The Clinical Research On The Effect Of New Yijin Treatment For Periarthritis Of Shoulder

BackgroundPeriarthritis of shoulder is a disease of chronic aseptic inflammation caused by parenchyma trauma and degeneration, such as the muscles around the shoulder joint, muscular legs, ligaments, bursa and joint capsule, called periarthritis for short. The disease is belong to the motherland medicine "Arthralgia", also known as frozen shoulder, drain wind shoulder, coagulation syndrome shoulder, freeze shoulder, old shoulder, lock wind shoulder and so on. It has a 1:3 M:F ratio and high rate of occurrence in people aged around 50. According to statistics, the total incidence rate is about 3%-5%. Mostly unilateral, the left shoulder patients are more than the right one, and the clinical characteristics of them are a slow onset, gradually increased, and free to reduce after several months or longer and then self-healing. The course often between several months to two years, generally do not recur. The early manifestations mainly display mild pain, or shoulder discomfort and sense of restraint. Then the pain gradually increased, particularly at night, which often affects sleeping. The shoulder mobility would gradually completely restrict and its abduction is difficulty. The elevation of shoulder can not move above their heads and the activity is limited. People will often feel shoulder tenderness when they sitting. All of above, not only bringing pain to the patients but also can substantially affect their daily life and work.Therefore, how to improve the therapeutic effect of the disease and relieve suffering is still the matter of concern. The purpose of clinical treatment of frozen shoulder is to relieve pain and restore shoulder function, mainly using non-surgical therapy in current.The general therapy methods as follow:local blockage, internal or external application of Chinese medicine, cupping, acupuncture, manipulation, non-steroidal anti-inflammatory drugs and so on.The New Yi Jin treatment of periarthritis of shoulder is from the original idea of Professor Lin's fasciaology. The supporting-storing system is the remaining, which have formed connective tissue fascia framework throughout the body after differentiation of mesenchymal mesoderm into multiple organ systems. The core of this framework is adipose derived stem cells, and has formed a new functional system with the participation of nervous system and immune system. Also known as two-system theory, the organism is divided into supporting-storing system and functional system and functional system.Chinese medicine external treatment have been considered to act some biological effects by fasciaology, as follow:producing injury factors; mechanical stretch pulling reaction effect; nerve reflection effect; local damage repair effect and body stress effect. External stimulations as follow:Promoting the proliferation and differentiation of stem cells, and then improve the repair and functional status of the whole body. Producing the shielding effect of endogenous neural information, and then improve the relaxation reflex of muscle tension. Improving local damage repair and stress ability, and then promoting the functional enhancements.The new fascia has considered that there are four-step mechanism of new Yi Jin treatment as follow:①Through the stretch of maximum angle, pulling and rotating the shoulder joint to stimulate deep sensory receptors in joint capsule, and fully relaxing of the muscles near the joints by neural regulation.②Stimulating intermuscular septum of lesion distribution by acupuncture and stimulating on the local connective tissue mechanical stretch to promote blood and lymphatic circulation by extruding local capillaries and lymphatic. Simultaneously, a lot of biological information has been generated, such as cell activity and injury factors, which can improve the proliferation and differentiation of undifferentiated stem cell in the fascia connective tissue. Furthermore,deep sensory nerve receptors are stimulated by a lot of biological information that can pass to the brain center to increase the stress ability to repair the damage of body.③Application of floating needles not only can stimulate skin nerve and inhibit pain signals, but also can kill the pain by releasing a neurotransmitter of nerve regulation.④Then by the maximum angle of the cross-joint traction, pulling and rotation, tiny dislocation caused by muscle tension or fatigue ligaments in reset to treat the disease.In this study,102 cases of shoulder periarthritis have been collected in orthopedic outpatients. By clinical observation, the new Yi Jin treatment effect is significant, which is an ideal way to treat shoulder periarthritis.ObjectiveThrough the observation of clinical cases to explore whether the new Yijin therapy is effective for periarthritis of shoulder. It provides a new way for periarthritis of shoulder treatment and clinical support for the improvement of fasciaology.MethodsIn this study,102 cases were collected from the God Save church in Hong Kong from September 2008 to September 2010, which contain 45 males and 57 females. According to the completely random distribution, patients were divided into treatment group 51 and control group 51. There were 23 males,28 females in treatment group and 22 males,29 females in control group, which aging from 40 to 70 years old, mean age of55 years old. Statistical analysis showed comparability that two groups of age, gender, disease locations and severity of disease were no significant difference. Treatment group were treated by the new Yijin treatment, and acupuncture therapy for the control group,Treatment group:treated by the new Yijin therapy, shoulder muscles were released by massage for 3-5 minutes, bend the elbow by the focal point of elbow joint and pulling, stretching, rotating shoulder, stimulating shoulder joint capsule, relaxing the muscle ligaments around the shoulder in the case of patients' the maximum tolerated pulling. Improving the body stress ability to promote damage repair by acupuncture in muscle intervals around the shoulder and abnormal points, and then retaining needles for 15 minutes. Acupuncture analgesia by using floating needles for cutaneous nerves in lateral, medial, posterior of arm, and lateral branch of clavicular nerve, in order to achieve complete relaxation of the muscles around the joints. The manual reduction was done in shoulder joint dislocation, which caused by muscle spasm, by pulling. Once three days, three weeks a course and lasting for two courses.Acupuncture treatment was done in control group, by taking the shoulder seam, t'ien tsung, shoulder yu, shoulder zhen, large bone for the main acupoints, and the bend pond, hegu, and article population for the second acupoints. Then penetrating the needles quickly and retaining needles for 15 minutes. Starting 2th course treatment after a interval of 3 days. Followed up for 3 to 5 months, to record and compare the improvement of knee symptoms between treatment group and control group. The data analysis was done by SPSS13.0 statistical software and displayed statistical difference, P<0.05, by m±s.ResultsThe treatment effect of treatment group was significantly better than control group by statistical analysis after treatment. And the evaluation of pain, shoulder range of activities were significant difference in before and after treatment (P<0.01). The comparison of pain, shoulder flexion, abduction, extension, internal rotation, external rotation (P<0.01) were taken in two groups.ConclutionThrough this clinical observation, we can find a new Yijin treatment for periarthritis of shoulder, which can improve the symptoms. Statistics shows that the new Yijin treatment of periarthritis of shoulder was better than acupuncture treatment. The New Yijin treatment of periarthritis of shoulder is effective, and worth clinical application.
