampness conversion into heat,para-impeded blood circulation due to blood stasis,para-deficiency in spleen and kidney and para-flaming of fire from yin deficiency is more. The distributions of all atherosclerosis diseases in para-obstruction of phlegm are more.
