The Application And Value Of Continuous Venovenous Hemofiltration To Rescue The Critical Patients

Objective To investigate the therapeutical and adverse effects of Continuous Venovenous Hemofiltration ( CVVH ) on Refractory Heart Failure ( RHF) , Severe Acute Pancreatitis (SAP ) and Multiple Organ Dysfunction Syndrome ( MODS ) , explore the appropriate treatment indications , occasions , lasted -time , et al ; and to evaluate the practical value of CVVH clinically. Methods 53 cases with RHF were received intermittent CVVH treatment by a low-blood flow ,low replacement fluid way ,10-hour as a coure ,clinical manifestation were observed ; 32 cases with SAP were received intermittent CVVH treatment by a high volume way , in which accumulated hemofiltration volume always reaches the maximum , 10 to 48-hour as a course , APACHE â…¡ scores were measured ; 51 cases with MODS were received a CVVH treatment as applying for SAP , the mortality and APACHE â…¡ scores were assessed as observing indications . Results â‘  Patients with RHF achieved a remarkable remission post-CVVH, the CVVH treatment significantly superior to the traditional hemodialysis ( HD ) treatment in term of more positive effects and less adverse effects, it was applicable to patients with severe cardiac arrhythmia and hypotension also. â‘¡ The intermittent CVVH treatment which was administrated with the SAP patients'APACHE â…¡scores > 10 and was stopped when the patients'APACHE â…¡scores decreased to the half of pre-CVVH was effective and feasibal.. â‘¢ Treatment SAP and MODS with CVVH were effective . â‘£ The mortality in patients with SIRS and two organs dysfunction after CVVH treatment was 0% . Conclusions â‘  Treatment patients with RHF , SAP ,and MODS with CVVH are effective and safe . â‘¡Application of the APACHE â…¡ scores was rather appropriate and practical valuable for instructing CVVH treatmant . â‘¢ Treatment patients with MODS with CVVH as early as possible was able to reduce the mortality significantly .
