Research Objective:Yu Wu-Yan was an important doctor of Traditional Chinese medicine in the early period of Republic of China, whose theory was classified as the "medical school of classical prescriptions". Because of interesting in integrative medicine intended to combine TCM with Western medicine, he advocated "scientific connotation of TCM, Chinese style of Western medicine" and therefore was regarded as the important physician of the "school of integrative medicine".Yu Wu-Yan was known for his application of classical prescriptions, especially good at the use of gypsum and rhubarb. Hence, he was called "Mr. Gypsum and Mr. Rhubarb" at the time. Yu Wu-Yan’s respected academic position in the Chinese medicine profession has long been affirmed.Cao Ying-Fu, Lu Yuan-Lei, and Yu Wu-Yan were the "three masters of classical prescriptions" at the time. However, according to author’s collecting relevant academic literature of Yu Wu-Yan, it explicitly proved that the research works is in the state of shortage. The purpose of the dissertation is to further explore both the academic thoughts and clinical experience of Yu Wu-Yan with systematic research on how classical prescriptions are modified for varied and severe diseases and dosage control strategies in order to rescue people in dangerous.Research Method:There are nine academic books written by Yu Wu-Yan, including four books in the field of surgery. From his research on abundant classical books of Traditional Chinese medicine specialized in Shang Han Lun, he wrote the representative theoretical book named Shang Han Lun Xin Yi in1939. During the period from1943to1946, he had assiduously written two books, Shi Wen Shang Han Bing Pian and Ban Zhen Shang Han Bing Pian. In addition, Jing Kui Yao Lue Xin Yi was edited by Yu Wu-Yan in1952. In1957, the highly condensed compilation of personal clinical experience known as Yi Jin Jin Yan Lu was published.The study of this dissertation intends to deeply explore both the academic thoughts of Yu Wu-Yan and the clinical experience on how the prescriptions and herbs he applied via the way of qualitative methods. In the first part, according to the works written by Yu Wu-Yan, the essence of the academic thoughts and syndrome differentiation and treatment which provided theoretical support and analysis are discussed.The fifty frequently used Chinese drugs (TOP50Drugs) chosen by studying team are the other part of the research objects. In the second part, the dosage features of prescriptions on medical records of Yu Wu-Yan will be studied by analytical statistics.In statistics, the prescriptions are only selected these decoctions which the decocting method is boiling by water. There are82formulas satisfied with the requirement. A total of104drugs are used of the formulas selected, while there are37drugs belonged to Top50Drugs.To establish the Excel table processing function and basic statistical methods, we acquire the data and information of the occurrence frequency, the minimum dosage, the maximum dosage, the average dosage, the most frequently used dosage, and the distribution extent from the minimum to the maximal dosage of the fifty Chinese drugs (TOP50Drugs) on medical records of Yu Wu-Yan. Moreover, the total weight of the descriptions and the total number of drugs used in the descriptions are also obtained in statistics.Especially, rhubarb is the most frequently used drugs and gypsum is the drugs that the dosages used appear maximum among the fifty Chinese drugs (TOP50Drugs). Both rhubarb and gypsum are individually investigated by the statistical data techniques in advance.Research Results:From statistical analysis of the study, the highest frequency of drug use is rhubarb to be occurred39times. The maximum dosage of drug use is gypsum to be reached as180g. The distribution extent from the minimum to the maximal dosage of the fifty Chinese drugs in Shanghan Zabing Lun (Treatise on Cold Pathogenic and other Diseases)(TOP50Drugs) is mostly located between3g-15g. Some are widely ranged included rhubarb, prepared root of ural licorice, gypsum, and fruit of Mongolian snakegourd. Particularly in the range of gypsum, the interval ranges from minimum amount of15g to the maximum amount of180g, which is extremely12times.The average dosages of drug use are mostly below15g, except for two of rehmannia and gypsum, the average dosages of them are more than15g. Especially the average dosage of gypsum is far more than the others, up to96.9g, which is farther6times more than the average dosages of other drugs.The highest occurred dosage of the most frequently used dosages of the fifty commonly used Chinese drugs (TOP50Drugs) is9g, followed by12g. The most frequently used dosages of TOP50Drugs are largely existed in the dosage range of2010edition national pharmacopoeia and Chinese Pharmacy textbooks, while only6drugs of them are beyond the extent. The most frequently used dosage of gypsum is120g, twice of the maximum amount recorded in the2010edition national pharmacopoeia. From the statistics of the comparison, it shows the characteristic of gypsum clinically exploited on large amount by Yu Wu-Yan.According to statistics of the number of prescriptions on medical records by Yu Wu-Yan, we conclude that the number of prescriptions contained rhubarb is39and possessed gypsum is31. Besides, the number of prescriptions contained both rhubarb and gypsum is20. To investigate the constructions of prescriptions contained rhubarb on medical records by Yu Wu-Yan, the category of Chengqi Analogous Formulas is the most frequently selected within21times, followed by the category of Xiexin Analogous Formulas for9times.In addition, the analysis of constructing prescriptions contained rhubarb indicatessignificant results for ten foundations of classical and non-classical prescriptions:Da Chengqi Decoction, Xiao Chengqi Decoction, Tiaowei Chengqi Decoction, Da Xianxiong Decoction, Liangge San, Xiexin and Chengqi Decoction, Dahuang Huanglian Xiexin Decoction, Fuzi Xiexin Decoction, Dahuang Moutan Decoction, and Dahuang Fuzi Decoction.Furthermore, the constructions of prescriptions contained gypsum can be classified into three categories of classical prescriptions, Baihu Decoction, Baihu Renshen Decoction, Zhuye Shigao Decoction. In these prescriptions contained gypsum, the Baihu Analogous Formulas is counted to12. The prescriptions contained rhubarb and gypsum on medical records by Yu Wu-Yan are mainly composed by three main classical categorized prescriptions mentioned above, by one or two or three of them, namely Chengqi Analogous Formulas, Xiexin Analogous Formulas, and Baihu Analogous Formulas. Some of them are also comprised by the method of increasing body fluid frequently accompanied with these drugs as fresh rhizome of common reed, fresh rehmannia root, fresh noble dendrobium herb, and fresh leaf of henon bamboo. Conclusion:Yi Jin Jin Yan Lu, the collection of personal clinical experience, includes the medical records written by Yu Wu-Yan. The content of medical records has both commonly encountered diseases and severe diseases and critical syndrome. The characteristic of treatment is represented by modifying classical prescriptions in principle.In the aspects of drug use, the dosage of drugs in the circumstances of commonly encountered diseases is in normal and common amount. While the diseases were severe and dangerous, the dosage of drugs is dramatically in large amount beyond the limit of dosage range recommended in2010edition national pharmacopoeia and Chinese Pharmacy textbooks.Yu Wu-Yan was known for his application of gypsum and rhubarb. Hence, he was called "Mr. Gypsum and Mr. Rhubarb." The study indicates that one of the major characteristics of clinical experience is to deliberately exploit the heat clearing method and purgation method in various pathogenic conditions. According to the related statistics of gypsum and rhubarb in our study, the maximum amount of dosage of both gypsum and rhubarb has occurred in the treatment of seasonal febrile diseases on the medical records.To cure seasonal febrile diseases, Yu Wu-Yan had taken either the method of heat clearing or the purgation method or both of them in together. It was said in the concise phrase which is "two Baihu Decoctions in the heat clearing method, three Chengqi Decoctions in the purgative method, Da Xianxiong Decoction and Xiao Xianxiong Decoction to heal stagnation of phlegm, the Xiexin Analogous Formulas to heal flatulence due to qi stagnation" by Yu Wu-Yan. From the analysis of the theoretical opinions of Yu Wu-Yan in the first part and the results of the statistical process in the study, the broader understanding helps to elucidate the principles of constructing decoctions, rules of drug use, and dosage strategies of drugs.
