Clinical Observation On Modified Tongfeng Decoction In Treating Acute Gouty Arthritis With Damp Heat Accumulation Type And Its Effect On Serum IL-1β And TNF-α

Objective Observe the clinical efficacy of Modified Tongfeng Decoction in the treatment of acute gouty arthritis with damp heat syndrome,and its effect on the expression levels of serum IL-1β and TNF-α,and evaluate its safety.Methods A total of 60 patients with acute gouty arthritis of damp heat accumulation type from January 2019 to December 2019 were collected from rheumatology department of Henan Hospital of traditional Chinese medicine.After numbering according to the order of treatment,the patients were randomly divided into experimental group and control group with 30 cases in SPSS 22.0 statistical software.The control group was given celecoxib capsule orally,and the experimental group was added with Modified Tongfeng Decoction on the basis of the control group.During the course of treatment,both groups were given basic treatment plan,the observation period was 7 days.Before and after treatment,the VAS score,TCM syndrome score,laboratory indicators(UA,ESR,CRP)and cytokines(IL-1β,TNF-α)of the two groups were observed,and the recorded data were statistically analyzed.Results(1)curative effect of TCM syndrome: the effective rate of TCM syndrome in the experimental group was 93.33%,and that in the control group was75.86%.The difference between the two groups was statistically significant(P<0.05);(2)VAS score: after treatment,the VAS score of the two groups decreased(P<0.05).After treatment,the VAS score of the experimental group was better than that of the control group(P<0.05);(3)TCM syndrome score: after treatment,TCM syndrome score of the two groups were significantly improved(P<0.05).After treatment,the experimental group was better than the control group in improving TCM syndrome score(P<0.05);(4)laboratory indexes: after treatment,ESR and CRP of the two groups were lower than before(P<0.05).After treatment,the experimental group was better than the control group in reducing inflammation indexes(P<0.05).There was no significant difference in UA between the two groups(P>0.05);(5)inflammatory factors: after treatment,IL-1 β and TNF-α in the two groups were lower than before(P<0.05).After treatment,the experimental group was better than the control group in reducing cytokines(P<0.05).(6)safety analysis: there were 1 case of adverse reactions in the experimental group and 3 cases in the control group.There was no significant difference between the two groups(P>0.05).Conclusion Modified Tongfeng Decoction can effectively alleviate the joint pain symptoms of acute gouty arthritis of damp heat accumulation type.It can reduce the VAS score,improve the TCM syndrome score,and reduce the ESR,CRP and other inflammatory indicators,which may be related to Tongfeng decoction can inhibit the expression of IL-1 β,TNF-α in serum,and reduce the inflammatory cascade amplification effect related to aga.Moreover,there is no obvious adverse reaction in this prescription,and the clinical application is safe and effective.
