The theory of Xuanfu is the important part of theory of Chinese Medicine,which instruct doctors' clinical practice.lt offered new breach and interfere point for the prevention and therapy of many difficulty diseases. Its application displays very great value of instruction and foreground of application.This paper expound systematically the theory of Xuanfu and apply it to the treatment of VD for the validation of value of clinical instruction.1. Objective1.1 Discuss Xuanfu's concept, physiology and pathology and clinical value comprehensively and systematically,which show its newly clinical value.1.2 Use Xuanfu theory in the treatment of VD to explore a new pathway and evidence of VD's therapy.1.3 Process a clinical study for clinical validation of Xuanfu theory.2. Methods2.1 Theoretic methods: This paper apply the methods of medical literature,theory of Zangxiang,logistic analysis and so on to explore Xuanfu's meaning, anatomical substance, physiology and pathology.The method of logic and discursion is applicated to connect Xuanfu and VD for further expatiation of Xuanfu's mechanism of VD and analysis the methods of treatment, prescription and drugs.2.2 Clinical study: This randomly control study investigate the patients with VD of early gently and middle degree which accord with the diagnose criterion of Chinese medicine and Western medicine. The control group use the drug of Xidezhen and treatment group Zengzhi Capsule which is summarized by academician Wang Yong-Yan.The main indexs are clinical syndrome of Chinese medicine,MMSE scales,BBS scales,evaluation of dementia.syndrome effect index and so on.3.Results3.1 Theoretic results: According to academic thinking of academician Wang Yong-Yan,the following can be summarized. VD happens Xuanfu of brain and the mechanism is suggested that Xuanfu is obstructed and spirit is unactivation.The principle of VD's treatment are to dredge brain's Xuanfu and flow Qi and blood inbrain. In selection of drugs,spice drugs should be emphases and it is firstly suggested that spice drugs and bitter drugs are used in the same time and so are cool drugs and warm drugs.3.2 Results of clinical studyThe results show that Zengzhi Capsule have affirmed effect on VD of gently and middle degree.It can impove cognition function,alleviate syndrome of dementia,revive the approach of disease,which are consistent with the result of other drugs in the world.MMSE analysis: Zengzhi Capsule can markedly raise total scores of MMSE compared with its before treatment(P<0.05). Determinanted by scores of MMSE scale,the effect ratio of treatment group is 55 percent and total effect ratio 85 percent,while the ratio of control group 40 percent and 60 percent respectively.BBS analysis: Zengzhi Capsule can markedly reduce total scores of BBS compared with its before treatment(P<0.05). Determinanted by scores of BBS scale,the effect ratio of treatment group is 30 percent and total effect ratio 90 percent,while the ratio of control group 20 percent and 75 percent respectively.Analysis of syndrome:. Zengzhi Capsule can markedly reduce total scores of syndrome compared with its before treatment and control group respectively(p< 0.05). Determinanted by scores of BBS scale,the effect ratio of treatment group is 70 percent and total effect ratio 100 percent,while the ratio of control group 45 percent and 80 percent respectively.In summary,the effect of Zengzhi Capsule have better effect than that of control group.Evaluation of degree of dementia: The ratio ofcure,reversion,remittance,deterioration of Zengzhi Capsule are 45 percent,35 percent,15 percent and one case,while those of control group are 40 percent,35 percent,35 percent and zero. There are not different significantly(P>0.05).4.Conclusion4.1 The theory of Xuanfu is the most constitutes of Chinese medicine,it gives novel theoretical evidence to current clinical of Chinese medicine and direct the prevention and therapy of difficulty diseases.4.2 Xuanfu is thinked as the minimum anatomical unit related with structe of human body in Chinese medicine,which distribute all of organs and tissues and form a system or pathway net connecting every part of human body.4.3 The physical function of Xuanfu have several aspects and have the main function of dredging brain's Xuanfu and flow Qi and blood in brain,which can be expressed that Xuanfu is the pathway and place of Qi and blood,and the entry andexit of spirit with the characteristic of expedition. Several factor of pathogenesis can lead to the disorder of opening and shutting of Xuanfu and Qi and liquid and then Xuanfu is obturated which is the pathological foundation and nosogenesis mine of several diseases.4.4 This paper is guided by the theoretical thinking of academician Wang Yong-Yan that there are Xuanfu all of five Zangs and six Fus. In selection of drugs,spice drugs should be emphases and it is firstly suggested that spice drugs and bitter drugs are used in the same time and so are cool drugs and warm drugs.The prescription is Shengjaing San added Renshen, Heshouwu, Shichangpu and Shexiang.4.5 Zengzhi Capsule with the function of dredging of brain's Xuanfu(invition prescription of academician Wang Yong-Yan) have affirmed effect on VD of gently and middle degree.It can raise the scores of MMSE reduce the scores of BBS and syndrome,concluded that it improve the cognition function and ease the dementia syndrome.The result validate the guidance value of Xuanfu theory.
