A few literatures foucsed the level of daily physical activity(PA) and relation between PA and CVD and the estimating of CVD risk during graded exercise test(GXT) for 40-49years of age people in mainland of China. Purpose:To investigate the level of PA and the relationship between PA and CVD for 40-49 age people in mainland of China; also to estimate risk of CVD for health people with different level of daily PA who were from the samples of investigation during GXT, to give the recommendations for them to decrease the risk of CVD during activity. Methods:First Part: TO design the questionnaire which was composed of the International Physical Acivity Questionnaire(IPAQ) and other questions for CVD. To test validity and reliability before using. After that, to investigate the level of PA, health and diet for 401 male (44.2±3. lyr) and 374 female (44.5±3.3) urban people who were 40-49 age. Second Part:173 male(44.0±3.5yr) and 174 female(44.5±3.4yr)with different PA evel(〈600;>=600;>=3000MET-minutes/wk) and BMI (18.5-23.9; 24.0-27.9>=28.0 kg/m2;) were chosen to participate the GXT who met the requirements for subjects and informed consents were obtained. The GXT was performed on a cycle ergomater of Custo-med with incremental work loads starting with 25 w and adding 25 w at 2 min intervals until the subject experienced any discomfort or irregularities which were referenced from general indications for stopping an exercise test in ACSM's Guidelines for Exexcise Testing and Perxcription(eighth edition)in 2009, which was include:objective indicators of abnormal ECG and BP value and subjective indicators of RPE and fatigue. BP, HR, PWV, ABI, cardiac enzymes, hCRP were recorded before and after GXT. The performace of GXT, BP, ECG and RPE were recorded during GXT. The Results: 1. The light activities were more than 30 percnets in daily PA. 2. Incidences of cardiac adverse effects for male were also 17% to 24% higher by low level of PA; Incidences of CVD for female were also 40% to 54% higher by low level of PA.3. Incidences of CVD and cardiac adverse effects were further decreased 0.4% and 1.2% with males and females respectively through overall PA.4.The more PA, the risk of CV during GXT in normal weight, overweight and fat males were down 17%,1.7% and 5% respectively; in normal weight and overweight females were down 14.1% and 20.2% respectively.5. The more PA, the risk of CV during GXT in all males and females were down 9.9% and 12.7% respectively.6. Compared with males in low level of PA, the more PA, the upper limit of safety exercise of normal weight, overweight and fat males were increased 6.2%HRR,19.7%HRR and 12.7%HRR respectively; the upper limits of safety exercise of normal weight, overweight and fat females were increased 5.9%HRR,29.2%HRR and 11.0%HRR.7.The more PA, the upper limit of safety exercise of all males and females were increased 12.7%HRR and 11.3%HRR respectively. Conclusions:1. The lower level of PA caused the increasing of risk in CVD and cardiac adverse effects in 40-49yr Chinese males and females.2. Low PA set up major proportion of 40-49yr Chinese people's daily PA.3. The effect on CV health of light PA can not be ignored.4.The more PA, the the upper limits of safety exercise was increased and the risks of CV during GXT were down in different BMI people.5. The more PA, the the upper limits of safety exercise was increased and the risks of CV during GXT were down in all people.
