Clinical And Experimental Studies On The Constitutional Features And Their Implications In Tumorogenesis Among Population At High Risk Of Nasopharyngeal Carcinoma

This study was carried out to investigate the constitutional patterns and their genetic characters of population at high risk (PAHR) of nasopharyngeal carcinoma (NPC), the relationship between special constitutional pattern experimentally prepared and the occurrence of induced NPC among rats, and effective therapy for preventive treatment of PAHR-NPC. It can be briefly summarized as following.Firstly, a survey was made to investigate the constitution patterns of PAHR-NPC. At first, the ancient literatures on NPC were screened from several classic works of Traditional Chinese Medicine(TCM), according to the criteria of clinical features on NPC. Then, on the basis of standards for constitutional pattern classifying, put forward by He Yuming and Kuang Tiaoyuan, a modified method of survey and corrected standards for pattern classifying were taken in our study through a closed questionnaire to investigate the distribution of constitutional patterns among the members of core pedigree, in which one or more cases with NPC had been diagnosed pathologically before, and among PAHR-NPC, that were diagnosed on the basis of serological test and/or histopathologically assay of nasopharyngeal tissue samples.Secondly, studies were carried out to determine the genetic basis of constitutional patterns among PAHR-NPC and its implications in the investigation of prevention and treatment for them. These included a series of work. (1)Assays for appearing rate of micronucleus from cultured peripheral blood leukocytes among PAHR-NPC, cases with chronic nasopharyngitis and patients with NPC. (2)Assays for subpopulation of T lymphocytes in peripheral blood among cases as above. (3)Assay for IgA level in serum among cases as above. (4)Observation on the therapeutic effects of Qi-Boosting Toxin-Resolving Granule on the improvement of constitutional statusamong PAHR-NPC. (5)Investigation on the therapeutic effects of Qi-Boosting Toxin-Resolving Granule on the levels of IL-2 and TNF- a in serum and infectious activity of EB virus among PAHR-NPC.Thirdly, an experimental study was taken to determine the association of constitutional pattern with the development of induced NPC among Wistar rats. Constitutional pattern models were prepared with over fatigue method (swimming in a deep water pool) for qi deficiency pattern and warming herbs taking method for heat pattern respectively among rats. Following the setting up of syndrome, secondary model of induced NPC with DNP and TPA was prepared among these animals with primary syndrome model, with them divided into several groups and with different controlling and treating drugs for them. Among these groups, one was orally given Qi-Boosting Toxin-Resolving Formula to observe the inhibitory effect of it on the development of induced NPC. This inhibitory effect was confirmed by histopathology of nasopharyngeal tissues collected from rats at different interval duration. Moreover, the technique of gene expression profile was taken to determine the features of gene expression profiling among rats with the syndrome of qi deficiency.The results of these studies showed that development of lesion in nasopharynx among PAHR-NPC was closely associated with qi deficient pattern of constitution. This could be supported by the following facts. Firstly, qi deficiency was the commonly seen constitutional pattern among PAHR-NPC, while deficiency-heat and deficiency-stasis patterns were the frequently met constitutional patterns among cases with NPC. Secondly, the appearing rate of micronucleus was increased and cellular immune function was decreased among both of PAHR-NPC and NPC patients, with a tendency to become aggravated further following the development of lesion. Moreover, the liters of IgA antibodies in serum against EB virus was declined, the levels of EL-2 and TNF- a in serum was increased, and the symptoms of constitutional pattern with qi deficiency was improved following the use of Qi-Boosting Toxin-Resolving Formula among PAHR-NPC.
