| Ventricular remodeling (VR) after acute myocardial infarction (AMI) was defined as complex alterations in the ventricular architecture and function involving both the infracted and non-infarcted zone. The early remodeling involves expansion of the infarct zone, which may result in wall thinning or ventricular rupture or aneurysm formation. The late remodeling involves the ventricular globally and is associated with time-dependent dilation, the distortion of ventricular shape, and mural hypertrophy. The statistic shows that in the Q wave acute myocardial infarction, the incidence of ventricular remodeling was 30-50%. This process of ventricular remodeling substantially influences the prognosis of patients who have survived the acute phase of myocardial infarction. Recently, interest has developed in limiting ventricular remodeling, with the objective of improving ventricular function and clinical outcome.Ventricular remodeling is initiated by the activation of the local tissue rennin-angiotensin system (RAS), the paracrine /autocrine factors. Angiotensin II (Ang II )may be the core of the series of biochemical reaction. It suggested that thrombolysis therapy and angiotensin-convering enzyme inhibitor (ACEI) could limit infarct sizes and relieve ventricular loading conditions, may be the better ways to prevent ventricular remodeling.Acute myocardial infarction belongs to xiongbi, xintong and zhengxintong in traditional Chinese medicines. They are a lot of scientific research involving clinical and experiment study on prevention acute myocardial infarction. They had dissuss a lot of questions such as the etiology and pathogenesis in acute myocardial infaction, and summarized the effective treatment and recipe on prevention acute myocardial infarction, they also have done experiment to prove herbs effectively not only in animal but also in mankind, all of this have made better curative effect. What they have done making base for prevention left ventricular remodeling. According to the many year's clinical practices, we believe that the major basement for ventricular remodeling is deficient of zhengqi, stagnation of Qi, stasis of blood, and accumulation of phlegm are the major etiological factor, deficiency complicated with excess, deficiency as primary and excess as secondary are the major mechanism for postinfarction left ventricularremodeling in traditional Chinese medicine. Kaixin capsule is a recipe made by professor Chen j ing-he, its function involves supplementing Qi, promoting the production of body fluid, activating blood circulation to dissipate blood stasis, eliminating phlegm. Previously experiment had approved that in rats whom had been ligated left coronary artery, Kaixin capsule have satisfactory curative effect in prevention postinfarction ventricular remodeling. But how about the curative effect in patients who taking Kaixin capsule, could Kaixing capsule effect in the late remodeling? All of this, we couldn't give the satisfactory answer. So in this study, one hand we will verificate its clinical effect, and another hand we will verificate its effect in late ventricular remodeling. ? Clinical studyVentricular remodeling is initiated by neurohormonal excessive activation, such as the excessive activation factors involved angiotensin II (Ang II), endothelin (ET), atrial natriuretic peptide (ANP) and aldosterone(ALD) etc. Hypotension after acute myocardial infarction activates the rennin-angiotensin aldosterone system, which enhance the production angiotensin II, angiotensin II may augment endothelin release, which is another stimulates for ventricular remodeling and stimulate the secretion of atrial natriuretic peptides. Atrial natriuretic peptides, in turn, inhibits the production of angiotensin II, endothelin,and aldosterone.
