| Objective According to the RIF stratified diagnostic criteria of acute kidney injury (AKI) comparing the difference of the renal function recovery,the ICU stay time,medical costs and the survival of 30th day between sepsis associated AKI (septic AKI) patients who received hemofiltration in the injury stage and who didn't,to provide clinical evidence for treating septic AKI by early hemofiltration.Method In 8 intensive care units,prospective,before and after controlled studied septic AKI patients,except for chronic renal failure and reversible AKI.16 cases in the control group (group A) did not receive hemofiltration in the injury stage,16 cases in the experiement group (group B) received hemofiltration in the injury stage.The termination of the hemofiltration was the serum creatinine and blood urea nitrogen presenting the progressive decrease,the urine volume presenting the progressive increase and over 1000ml/day.Collect the data of the two groups:the urine volume and serum creatinine when they were selected,the third day and out of ICU,APACHE-â…¡score,the number of failing organs,ICU stay time,medical costs, the survival of 30th day.Results 1) There were no statistics difference in the sex,APACHE-â…¡score and the number of failing organs of two groups when they were selected,the age of group B was greater than group A;2)There were no satistics difference in the change of the urine volume and serum creatinine when they were selected,the third day and out of ICU,however the urine volume increasing of group B had the trend of better than group A;3)There was no ststistics difference in the APACHE-â…¡score of two groups when they were selected and the third day;4)There were no statistics difference in the ICU stay time,medical costs,hemofiltering time and the survival of 30th day of two groups,the hemofiltering time of group B had the trend of shorter than group A;5)Accroding to regression analysis,the age of septic AKI patients was related to the mortality of 30th day,yet received hemofiltration in injury stage was a protection influence of septic AKI patients.Conclusion Early hemofiltration maybe promote renal function recovery of septic AKI patients,and reduce their hemofiltering time needed. |
