| Objective: To observe the clinical efficacy of external application of Shi Re Bi Tong fluid extract in the treatment of acute gouty arthritis with damp heat accumulation syndrome.Methods: From September 2021 to December 2022,60 patients with gouty arthritis of damp-heat accumulation type diagnosed by the Department of Rheumatology and Pain of Dian Jiang County Hospital of Traditional Chinese Medicine of Chong Qing were selected and divided into basic treatment+ Shi Re Bi Tong ointment group and basic treatment+ Shi Re Bi Tong fluid extract group according to the random number table,with 30 cases each.The two groups were compared in terms of joint pain,local skin temperature,TCM syndrome score,blood uric acid(BUA),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),interleukin-6(IL-6),and medication compliance before and after treatment to evaluate the clinical efficacy of damp-heat bi tong liu extract in the treatment of acute gouty arthritis.Results:1.Analysis of joint pain relief: There was no significant statistical difference in VAS scores between the two groups before and 4 hours after treatment(P>0.05);The comparison of VAS scores between the two groups before and after treatment and between the groups after 7 days of treatment was statistically significant(P<0.05).The results showed that both groups could effectively relieve joint pain,and the degree of pain relief in the two groups was equivalent after 4 hours of medication application.After the full course of treatment,the degree of pain relief in the treatment group was better than that in the control group.2.In terms of local skin temperature of joints,there was no statistically significant difference between the two groups in skin temperature before treatment,12 hours after treatment,and 7days after treatment(P>0.05).There was a statistically significant difference in endothelial temperature between the two groups after treatment for 12 hours and 7 days(P<0.05).It is suggested that both groups can effectively reduce the local skin temperature of joints,and the efficacy of the two groups is equivalent.3.The results of BUA,CRP,ESR,and IL-6 in the treatment group and the control group before and after treatment were statistically analyzed.Before treatment,the two groups were comparable(P>0.05).Compared within the two groups before and after treatment,both groups were significantly improved in laboratory indicators.After treatment,there was no significant difference between the BUA and ESR groups(P>0.05),indicating that the two groups had similar efficacy in reducing blood uric acid and ESR.After treatment,there was a statistically significant difference between the CRP and IL-6 groups(P<0.05),indicating that the treatment group was more effective in reducing the levels of CRP and IL-6 than the control group,and the Shi Re Bi Tong fluid extract was more effective in improving inflammation indicators.4.In terms of TCM syndrome scores,there was no significant difference between the two groups before and after treatment(P>0.05);The comparison between pre treatment and post treatment in the two groups was statistically significant(P<0.05).5.The total effective rate of the disease was 93.3% in the control group;The treatment group was 96.7%.It is suggested that both groups have good therapeutic effects on acute gouty arthritis.6.There was a statistically significant difference in drug compliance between the two groups(P<0.05).The drug compliance of the treatment group was better than that of the control group.7.During the study and observation period,there were no significant adverse reactions in both groups of patients,and there were no significant abnormalities in the observed safety indicators.Conclusion:1.This study shows that the Shi Re Bi Tong recipe has a good therapeutic effect on patients with acute gouty arthritis with dampness heat accumulation syndrome.Both groups of treatment can effectively improve the VAS score,joint local skin temperature,TCM syndrome score,blood uric acid,erythrocyte sedimentation rate,C-reactive protein,interleukin-6,etc.of patients with acute gouty arthritis.Moreover,the Shi Re Bi Tong fluid extract group is superior to the Shi Re Bi Tong ointment group in relieving pain and reducing inflammatory indicators,which can improve the patient’s medication compliance.2.Shi Re Bi Tong fluid extract has the advantages of convenient use,safety,non-invasive,significant efficacy,and improved patient compliance.It provides a new idea and effective external treatment of traditional Chinese medicine for the clinical treatment of acute gouty arthritis,and is worth further promotion and application.". |
